<p>BIRMINGHAM, Ala. (AP/WIAT) — Alabama’s largest health care unit for treating HIV patients has relocated to a larger location.</p>
<p>The 1917 Clinic at the University of Alabama at Birmingham has moved from its old home in downtown to a new site in Birmingham’s Lakeview district. The clinic has more than doubled in size to 27 examination rooms. It also has added a second room for medical procedures and new space for counseling services.</p>
<p>“This move will allow for increased capacity to meet the existing and burgeoning need for HIV comprehensive multidisciplinary care and support service to people with HIV and the community,” said clinic director James Raper, Ph.D., CRNP. “It also allows us to more effectively do outreach to identify persons with HIV who are not engaged in care, and simultaneously do outreach and provide PrEP services to people at risk for HIV.”</p>
<p>With more than 3,600 active patients, clinic director James Raper says the expansion will help provide comprehensive care for people with HIV, which can cause AIDS.</p>
<p>While the 1917 Clinic occupies the majority of the Dewberry Building, Birmingham AIDS Outreach has space in the building as well, which is indicative of 1917’s mission to work with community partners and agencies to support all patient needs.</p>
<p>“This move will provide renewed synergy between 1917 Clinic and BAO as community partners in the fight to end the HIV epidemic while addressing the immediate needs of the HIV community,” Raper said. “I am excited about the decades of HIV patient care ahead of us in this new space.”</p>
<p>The 1917 Clinic has treated more than 12,000 patients with HIV in its three-decade tenure.</p>

<p>I mean, sometimes they just want to delay bedtime, but sometimes it&#8217;s really a health concern.</p>
<p>I have never been a good sleeper. If you ask my mother, this started the day I was born. I didn&#8217;t sleep through the night for the early years of my life, and as a child, I was routinely up very late. This all sounds par for the course — &#8220;My kids never sleep!&#8221; is a frequent parenting snafu — but are some kids just bad sleepers, or can children have insomnia?</p>
<p>As it turns out, children can and <em>do</em> get insomnia, just like adults, says Yale pediatric sleep specialist and author of <em>It&#8217;s Never Too Late To Sleep Train</em><em> </em>Dr. Craig Canapari. He tells Romper that infants, children, and teens can have insomnia, and according to the International Classification of Sleep Disorders, there are two parts to insomnia, regardless of age. &#8220;The first is difficulty with sleep, whether with difficulty with falling asleep, staying asleep, getting up too early in the morning, resistance to bedtime, or needing a parent to fall asleep,&#8221; Canapari says. &#8220;The latter two are quite common in young children.&#8221;</p>
<p>The second part of &#8220;oh yes, this is insomnia&#8221; is that the trouble your child is having at bedtime is causing problems with themselves or their family. And that gets to be quite a lot.</p>
<p>But Canapari says that parents <em>might</em> also be setting expectations too high when it comes to the amount of sleep children need. &#8220;Your 2-year-old may sleep 12 hours a night and take a two-hour nap every day. But if you expect your 5-year-old to sleep 24 hours a day in a 24-hour period, you will be disappointed.&#8221;</p>
<p>My son would take two two-hour naps each day, but at night would only sleep 10 hours. My daughter would take a single two-hour nap during the day, but she&#8217;d sleep a solid 13 hours at night. I didn&#8217;t think anything of it until I realized that my daughter was never going to sleep past 5 a.m. (And is still, at age 9, a very early riser.)</p>
<p>But it wasn&#8217;t insomnia for either of them. For me as a child, it definitely was. Canapari says that the first sign of insomnia is fighting at bedtime, and holy crap we&#8217;ve all been there, but amp that up for a kid with insomnia. &#8220;Prolonged periods between lights out and falling asleep,&#8221; can also be a sign, but night time awakenings after 6 to 9 months of age are not uncommon.</p>
<p>Thankfully, there are resources available and steps parents can take to help their children when they&#8217;re suffering. &#8220;The first step is checking with your pediatrician to see if your child’s sleep patterns are normal for ages, and making sure there is no medical problem causing these issues,&#8221; Canapari says. After that, he says to parents, &#8220;You owe it to yourself to try some behavioral techniques to help her — and you — sleep better.&#8221; It&#8217;s a rough time, so strategizing is critical. &#8220;Remember, your child may be able to make up lost sleep in the car seat, at day care, etc. But you can’t be the parent you want to be if you are exhausted.&#8221;</p>
<p>Boston Children&#8217;s Hospital also noted that your child&#8217;s insomnia can stem from other sleep disorders (like obstructive sleep apnea or restless legs syndrome); anxiety or stress; a mental health or developmental condition like autism, depression, ADHD, or asthma; certain medications (like antidepressants or steroids); and even caffeine. If you can find something here that resonates with you and your child, it&#8217;s worth seeing if removing that factor will help. Less caffeine during the day, finding out what causes your child stress, etc.</p>
<p>Above all, give yourself some grace. You&#8217;re tired, too, and while you can classify some children as &#8220;night owls,&#8221; it&#8217;s best if they get adequate sleep. Talk to your child&#8217;s providers, and make a plan. It might take a while, but there is hope.</p>

