AbdominoPerineal Resection Of Rectum

Abdomino-perineal excision is a surgery for rectal cancer. Surgery is the main treatment for tumors of the bowel. Usually, the tumor and a length of normal bowel on either side of the tumor (as well as nearby lymph nodes) are removed. The healthy parts of the bowel are then sutured or stapled together (anastomosis).

In certain cases where it is not able to join the bowel back together, an opening (stoma) will be made on the outside of the body for waste to pass out of the body.

This is called a colostomy. A colostomy is made to allow waste to pass through an opening in the abdominal wall. Sometimes, a temporary colostomy is needed until the joined bowel has healed, and then it can be put back. This is done by further surgery. However, in some cases, the colostomy is permanent, which means it can never be put back, and there will always be an opening on the skin for bowel waste to pass through.

Symptoms

The basic symptoms associated are:

  • Large amounts of bloody leakage from the wound
  • Blood in the stool
  • Fever and chills
  • Pain that is not relieved by prescribed pain killers
  • Swollen abdomen

Causes

The exact cause of polyps is not well understood. Sometimes polyps are genetic. Polyps are more common as a person gets older. Diets high in fat and low in fiber are thought to play a role in causing polyps. Alcohol use, cigarette smoking, and obesity may also increase the risk of developing polyps.

BRIEF ABOUT THE PROCEDURE

The surgery will be done under general anesthesia, and the patient will be asleep through out the operation. An incision 12 to 15 inches long is made in the skin and muscle of the central lower part of the tummy wall. The lower bowel within reach is freed from its bed. Another incision is made around the back passage, which is also freed and the entire lower bowel is taken out. A fresh opening is made in the tummy wall for the remaining bowel which is made into a colostomy. The wounds are stitched up. You should plan to leave hospital about two weeks after the operation.

The recovery period after colon surgery varies. It usually involves a stay in the hospital from 6 -10 days in uncomplicated cases. A catheter (plastic tube) in the bladder is used to measure and drain your urine. After surgery you will be given intravenous fluids (a drip) through which antibiotics may be given. The drip will remain in place until you are able to drink enough fluids.

Your wound will have stitches and/ or staples and is usually covered with a dressing, which may be adhesive plaster or a spray-on plastic covering.
