Alcoholic liver disease (ALD) is a spectrum of progressive liver injury caused by chronic excessive alcohol consumption, ranging from reversible fatty change to irreversible cirrhosis. It occurs in three main stages: alcoholic fatty liver (hepatic steatosis) — seen in up to 90% of heavy drinkers, where fat accumulates within hepatocytes due to impaired fatty acid oxidation; alcoholic hepatitis — acute inflammatory liver injury occurring in 10-35% of heavy drinkers, ranging from mild to life-threatening (severe alcoholic hepatitis has 28-day mortality up to 50%); and alcoholic cirrhosis — irreversible fibrotic replacement of normal liver architecture occurring in 10-20% of heavy drinkers after 10-20 years of heavy intake. Cirrhosis from ALD causes portal hypertension and its complications (variceal bleeding, ascites, encephalopathy) and significantly increases hepatocellular carcinoma (HCC) risk. Causes & Risk Factors

Ethanol is metabolised to acetaldehyde by alcohol dehydrogenase (ADH) in the liver — acetaldehyde is directly hepatotoxic, forming protein adducts, impairing mitochondrial function, increasing oxidative stress (via reactive oxygen species from CYP2E1 induction), and activating inflammatory pathways (TNF-alpha, IL-1, IL-8 from activated Kupffer cells and recruited neutrophils). Risk factors: female sex (women develop ALD at lower intake thresholds due to lower first-pass gastric ADH activity, smaller body water volume, and possible oestrogen effects on gut permeability); genetic polymorphisms in ADH1B, ALDH2, PNPLA3 (I148M variant is the strongest genetic risk factor for fibrosis); obesity and metabolic syndrome (synergistic hepatic injury with alcohol); concurrent hepatitis C (dramatically accelerates fibrosis — HCV-ALD cirrhosis occurs 10 years earlier than either condition alone); chronic hepatitis B; poor nutritional status (protein-calorie malnutrition and zinc deficiency impair hepatic regeneration); and pattern of drinking (binge drinking is particularly injurious).

Symptoms & Signs

Symptoms depend on the stage. Alcoholic fatty liver (steatosis) is typically asymptomatic or causes mild right upper quadrant discomfort; incidentally discovered as hepatomegaly or echogenic liver on ultrasound. Alcoholic hepatitis presents acutely or subacutely with jaundice (yellow skin and eyes), right upper abdominal pain and tenderness, fever, nausea, vomiting, anorexia, and malaise. Severe alcoholic hepatitis causes liver failure with coagulopathy (elevated INR causing bleeding), hepatic encephalopathy (confusion, asterixis, stupor), renal failure (hepatorenal syndrome), and a Maddrey discriminant function (DF) score above 32 — indicating high short-term mortality. Alcoholic cirrhosis presents with the consequences of portal hypertension and reduced hepatic synthetic function: fatigue and weakness; jaundice (scleral icterus); spider angiomas (vascular telangiectasias on the upper trunk); palmar erythema; Dupuytren's contracture; bilateral parotid enlargement; gynaecomastia and testicular atrophy (from impaired oestrogen metabolism); abdominal distension from ascites (fluid accumulation in the peritoneal cavity) with a fluid thrill and shifting dullness; bilateral ankle and leg oedema; easy bruising and bleeding (thrombocytopenia from hypersplenism, coagulopathy); and hepatic encephalopathy — progressive confusion, personality change, asterixis (liver flap), and, if severe, stupor and coma from ammonia accumulation.

