Overview: Alcoholic Liver Disease
Alcoholic liver disease (ALD) is a spectrum of liver pathology caused by chronic, excessive alcohol consumption: alcoholic fatty liver (steatosis) → alcoholic hepatitis (AH) → alcoholic cirrhosis → hepatocellular carcinoma (HCC). ALD is the most common cause of liver disease in high-income countries and accounts for approximately one-third of all liver-related deaths globally. In the UK, alcohol-related liver disease is the leading cause of premature liver-related death. Alcoholic steatosis occurs in approximately 90% of people drinking more than 40 g alcohol/day (women) or 60 g/day (men); it is reversible with abstinence. Alcoholic cirrhosis affects 10-20% of heavy drinkers — only a minority progress despite similar drinking patterns (suggesting genetic susceptibility: PNPLA3 I148M polymorphism is the strongest known genetic risk factor for ALD severity). Severe alcoholic hepatitis (AH) — acute clinical syndrome of jaundice, liver failure, and systemic inflammation — carries a 28-day mortality of 30-50% without treatment, making it one of the most serious acute liver conditions.
Causes & Risk Factors
The primary cause is chronic heavy alcohol consumption. UK low-risk guidelines: no more than 14 units per week for both sexes; at least 2 alcohol-free days per week. Harmful drinking: above 50 units/week (men) or 35 units/week (women) — significantly elevated ALD risk. Risk factors for progression from steatosis to cirrhosis: female sex (women develop ALD at lower alcohol doses — due to lower gastric alcohol dehydrogenase, higher body fat percentage, and hormonal differences); PNPLA3 I148M genetic variant — 3-4x increased risk of cirrhosis; TM6SF2 and MBOAT7 variants; obesity (BMI above 30 synergises with alcohol to dramatically accelerate fibrosis — alcohol + obesity + diabetes = highest risk); malnutrition (protein-calorie malnutrition is universal in severe ALD and worsens outcomes); smoking; HCV or HBV co-infection; duration and pattern of drinking (binge drinking particularly harmful); and drinking outside mealtimes. Pathophysiology: ethanol metabolism via alcohol dehydrogenase (ADH) produces acetaldehyde (direct hepatotoxin) and NADH (shifts metabolism favouring fat accumulation); reactive oxygen species generation; endotoxin-driven Kupffer cell activation and TNF-alpha/IL-6 release; and hepatic stellate cell activation driving fibrosis.
Symptoms & Stages
Alcoholic steatosis (fatty liver): usually asymptomatic; may have hepatomegaly (enlarged, smooth, non-tender liver); raised ALT/AST (AST:ALT ratio typically less than 2); elevated gamma-GT (sensitive marker of alcohol use — elevated in 90% of heavy drinkers); completely reversible with abstinence. Alcoholic hepatitis (AH): clinical spectrum from mild (incidental liver test abnormalities) to severe (Maddrey's Discriminant Function score above 32 — high mortality). Typical presentation: rapid onset jaundice (bilirubin often above 80 micromol/L); tender hepatomegaly; fever; anorexia; nausea and vomiting; AST above 50, AST:ALT ratio typically 2:1 (greater than 2:1 strongly suggests ALD rather than other cause); INR above 1.5; CRP elevation. Severe AH: hepatic encephalopathy (confusion, asterixis — liver flap); ascites (peritoneal fluid from portal hypertension and low albumin); acute kidney injury (hepatorenal syndrome — high mortality). Alcoholic cirrhosis: early — asymptomatic or mild fatigue; decompensated — the same portal hypertension complications as any cirrhosis: ascites (most common — up to 5 litres), oesophageal variceal bleeding (haematemesis or melaena — life-threatening), hepatic encephalopathy, spontaneous bacterial peritonitis (SBP), and HCC (2-5% annual incidence in cirrhosis).
Diagnosis & Investigations
Liver function tests: elevated ALT, AST (AST:ALT ratio above 2:1 is characteristic of ALD — if ratio above 3:1, strongly supports ALD; note the absolute levels are rarely above 300-400 IU/L in ALD — very high transaminases suggest alternative diagnosis); elevated gamma-GT (most sensitive marker of alcohol use); elevated bilirubin (in AH and decompensated cirrhosis); elevated INR (reflects impaired hepatic synthesis); low albumin (in cirrhosis and severe AH). FBC: macrocytosis (MCV above 100 fl — alcohol effect on red cell production and folate deficiency); thrombocytopenia (hypersplenism in cirrhosis); leukocytosis in AH (neutrophilia). Severity scoring in alcoholic hepatitis: Maddrey's Discriminant Function (MDF) = 4.6 × (PT – control PT) + bilirubin (micromol/L / 17.1); MDF above 32 = severe AH — prednisolone treatment recommended; Glasgow Alcoholic Hepatitis Score (GAHS) above 9 — poor prognosis; Model for End-Stage Liver Disease (MELD) score above 21 — high 28-day mortality. Liver biopsy: gold standard — confirms ALD, excludes other causes, assesses degree of fibrosis (Metavir F0-F4); features: macrovesicular steatosis, hepatocyte ballooning, Mallory-Denk bodies, neutrophilic infiltrate, pericellular fibrosis. Non-invasive fibrosis markers: FIB-4 score (age × AST / platelets × sqrt ALT); liver stiffness by transient elastography (FibroScan); elevated in fibrosis and cirrhosis. Abdominal ultrasound: assesses echogenicity (bright liver = steatosis), liver size, surface nodularity (cirrhosis), portal hypertension signs, splenomegaly, ascites; Doppler assesses portal flow and HCC screening in cirrhosis. CT/MRI abdomen and 6-monthly surveillance ultrasound for HCC in cirrhosis.
