The very thought of a surgery gone wrong is a nightmare in itself. A 32-year-old mom of three waded through a similar misery at the start of the New Year. Abigail Fernandez, who hails from Newquay, Cornwall signed up for a tummy tuck surgery in Tunisia, which almost cost her life.

Fernandez, who seemed to have body image issues post the delivery of her children, admitted to going under the knife to get back in shape. While most cosmetic surgeries are expensive, Fernandez came across a cheaper alternative via an advert. The procedure which costs about 10,000 euros in the UK, seems to have cost only 2,500 euros in Tunisia. Soon after, Fernandez called up the clinic which flooded her with information, including how she’d recover in a five-star hotel post-surgery. She wanted a breast lift as well but stuck to just the tummy lift owing to budget constraints.

But, much to her dismay, things seemed a little amiss right from the start. Far from what it looks like at a star hotel, the accommodation offered bore a dilapidated appearance—the tiles were broken, the floor was dirty and the bedsheets were dirty. And, the horror had just begun. The clinic, however, seemed spotless. She went ahead with the surgery which lasted for about four hours. When she woke up, she felt just fine. But, the next morning was far from what she imagined.

“My body felt like it was on fire and I begged nurses for painkillers,” said Fernandez, while recollecting the nightmare.

“They told me I was being discharged that day, I was horrified,” she added.  

The staff finally agreed to let her stay for one more day after Fernandez alleged that she was unable to move. Despite the pain having persisted the next day, Fernandez was discharged by the hospital staff the very next back and dropped back into the same hotel.

On her return, Fernandez remembers feeling dizzy and her vision was blurry. And well, her tummy had swollen to the size of a basketball. What unfolded in the next couple of days were plain ghastly. A green puss-like liquid oozed out of her tummy. While repeated attempts were made to bring this to the clinic staffs’ notice, they overlooked it and termed the condition as “normal.”

Unable to take it any longer, Fernandez was rushed to the doctor after being in acute pain. The excruciating pain was so bad, that she had to call for an ambulance.

At the hospital, doctors examined her condition only to discover that swelling was due to a hematoma – a clot of blood that had formed outside of Abigail’s blood vessels. The swelling was eventually busted and revealed that if she had left it longer, it could’ve cost Fernandez her life. The young mother was hospitalized for four weeks and the infection was cut out to stop it from spreading.

One in eight Australian women with endometriosis are turning to cannabis in a bid to relieve their chronic pain.

Researchers from NICM Health Research Institute, Western Sydney University, and UNSW Sydney surveyed 484 women on how they self-manage the symptoms of the debilitating condition. The results are published in the Journal of Obstetrics and Gynaecology Canada.

What they found was that more than three-quarters of women surveyed turned to strategies like yoga, dietary changes, heat packs, and exercise. The most effective strategy, however, was reported to be cannabis.

Researchers estimate that Endometriosis affects more than 1 in 10 Australian women. The chronic condition occurs when cells like those that line the uterus – the endometrium – are found outside the uterus.

During ovulation, the cells thicken. But unlike the cells that are found inside the uterus, these outside cells cannot leave the body. They bleed, cause inflammation and pain before they heal. Over time this process can lead to scar tissue.

The pain of endometriosis can be chronic and so severe that it stops women from going about their daily life. Symptoms include chronic pelvic pain, painful periods and infertility.

Cannabis does more than relieve pain

The participants reported that cannabis use reduced their pelvic pain, improved their sleep and decreased nausea and vomiting. They also reported that cannabis reduced their anxiety and depression.

43 per cent of the participants reported daily cannabis use, with 50 per cent of participants reporting that they smoked the plant-based medicine. A little under 12 per cent used edibles or oils.

Adverse effects associated with cannabis use were reported to be low, with 10 per cent of women reporting drowsiness, increased anxiety or tachycardia.

Lead author on the study Justin Sinclair from NICM Health Research Institute, says that in the past, cannabis has demonstrated effective pain-relief properties.

“Cannabis has a long history of use in the ancient and scientific literature for various conditions such as period pain, however until now nothing has been investigated for cannabis being used for endometriosis,” Sinclair says.

“Past research has demonstrated that certain compounds within cannabis known as cannabinoids exert analgesic and anti-inflammatory activity. Our research sought to determine the prevalence, tolerability, and self-reported effectiveness of cannabis in women with Endometriosis.”

For some, current treatments just don’t cut it

In Australia, there is a general lack of awareness surrounding endometriosis. Currently, there is no known cause and there is also no cure.