How It Is Diagnosed

Liver function tests (LFTs) in ALD show elevated AST and ALT — the classic AST:ALT ratio above 2:1 (often 2-3:1) is characteristic of ALD and distinguishes it from viral hepatitis or NAFLD; GGT is disproportionately elevated (sensitive marker of chronic alcohol use and microsomal enzyme induction); elevated total bilirubin and alkaline phosphatase in alcoholic hepatitis. Full blood count shows macrocytic anaemia (from folate deficiency, alcohol toxicity to erythropoiesis, and haemolysis), thrombocytopenia (from hypersplenism and bone marrow suppression), and elevated white cell count in alcoholic hepatitis. Serum albumin and prothrombin time/INR assess synthetic liver function — low albumin and elevated INR indicate significant hepatic dysfunction. Ultrasound scan detects bright echogenic liver (steatosis), hepatomegaly, splenomegaly (portal hypertension), portal vein diameter, varices, ascites, and liver nodularity suggesting cirrhosis. Fibroscan (transient elastography) non-invasively measures liver stiffness, correlating with fibrosis stage, often replacing liver biopsy in clinical practice. Liver biopsy — performed transvenous (via jugular vein) in coagulopathic patients — confirms stage (steatosis, hepatitis, fibrosis grade F0-F4, cirrhosis) and provides prognostic information. Prognostic scoring: Maddrey discriminant function (DF) = 4.6 × (PT-control) + bilirubin (μmol/L)/17.1 — DF above 32 predicts high 28-day mortality (20-30%) and indicates corticosteroid eligibility. Model for End-Stage Liver Disease (MELD) score and Glasgow Alcoholic Hepatitis Score (GAHS) also guide severity and treatment.

Treatment Options

Complete and sustained alcohol abstinence is the single most important treatment at all stages — fatty liver resolves within weeks, early fibrosis can regress significantly, and even in cirrhosis, abstinence slows progression and improves survival. Medical management of alcohol dependence: supervised medically assisted withdrawal using chlordiazepoxide or diazepam (to prevent delirium tremens and seizures); pharmacological relapse prevention with acamprosate (reduces craving), naltrexone (blocks opioid reward system), or disulfiram (aversion therapy — supervised use only). Nutritional support is critical — ALD causes protein-calorie malnutrition; high-protein, high-calorie diet (35-40 kcal/kg/day, 1.5 g protein/kg/day) is recommended; thiamine (vitamin B1) 300 mg/day IV or IM is given urgently in all malnourished drinkers to prevent Wernicke's encephalopathy and Korsakoff's syndrome; oral multivitamins, zinc, and folate supplementation. Liver transplantation: appropriate for selected patients with end-stage ALD-related cirrhosis (MELD above 15) with documented 6-month abstinence; some centres now consider early liver transplantation for severe acute alcoholic hepatitis not responding to medical therapy.

Complications If Untreated

Untreated ALD progresses inevitably to cirrhosis, precipitating life-threatening complications. Oesophageal and gastric variceal haemorrhage from portal hypertension causes massive upper gastrointestinal bleeding — 20-30% mortality per bleeding episode, even with emergency endoscopic band ligation and vasoactive drugs (terlipressin or somatostatin analogues). Spontaneous bacterial peritonitis (SBP) — infection of ascitic fluid, most commonly by Escherichia coli, Klebsiella, and Streptococcus — carries 20-30% in-hospital mortality and requires immediate IV cefotaxime or ceftriaxone. Severe acute alcoholic hepatitis without corticosteroid treatment has a 28-day mortality of up to 50% in high Maddrey DF cases.

Prevention & Lifestyle Management

Abstinence from alcohol is the single most effective prevention and treatment at all disease stages — it is the only intervention shown to reverse fatty liver, stabilise alcoholic hepatitis, and halt progression to cirrhosis. Population-level safe limits: the NHS and WHO recommend no more than 14 units per week spread over at least 3 alcohol-free days; avoid binge drinking (over 6 units in a single session for women, over 8 for men). Thiamine (vitamin B1) supplementation is essential in all heavy drinkers to prevent Wernicke-Korsakoff syndrome (Pabrinex IV or oral thiamine 100 mg TDS). Nutritional rehabilitation with a high-protein, high-calorie diet (35-40 kcal/kg/day) and multivitamin supplementation. When to Seek Medical Help for Alcoholic Liver Disease

Seek emergency care immediately (999/911 or nearest emergency department) for: vomiting blood or passing black tarry stools (possible oesophageal variceal haemorrhage — a life-threatening emergency requiring urgent endoscopy and vasoactive drugs); sudden confusion, disorientation, or aggressive behaviour in a person with liver disease (possible hepatic encephalopathy); severe abdominal pain with fever and jaundice (possible spontaneous bacterial peritonitis — requires urgent antibiotics); or rapidly worsening jaundice with drowsiness (possible acute-on-chronic liver failure). See a GP or hepatologist urgently for: any jaundice in a person who drinks alcohol; newly developing ankle swelling or abdominal distension; recurrent nausea, right upper quadrant pain, and fatigue.