<p>A raft of new reports have been issued about obesity and ill-health effects. Included within these reports are on-going concerns about obese people and a greater chance of developing more severe COVID-19 symptoms.<br /><br /><br /></p>
<p>Obesity continues to a major problem worldwide and it is associated with a range of metabolic disorders. Definitions of obesity vary, although it is generally accepted that obesity refers to the condition where excess body fat accumulates to an extent that it may have a negative effect on health.</p>

<p>There are different causes of obesity and the condition is not only linked to poor diet and insufficient exercise (such as eating excessive amounts of cheap high-calorie food and spending a lot of time sitting down at desk), for there are genetic issues to consider as well.<br /><br /><br /></p>
<p>According to the University of Virginia there are differences in fat storage and formation between men and women, and this is influenced by different genes found in fat tissue. Furthermore, some of those genes identified are connected with conditions like type 2 diabetes and cardiovascular disease.</p>

<p>The reason for highlighting the genetic basis is to indicate that the causes of obesity and strategies of address obesity are more complicated than sometimes portrayed. <p>This analysis shows that school-aged children&#8217;s&#8217; height and weight, serving as indicators of their health and quality of their diet, vary enormously around the world. The analysis reveals a 20 centimeter difference between 19-year-olds in the tallest and shortest nations. The key variable is with childhood nutrition, including a lack of quality food. This important factor is associated with stunted growth and a rise in childhood obesity.</p>

<p>It also follows that the lifestyle of women with obesity during pregnancy can lead to long-term cardiovascular benefits for their children. If exercise is not regularly performed, the reverse can occur and children from obese mothers carry a risk of being less healthy and also becoming prone to developing obesity themselves.</p>

<p>The findings appear in the medical journal <em>The Lancet</em>, titled “Height and body-mass index trajectories of school-aged children and adolescents from 1985 to 2019 in 200 countries and territories: a pooled analysis of 2181 population-based studies with 65 million participants.”</p>

<p>At the other end of the age scale, severely overweight people are less likely to be able to re-wire their brains and find new neural pathways. This is something of importance in relation to stroke recovery. This finding appears in the journal <em>Brain Science</em> (“Obesity is Associated with Reduced Plasticity of the Human Motor Cortex”).</p>

<p><strong>Too much sugar</strong></p>

<p>A related area of research is linked to sugar in the diet. A study, looking at an animal model, has discovered that mice fed diets high in sugar developed worse forms of colitis than mice fed a standard diet.</p>

<p>Colitis is a form of inflammatory bowel disease. To explore the effects further, the researchers from UT Southwestern Medical Center examined the large intestines of the mice and discovered more of the bacteria that can damage the gut&#8217;s protective mucus layer to be present.<br /><br /><br /></p>
<p>Importantly, the study pointed to sugar, such as the type of glucose found in high fructose corn syrup (used by parts of the food industry since the 1960s0 as the mist significant contributor to the effects.</p>

<p>The researchers have indicated that effect seen with mice can be extrapolated to humans, and this should act as some form of warning for a diet too rich in sugar.</p>

<p><strong>Obesity and coronavirus risk</strong></p>

<p>An individual’s health is important when considering coronavirus risk. A new study finds that factors inherent to obesity could increase vulnerability to COVID-19. Correlations in many countries exist between obesity and COVID-19 deaths.</p>

<p>In particular, inflammation in the lungs, when combined with high viral loads of the coronavirus, appears to create ‘a perfect storm’ for obese patients with COVID-19. Fat has high amounts of ACE2 receptors. These receptors serve as the entryways for the SARS-CoV-2 virus into the cells of the lungs.<br /><br /><br /></p>
<p>Another concern is with the higher overall inflammatory state that accompanies obesity, and which primes many tissues to show a poor response to infection.</p>

<p>The outcome from the research is that medications used to lower inflammation in the lungs are beneficial to obese patients who have coronavirus. This may lead to a new medical intervention.</p>