Endometriosis symptoms can be treated with drugs, most treatments are not suitable long term. The current treatments involve oral contraceptives or progestogens as well as non-steroidal anti-inflammatory drugs. However, sometimes these medications don’t always provide adequate pain control. Sometimes, there are side effects.

Surgery can also be performed that removes the endometriosis lesions and scar tissue. In severe cases, a Hysterectomy may be performed.

However, the researchers found in 56 per cent of participants who reported using cannabis, they also reported a significant reduction in the use of pharmaceutical medications.

“Clinical trials of cannabis use in this group are warranted to determine efficacy, compliance, and side effect profile,” the researchers write.

Most women are using illicit cannabis

Currently, Australian law requires legal medicinal cannabis use to follow regulated pathways. However, researchers say that few prescriptions for medicinal cannabis have been dispensed. This has led Australian women to turn to illicit use of cannabis.

“Due to the timing of when the survey was administered, most, if not all of the women in the survey would have been using illicit cannabis as access to medicinal cannabis was still in its infancy,” says Chief Investigator on the study, Mike Armour from NICM Health Research Institute.

“This means we don’t have any information about the different varieties of cannabis that women were using, or what might have been in the cannabis that was being used as it was derived from illicit sources which are not quality assured,” he says.

“Further research is needed to assess the effectiveness of quality-controlled medicinal cannabis and women with endometriosis.”

Tipping the scales at 20 stone, she started to notice her weight was getting in the way of life.

Sarah, 29, from Brisbane, Australia, was given a wake-up call when she had her second daughter – and she began to struggle to keep up.

Determined to do something about her size, Sarah overhauled her lifestyle and underwent sleeve gastrectomy surgery, to reduce the size of her stomach.

The mum-of-two has since dropped more than nine stone and is happier than ever in her trim 11 stone figure.

She said: “I have never been happier. I haven’t just lost weight, I’ve gained a new life.

“One that isn’t ruled by weight restrictions.”

Sarah gained more than seven stone over the course of six years after constantly eating out with her partner and enjoying midnight feasts.

She said: “My weight crept up between 2012 and 2018.

“My partner and I used to enjoy going out and eating together. It was one of our passions.

“At my heaviest I was bingeing on one litre of energy drink a day, eating at least one take away meal every single day and could easily eat a block of chocolate in one sitting.

“I used to also wake up during the night to snack on chocolate or chips.”

After her second daughter was born, Sarah decided she wanted to be more active for her children but her weight got in the way.

Her older sister underwent a gastric sleeve surgery and lost nine stone, and seeing her results, Sarah was encouraged her to do the same.

She went for a vertical sleeve gastrectomy in April last year, removing a large portion of her stomach which helped her feel full even after eating small amounts of food.

She was put on an 800 calorie diet which included high protein and low carbs, causing her to lose more than nine stone.

Sarah said: “I had the vertical sleeve gastrectomy (VSG or Gastric Sleeve).

“There were no complications. I stayed in hospital for three nights after my operation.

“To lose weight I ate an 800 calorie a day diet that was high protein and low-carb.”

Sarah is now charting her weight loss transformation on her Instagram page.

And she says she wants to be open and honest about the effects of weight loss surgery.

She said: “I was inspired by other weight loss accounts and wanted to be open and honest about the effects of weight loss surgery.

“The support has been fantastic!

“I have met some amazing people through the weight loss surgery community.”

Since losing the weight, Sarah said she never felt happier and now lives her life to the fullest.

She added: “I feel fantastic. I have so much energy to play with my daughters and nieces and nephews.

“I used to sweat a lot on my face and was so embarrassed by it. I don’t have that problem at all now.”

Sarah has two children aged 5 and 2 with her partner, Chris, and has been with him for seven years.

At first, he was apprehensive of surgery at the start but became a pillar of support for Sarah throughout her weight loss journey.

She added: “He always has said I was beautiful no matter my size. Our relationship hasn’t changed at all.”

Sarah now hopes to maintain her current weight and doesn’t plan to have any further surgery.

She said: “I haven’t had any skin removal surgeries and don’t plan to in the future.

“I would have a tummy tuck if the loose skin was uncomfortable, but at this stage it’s fine.

“I weigh myself every morning and try and maintain my current weight.

“I’ve been in maintenance since July 2019.”

When it comes to losing weight, Sarah has advised others to work hard in their goals in order to achieve their desired results, saying there is no “quick fix” even with surgery involved.

