<h2 class="intro">There is a reduction in new HIV infections among children and in AIDS-related deaths in India</h2>

<p>In this challenging moment when we are confronted with one of the biggest health emergencies in our history, the COVID-19 pandemic, we find our strength in the gains we are making despite this disruption.</p>
<h2>Steps forward</h2>
<p>The newly released 2019 HIV estimates by the National AIDS Control Organization (NACO)/Ministry of Health and Family Welfare with the technical support of UNAIDS tell us that there has been a 66.1% reduction in new HIV infections among children and a 65.3% reduction in AIDS-related deaths in India over a nine-year period. The number of pregnant women living with HIV has reduced from 31,000 in 2010 to 20,000 in 2019. Overall, antenatal coverage has expanded, and HIV testing has increased over time and within target range. Treatment coverage has also expanded.</p>

<p>Under the leadership of NACO, a ‘Fast-Tracking of EMTCT (elimination of mother-to-child transmission) strategy-cum-action plan’ was outlined by June 2019, in the run-up towards December 2020: the deadline to achieve EMTCT. The plan entailed mobilisation and reinforcement of all national, State and partners’ collective efforts — in a strategic manner, with district-level focus, and considering latest evidence — so that the States/Union Territories and the country as a whole achieve the EMTCT goal. Additionally, in March 2020, we began efforts to minimise challenges posed by the COVID-19 pandemic.</p>
<p>From 2010 to 2019, India made important progress in reducing the HIV impact on children through prevention of mother-to-child transmission of HIV. This was done through education and communication programmes; increased access to HIV services with innovative delivery mechanisms for HIV testing (community-based testing, partner testing or index testing); counselling and care; and treatment and follow-ups. India made HIV testing for all pregnant women free and HIV treatment is offered the same way nationwide without cost to pregnant mothers living with HIV through the national ‘treat all’ policy.</p>
<p>Cognisant of the challenge of diagnosing 20,000 pregnant women living with HIV in an estimated 30 million pregnancies annually in India, for two years UNICEF has worked with the World Health Organization and NACO to identify high burden districts (in terms of density of pregnant women living with HIV) as the last mile towards disease elimination.</p>
<p class="atd-ad">Since 2002, when the EMTCT of HIV programmes or prevention of parent-to-child transmission of HIV were launched in India, a series of policy, programmatic and implementation strategies were rolled out so that all pregnant women can access free HIV testing along with other services at antenatal clinics, and free treatment regimens for life to prevent HIV transmission from mothers to babies. This has been made possible in government health centres and grass-root level workers through village health and nutrition days and other grass-roots events under the National Health Mission.</p>
<p>Indeed, the approach being promoted by UNICEF in focusing attention and resources in high burden districts is supported by the HIV strategic information division of NACO and UNAIDS to better understand the locations and populations most HIV affected, so that technical support and HIV services can be directed towards these areas.</p>
<h2>Still a long way to go</h2>
<p>However, there remains a need for increased treatment saturation coverage and for early HIV testing and treatment initiation to become the normal. While periodic monitoring of the data and reviews are the mainstay of the programme response, by 2019 it was very evident to all the stakeholders that while there are successes, we have a long way to go towards the final targets.</p>
<p class="atd-ad">Using data-driven and decision-making approaches, we are certain that AIDS will no longer be a public health threat for children in India by the end 2030, if not before.</p>