<p>Prime Minister Yoshihide Suga&#8217;s pledge to boost fertility treatment support has offered rays of hope to couples longing for babies, but experts say the policy will not be a panacea for reversing the nation&#8217;s declining birth rate.</p>
<p>As Japan, with the world&#8217;s oldest population, struggles with dwindling numbers of newborns, Suga has vowed to make often costly fertility treatment eligible for coverage by national health insurance, with government officials eyeing the change as early as 2022. In the run-up, subsidies for couples undergoing such treatment will be sharply increased.</p>
<p>Many couples who have undergone years of fertility treatment at the cost of millions of yen were encouraged by the move, but some experts say it may not be enough to boost the birth rate at a time when it is under fresh strain as the coronavirus pandemic darkens the economic prospects of working-age people.</p>
<p>Japan&#8217;s total fertility rate &#8212; the average number of children born per woman during their reproductive years &#8212; stood at 1.36 in 2019 with a record-low 865,234 babies born that year. The government aims to raise the rate to 1.8.</p>
<p>A 37-year-old Tokyo woman, who gave birth to a child after spending 2 million yen on four years of fertility treatment since she was 30, said she was &#8220;heartened&#8221; by Suga&#8217;s push for expanded support.</p>
<p>The woman, who changed clinics twice in the pursuit of better treatment, expressed hope that the public insurance coverage &#8220;will help improve the quality of medical care&#8221; provided at fertility clinics because their technology and equipment differ.</p>
<p>She also said she had not been correctly informed about rates of successful treatment at fertility clinics. Currently, as they operate outside insurance coverage, there are no standards covering their disclosure of clinical outcomes.</p>
<p>Another woman, a 35-year-old resident in an area north of Tokyo, also welcomed the new premier&#8217;s initiative after spending 8.5 million yen on six years of fertility treatment. But she expressed concern that &#8220;women could be regarded as &#8216;child-bearing machines&#8217; if the policy is only targeted at reversing the declining birth rate.&#8221;</p>
<p>At present, most of the fertility treatment in Japan, except for the initial phase, such as tests to look into infertility and provision of ovulation-inducing drugs for timed intercourse, is not covered by the public health insurance, under which patients shoulder 30 percent of costs.</p>
<p>More advanced methods such as in vitro fertilization, where eggs are collected from ovaries and fertilized by sperm in a lab, and microinjection, which resembles regular IVF but in which a single sperm is injected into an egg, can cost several hundred thousand yen per cycle.</p>
<p>Even though public subsidies totaling 1.05 million yen are available for couples for up to six IVF cycles, there is an age limit for women, currently set at 43. There is also a cap on a couple&#8217;s combined annual income, set at 7.3 million yen in most municipalities.</p>
<p>&#8220;It&#8217;s easy for couples comprising two full-time workers to exceed the annual income threshold,&#8221; said Ran Kawai, a journalist specialized in childbirth issues. Furthermore, many recipients of the subsidies cannot cover the total costs despite the assistance, she added.</p>
<p>Under Suga&#8217;s initiative, the health ministry is considering scrapping the annual income cap as well as relaxing restrictions on the number of IVF cycles and boosting the amount of subsidies from next April, ministry officials said.</p>
<p>In countries such as France, Germany, Belgium and Israel, fertility treatment is covered by public health insurance to some degree, mostly with limits on the number of cycles and women&#8217;s age, according to their authorities and data compiled by the NLI Research Institute.</p>
<p>But Japan has not opted for insurance coverage, with infertility not defined as a disease, despite a record 56,979 babies born in 2018 via 454,893 IVF cycles, or one in 16 newborns that year for the highest rate ever. In 2015, 18.2 percent, or one in every 5.5 couples, underwent infertility tests or fertility treatment.</p>
<p>The number of IVF cycles conducted in Japan is the largest among major countries, according to the International Committee for Monitoring Assisted Reproductive Technologies.</p>
<p>Akiko Matsumoto, head of nonprofit group Fine that supports those suffering from infertility, said momentum for seeking expanded assistance for them grew among Japanese political circles with then-Chief Cabinet Secretary Suga receiving a petition from ruling party members on the matter in June.</p>
<p>&#8220;When we organized a study session in parliament on fertility treatment in January, about 100 participants came, which was much larger than expected, and some of them enthusiastically discussed the matter,&#8221; she said.</p>
<p>&#8220;It was encouraging to see the issue has gained public awareness, with some talking about their own fertility treatment experiences or someone close to them,&#8221; Matsumoto said.</p>
<p>Among the lawmakers of the ruling Liberal Democratic Party pushing for insurance coverage of fertility treatment and other support is Seiko Noda, currently the party&#8217;s executive acting secretary general, who gave birth to a boy at the age 50 in 2011 through IVF using a donated egg in the United States.</p>
<p>The major opposition Constitutional Democratic Party of Japan similarly submitted a petition to the health ministry in February, calling for insurance coverage and other patient support. CDPJ leader Yukio Edano&#8217;s twin sons were born in 2006 after four years of fertility treatment.</p>
<p>The CDPJ also stressed that expanded public support for fertility treatment should not lead to increased pressure on couples to bear a child.</p>
<p>Both Kawai and Matsumoto said they hope lowering the economic burden through insurance coverage leads more young couples to start advanced fertility treatment early, as their success rate is higher compared with that for older couples, possibly shortening the treatment period.</p>
<p>According to 2018 data from the Japan Society of Obstetrics and Gynecology, the rate of childbirth per total IVF cycles stood at 21.6 percent for women aged 30, but fell to 9.5 percent for those aged 40.</p>
<p>&#8220;About two-thirds of advanced fertility treatment patients in Japan are aged 35 or older. If the government can boost financial assistance to people through the universal insurance coverage, they can feel free to start receiving it any time,&#8221; Kawai said.</p>
<p>The journalist also argued it is unreasonable that IVF treatment for those with such problems as tubal obstruction and lack of sperm in semen has not been covered by health insurance.</p>
<p>A Fine online survey conducted for four months through January 2019, which drew responses from some 1,500 people who have undergone fertility treatment, found the largest group of 24 percent shouldered costs of from 1 million yen to less than 2 million yen.</p>
<p>More than half of the respondents said they have become hesitant about, postponed or given up receiving advanced treatment due to economic burdens, with percentages high among those under 35.</p>
<p>The health ministry is set to conduct by next March a survey on some 600 IVF clinics and the general public in Japan on fertility treatment costs. Since the amount of the current subsidies is based on the ministry&#8217;s fiscal 1998 survey, Matsumoto said such research is &#8220;long overdue.&#8221;</p>
<p>Kawai said boosting support for fertility treatment should be seen as only one of several measures needed to help reverse the declining birth rate as Japanese women&#8217;s lifestyles diversify, with many marrying later in life.</p>
<p>She suggested that another factor stemming from the coronavirus pandemic that may affect the birth rate besides the expected drag from the economic impact is that many young people may be missing opportunities to go out and find partners.</p>
<p>&#8220;The number of newborns is expected to sharply drop this year,&#8221; she said.</p>