She said: “It’s not a quick fix. You need to try hard every day to be successful.

“But it is life changing and the best thing I have ever done for myself.

“It wasn’t out of self loathing.

“I loved myself enough to give myself a second chance.”

Mikayla Whitworth says she began vomiting and was diagnosed with sepsis and the life-threatening flesh-eating disease necrotising fasciitis after returning to the UK after the procedure in 2015.

The 30-year-old says that upon her return, doctors were forced to cut out a 2.4in-deep, foot-wide slab of infected flesh from her stomach, MailOnline reported.

She claims that the horrific ordeal caused the breakdown of her marriage and even left her unable to play with her young son properly.

Mikayla from Ashington, Northumberland, also fears it may mean she cannot have more children.

She has begun a £100,000 legal action against the Czech firm, Perfect Clinic, and Prague Beauty, the travel agency in Mayfair, west London, where she booked the all-inclusive trip.

She decided to fork out £3,250 for a tummy tuck, liposuction, flights and accommodation after ‘ballooning’ to a size 20 after her pregnancy.

After trying ‘every weight loss program she could’, Mikayla decided to opt for cosmetic surgery.

Unable to afford the prices advertised by private clinics in the UK, she decided to travel to Prague because it offered ‘the right price, with the best reviews’.

Had she paid for a similar procedure in the UK, it would’ve cost around £10,000.

In court documents filed at Central London County Court, Mrs Whitworth claims she was told the quality of surgery and care at the clinic were ‘the same or better’ than in Britain.

However, she began to develop a fever, headache, a rash and a swollen stomach following the operation.

She says that she raised her fears about the wound being infected with Czech medics, but was dismissed and simply told to ‘eat biscuits and drink sweet tea’.

As her symptoms gradually worsened, she says she contacted the clinic once more, but received no reply.

After her return to the UK, Mikayla had to spend 6 weeks in hospital.

The mother of one says she has had to undergo four reconstructive operations and also suffers from nightmares and panic attacks.

Roman Kufa, owner of Perfect Clinic, told the Mail it was ‘normal’ for complications to happen after cosmetic surgery, however he went on to add that they affected only a very small percentage of patients.

He also said that it was more likely that Mikayla’s symptoms were a result of her failure to follow the post-surgery care plan.

Dr Kufa said: ‘I do 800 operations to 900 per year. Some people, of course, they have complications – it’s normal. If they don’t do everything correctly after the operation, it’s a very big problem.

‘If complications happen our clinic offers everything free.’

The agency and the clinic have been given more time by the court to formally respond to the legal claim.

Diabetes patients take insulin to get blood sugar, or glucose, into their cells. However, insulin can make people feel hungry, making it even harder to lose weight.

Costly treatment

Bariatric surgery is costly. In the United States, insurance companies will not always pay for the surgery. They require a patient’s body mass index (BMI) to be at least 35, even if a patient’s diabetes is uncontrolled. BMI is the measure of body fat based on the person’s height and weight.

Stacy Brethauer is a surgeon at The Ohio State University Wexner Medical Center in Columbus, Ohio. He says the BMI number does not always show how a patient is reacting to treatment for diabetes.

“BMI is not really a fair …representation of who should be getting therapy,” he said.

Brethauer added that the rules for who gets bariatric surgery need to be changed.

“The patient who doesn’t get the operation, we know very well that their disease will progress, their lifespan will be shortened if they don’t get effective treatment,” he said.

The American Medical Association identifies obesity as a complex, chronic disease that requires medical attention. If it is not treated, obesity can lead to heart disease, cancer and diabetes. Brethauer believes early treatment is necessary for both obesity and diabetes.

Double standard?

“No one asks somebody with cancer or heart disease to ‘just do it themselves…’ but there’s also effective therapy being offered to those patients at the same time,” he said.

Since he had surgery to reduce the size of his stomach, Shaun Rogers has lost a lot of weight. His diabetes is also under control. Rogers no longer needs oral medication. And he has cut down on the use of insulin, going from nearly 500 units a day down to about 10.

“It’s changed my life so much,” he said, “I would tell anyone do it.”

Diabetes is a health issue worldwide. More than 400 million people have the disease. That number represents one in 11 adults. The World Health Organization estimates that number will rise sharply in the near future.

If people with diabetes have bariatric surgery early on, their diabetes can be cured. For those who have had the disease for 10 years or more, and need insulin, bariatric surgery will not cure their diabetes. But as Stacy Brethauer notes, it can greatly improve their health.