<p>While the rate of increase of new HIV infections in India has been dropping over the last decade, it appears the National Aids Control Organisation’s (NACO’s) targeted interventions have been skipping over important vulnerable groups at higher risk of contracting HIV. Part of the problem appears to be that high-risk groups have been using apps, such as Grindr and Blued, and voice-calling services on their mobile phones to find sexual partners, and in the process slip off NACO’s radar.</p>
<p>The last available census of female sex workers and homosexual men happened in 2009-2011, and estimated the size of these two groups to be 11.58 lakh and 4.27 lakh people, respectively.</p>
<p>Maharashtra, Telangana, Karnataka and Andhra Pradesh are the top four states that receive NACO’s targeted interventions. Female sex workers in these states have some of the country’s highest prevalence of HIV. Similarly, Andhra Pradesh, Karnataka and Maharashtra, along with Gujarat, have the most number of identified homosexual men, and HIV prevalence among them is higher than the national average.</p>
<p>Since 2014, NACO claims to have conducted site revalidation exercises once every three, six or 12 months in areas where it implements its interventions, reexamines previously established hotspots, identifies new hotspots, and updates its data pertaining to high-risk groups. However, its own annual reports show that NACO hasn’t revised these groups’ population estimates since the 2009-2011 census.</p>
<p class="_yeti_done">According to Purnima Parmar, a team leader with the targeted interventions group at the Indian Health Actions Trust (IHAT), Bengaluru, “Plans for conducting a new estimation exercise are already under process in NACO.”</p>
<p><strong>Need for better mapping and estimation</strong></p>
<p>But as more conversations happen within apps designed (rightly) to protect people’s identities, more potential cases of HIV infection disappear from NACO’s radar, and more people are denied its targeted interventions. All together, these gaps increase the risk of HIV entering the bridge and then the general populations.</p>
<p>The 2009-2011 census was restricted to venue-based solicitation, such as at brothels and certain parts of the streets that female sex workers had been known to frequent. So unless the new exercise – the plans for which are already in the works, according to IHAT’s Parmar – is able to account for the rising popularity of mobile phones, it will underestimate the amount of work NACO has left to do, which in turn will postpone the day India can be HIV-free by years.</p>
<p>That said, NACO has been aware of this blindspot – as it indicated in its IBBS report – but hasn’t been doing enough to cover it. Multiple studies, both independent and sponsored, conducted since 2015 have reported the same gaps in its estimation process. A midterm review of NACP 4 published in 2016 recommended upgrading the methods, tools and guidelines used in population estimation and mapping exercises vis-à-vis high-risk groups.</p>
<p>In 2019, NACO published a whitepaper acknowledging that virtual spaces have emerged as a new risk environment for HIV transmission. The paper discussed one pilot density-mapping survey of homosexual men who used one, some or all of Grindr, Hornet, Scruff and Blued in Pune, Mumbai and Vijayawada. Based on its outcomes, NACO members have been deliberating the use of two methods to address the blindspot: a census-based mapping procedure and a capture-recapture method.</p>
<p>However, no one knows when and how the organisation plans to roll the procedure out nor how it will modify its targeted interventions. NACO had not responded to requests for comment at the time of publication.</p>

<p class="wp-block-paragraph">Endometriosis is that curse of a condition where women have to endure mind-numbing pain during their periods. Its symptoms include nausea, heavy bleeding, and unbearable pain. The Endometriosis Society of India estimates that 25 million women suffer from this condition in India alone and globally the number shoots up to 176 million women.</p>



<p class="wp-block-paragraph">Recently, to tackle this condition and enable women with mechanisms to deal with endometriosis, Federation of Obstetrics and Gynaecological Societies of India (FOGSI) along with Bayer Zydus Pharma launched key practice points for endometriosis – VISION (Valuable Insights in Indian Endometriosis – Redefining Outcomes) at the National Conference on Technology, Advances and Infections in Obstetrics and Gynaecology’ in Hyderabad.</p>



<p class="wp-block-paragraph">VISION aims to understand the current usage pattern of various therapeutic options in endometriosis and create an algorithm to guide doctors in the management of endometriosis based on different patient profiles.</p>



<p class="wp-block-paragraph">“The key practice points aim to engage the medical fraternity to be more cognitive of diseases such as endometriosis and provide patients with the right diagnosis and treatment options. Since management of endometriosis can be challenging for the patients and the treating doctors, VISION will enable us to execute a multi-disciplinary approach and expertise to improve patient’s quality of life. I urge all gynaecologists to refer to these practice points and ensure best treatment methods are followed,” FOGSI President Nandita Palshetkar said, TNIE reported.</p>



<p class="wp-block-paragraph">She added, “Girls and women can suffer for 5-10 years before they are diagnosed with endometriosis. Since pain during periods is common, most women are not rushed to the doctors even when the pain is unbearable. This disease also affects the fertility of women.”</p>



<p class="wp-block-paragraph">It came into place with help from over 200 thought leaders who came together formulated the protocols of practice on endometriosis after a national meeting in Mumbai followed by regional meetings in Ahmedabad, Delhi, Chennai, Kolkata earlier this year.</p>



<p class="wp-block-paragraph">Endometriosis affects women severely and in close to 30% cases it has led to women facing difficulty in getting pregnant. Dr Amita Shah of Columbia Asia Hospital, Gurugram, told ET, “It does not necessarily make one infertile but clearly constitutes a hindrance in getting pregnant naturally. The excess adhesion caused by tissue growth can confine the egg and stop it from moving down the fallopian tube which in turn prevents a natural pregnancy.”</p>



<p class="wp-block-paragraph">This condition affects women of all ages from early menstruation to way past menopause. The age of diagnosis is not predictive for the stage or severity of the disease. Experts say, TOI reports, that they have seen adolescent girls with severe, debilitating pain and advanced disease and also older women with severe pain caused by a superficial disease that had been missed by inexperienced surgeons during their previous laparoscopies.</p>