Treatment Options
Abstinence is the cornerstone of ALD management — the only intervention proven to reverse steatosis, halt fibrosis progression, and improve survival at all stages. Even in decompensated cirrhosis, 6 months of abstinence significantly improves Child-Pugh score and may allow downlisting from transplant list. Alcohol dependence treatment: referral to addiction services; medically supervised alcohol withdrawal (chlordiazepoxide or lorazepam taper to prevent withdrawal seizures and delirium tremens); acamprosate, naltrexone, or disulfiram for relapse prevention; supportive counselling. Nutritional support: malnutrition is universal in severe ALD — enteral nutrition (nasogastric tube if needed) providing 35-40 kcal/kg/day and 1.2-1.5 g protein/kg/day; thiamine replacement (Pabrinex IV) before any glucose to prevent Wernicke encephalopathy. Severe alcoholic hepatitis (MDF above 32 or GAHS above 9): prednisolone 40 mg/day for 28 days (Lille score at 7 days — if bilirubin not improved by more than 56 micromol/L, poor response — stop prednisolone as continued immunosuppression without benefit causes harm); Lille score above 0.45 at day 7 = non-responder. No benefit from pentoxifylline, TNF-alpha inhibitors, or NAC in addition to prednisolone. Management of cirrhosis complications: ascites — low sodium diet (below 2 g/day), spironolactone 100-400 mg + furosemide 40-160 mg; therapeutic paracentesis with albumin infusion for tense ascites; SBP prophylaxis with norfloxacin 400 mg BD; variceal bleeding — urgent terlipressin + endoscopic band ligation + broad-spectrum antibiotics; secondary prophylaxis with propranolol + band ligation; hepatic encephalopathy — lactulose + rifaximin (NICE approved). TIPSS (transjugular intrahepatic portosystemic stent shunt): for refractory ascites or uncontrolled variceal bleeding. Liver transplantation: for end-stage ALD — standard requirement is 6 months of abstinence before listing (some centres offer early transplant for carefully selected severe AH non-responders — evidence supporting this approach growing). 5-year post-transplant survival above 70% in ALD; alcohol relapse occurs in 20-30% post-transplant.
Complications
Alcoholic liver disease progresses through predictable stages, each with life-threatening complications. Acute alcoholic hepatitis — with Maddrey's Discriminant Function (DF) above 32 — carries 28-day mortality of 25-50% without treatment; Maddrey DF above 54 or MELD score above 21 indicates severe disease with greater than 40% 1-month mortality. Cirrhosis, the irreversible end-stage of chronic alcoholic liver damage, leads to portal hypertension with its major complications: variceal haemorrhage (gastro-oesophageal varices rupturing — presenting with haematemesis and melaena — carries 15-20% mortality per episode and 70% rebleeding risk within 1 year without beta-blocker prophylaxis or endoscopic band ligation), ascites (fluid accumulation in the peritoneum — 50% 2-year mortality once ascites develops), and spontaneous bacterial peritonitis (infection of ascitic fluid — mortality 30% per episode). Hepatorenal syndrome — functional acute kidney injury from severe hepatic dysfunction and renal vasoconstriction — carries extremely high mortality without liver transplantation. Hepatic encephalopathy (ammonia-driven cerebral dysfunction causing confusion, asterixis, and coma) impairs quality of life and predicts poor prognosis. Hepatocellular carcinoma develops in 1-2% of cirrhotic alcoholic liver disease patients annually. Portal hypertension also causes hypersplenism and coagulopathy from impaired hepatic clotting factor synthesis, increasing bleeding risk.
Prevention & Lifestyle
Primary prevention: drink within UK Chief Medical Officers' low-risk guidelines (no more than 14 units/week; spread over 3 or more days; at least 2 alcohol-free days/week). Avoid drinking during pregnancy (fetal alcohol spectrum disorder). Early intervention for hazardous drinking: AUDIT-C screening in primary care; NICE recommends Brief Alcohol Intervention (FRAMES approach) for hazardous/harmful drinking — 5-minute structured motivational advice. Minimum unit pricing (MUP) in Scotland (50p per unit) has demonstrably reduced alcohol-related liver deaths. For patients with known ALD: complete and sustained abstinence; hepatitis B vaccination (immunisation response impaired in cirrhosis — check titres); HAV vaccination; biannual HCC surveillance (ultrasound ± AFP) in cirrhosis; upper GI endoscopy at diagnosis of cirrhosis for variceal screening; portal hypertension prophylaxis as above; low-sodium diet; avoid NSAIDs (worsen renal function and precipitate variceal bleeding).

Call emergency services (999/911) immediately for: haematemesis (vomiting blood) or passing black tarry stools (melaena) — likely variceal bleeding — a life-threatening emergency requiring urgent endoscopy, terlipressin, and IV access; sudden severe abdominal pain with fever in someone with cirrhosis and ascites — spontaneous bacterial peritonitis (SBP) — emergency diagnostic tap and IV antibiotics; confusion, drowsiness, or unresponsiveness in someone with known liver disease — hepatic encephalopathy or hyponatraemia — urgent assessment; severe breathlessness in someone with known ascites — large pleural effusion (hepatic hydrothorax) or hepatopulmonary syndrome. Contact GP or hepatology team urgently for: rapidly increasing jaundice, abdominal swelling, or ankle oedema — decompensation; first episode of ascites — requires paracentesis and hepatology review; MELD score escalating — may require urgent transplant listing consideration; fever in someone with known cirrhosis — high infection risk.