<p class="text | article-text">LINCOLN, Neb. (KOLN) &#8211; National Health Statistics show that ear, nose and throat disorders remain some of the top reasons kids visit the doctors. For one Lincoln mom, she and her 4-year-old daughter know this far too well.</p>
<p class="text | article-text">Emorie Quakenbush is just like any other 4-year-old girl. She loves watching “Frozen” and “Peppa Pig” and eating chicken nuggets. What’s separating her from the rest is that she’s already undergone five surgeries. Hopefully her surgery in October of this year will be her last.</p>
<p class="text | article-text">When Hailee Quakenbush first had Emorie, she was born as a healthy little girl at six pounds, five ounces and no complications.</p>
<p class="text | article-text">It was only months after that when her mom started noticing a few things. Emorie began pulling at her ears, wouldn’t eat, couldn’t sleep and always had red, watery eyes.</p>
<p class="text | article-text">After countless doctor’s visits, being told Emorie just had ear infections and antibiotic after antibiotic, nothing seemed to work. That was until Emorie was finally referred to an otolaryngologist, commonly known as an ENT.</p>
<p class="text | article-text">At two and a half years old, Emorie had tubes put in her ears multiple times and had her tonsils taken out. By the time she was three years old, she had undergone four different surgeries.</p>
<p class="text | article-text">Emorie’s mom says it’s hard, but she knows it’s all to help her daughter. “After a surgery and a few days, she’s back to my daughter. She’s happy, and she wants to play. She’s eating, and I’m not wiping snot off of her nose every five minutes,” said Quakenbush.</p>
<p class="text | article-text">From bulging ear drums, to excessive drainage, Emorie’s ENT says it’s a good thing her mom got her in when she did.</p>
<p class="text | article-text">Sleepless nights and surgery after surgery have been challenges for this mother-daughter duo, but Quakenbush says there’s one thing that keeps her going. “Faith. God has been a huge part of helping me and working me through this. I just pray, and I’m like, ‘Please give me the strength.’ We got this. We’re going to do this,” Quakenbush told 10/11 NOW.</p>
<p class="text | article-text">As Quakenbush did, Lincoln ENT, Dr. Aaron Robinson at CHI Health ENT Nebraska suggests parents get on top of these issues early on. It may help your child have a more normal development and may avoid future possible issues or surgeries.</p>
<p class="text | article-text">You may have seen your child experience some issues similar to Emorie, such as hearing issues, chronic ear infections, even snoring. Dr. Robinson says another thing to look out for, especially in younger children, is speech delay. In these cases, he says see a professional as soon as you can.</p>
<p class="text | article-text">Like in Emorie’s case, Dr. Robinson says the best options for kids experiencing ear, nose and throat issues are antibiotics and in worse cases, it’s surgery.</p>
<p class="text | article-text">Before these become options Dr. Robinson feels like your best bet to avoiding complications in your child is to get referred to a specialist as soon as you can.</p>
<p class="text | article-text">Children with these chronic issues can sometimes develop behavioral issues like having a tough time in school or even wetting the bed at older ages. Dr. Robinson tells 10/11 that’s why getting an appointment for your child in these cases is so important.</p>

<p><strong>Kampala, Uganda | URN |</strong> There is a rise in HIV infection among older men according to health experts. Information from Uganda Aids Commission (UAC) shows that majority of men between 45 and 49 years of age who have been seeking for health services in the past years tested positive.</p>
<p>This according to health experts is worrying and likely to see an increase of the disease burden among the already 1.2 million adults out of the 1.4 million people living with HIV/AIDS in the country. Victor Rwengabo the Uganda Aids Commission zonal coordinator for Gulu notes that their finding indicates older men between 45 and 49 years of age are contracting HIV/AIDS compared to young men.</p>
<p>Young people (between 15-24 years) are responsible for 34% of new HIV infections annually. He notes that although they are yet to get concrete reasons for the new trend, he says the key factors among them is financial stability, long travels out of home and high sexual libido among men between 45 and 49 years of age.</p>

<p>According to Rwengabo, this is directly contributing to the high HIV/AIDS burden among young girls who target these men adding that it’s the reason the presidential fast-track initiative is focusing on men.</p>
<p>Dr. Hilary Alima, an HIV/AIDS specialist says men still have low health seeking behaviors in regards to understanding their HIV/AIDS status which he said also applies to young men. He however noted that the high HIV prevalence among older persons is perturbing saying it begs answers from the target group.</p>
<p>“The men, the boys and children are a gap and are not testing. We have actually found out of late that older men above 49 years-old are beginning to get HIV/AIDS infection. The data is very clear. We are trying to understand what could be the driving factor,” Dr. Halima said.</p>
<p>He says the index and partner testing where a person who has been diagnosed with HIV brings or identifies their sexual partner for voluntary HIV/Aids testing is positively helping in the fight against the scourge in the</p>
<p>He however says that although such an initiative is helpful in ensuring that men of corporate class don’t miss out treatment, their efforts to scale it up are hampered by limited finance and stringent donor fund directives. Early this year, Kitgum district health officials revealed an increase in HIV/AIDS cases within Namokora, Orom and Lagoro sub counties, which stood at 3.8 per cent above the district rate of 3.4 per cent mostly among boys and men of 15-49 years old.</p>
<p>Kitgum district HIV/AIDS prevalence rate stands at 7.3 per cent, highest in Acholi sub region where some nearly 60,000 people are living with disease. Uganda still registers 1,000 new infections and 500 deaths every week, according to the Presidential Fast -Track Initiative on Ending HIV and AIDS in Uganda fact sheet released last year.</p>

<p>Dr. Michael Ochwo, an expert on HIV/Aids at The Aids Support Organization (TASO), says the high prevalence mostly among adults has now been compounded with the low response for treatment from corporates. He says many are dying silently fearing to be stigmatised while accessing some of the health facilities providing the life prolonging antiretroviral therapy while others are now purchasing drugs from Kampala or sharing with their spouses.</p>
<p>Dr. Ochwo explains that they launched an initiative last year that provides exclusive services to the corporate class in their comfort outside the normal times of treatment.</p>
<p>He says currently 64 corporate clients are receiving treatment through this arrangement which wouldn’t have been possible. At least 8,706 clients are currently enrolled on ART at Gulu TASO center. This is followed by Gulu Regional Referral Hospital with 6,000 clients and St. Mary’s hospital Lacor with 4,500 clients.</p>
