What Is Jaundice? Bilirubin & Clinical Significance

Causes of Jaundice by Type

Post-hepatic/cholestatic jaundice (conjugated — direct bilirubin elevation, with dark urine and pale stools): choledocholithiasis (common bile duct stones — most common cause); biliary strictures; cholangiocarcinoma; pancreatic cancer; primary sclerosing cholangitis; primary biliary cholangitis (PBC); and post-surgical bile duct damage.

Symptoms & Clinical Features

The cardinal feature is yellowish discolouration of the sclera and skin. Associated symptoms vary crucially by aetiology. Pre-hepatic jaundice (haemolytic): anaemia symptoms (fatigue, pallor, breathlessness), mild jaundice, splenomegaly, normal or pale urine, normal stool colour. Hepatic jaundice: progressive jaundice, fatigue, anorexia, nausea, right upper quadrant discomfort, dark urine (tea or cola coloured from bilirubin), pale stools (clay-coloured from reduced bilirubin excretion), and constitutional symptoms (fever in hepatitis, cachexia in malignancy). Signs of chronic liver disease: spider naevi, palmar erythema, leukonychia, finger clubbing, hepatomegaly, splenomegaly, ascites, and caput medusae. Cholestatic jaundice: progressive deep jaundice, pruritus (severe itching from bile salt deposition in skin — can be debilitating), pale stools, dark urine, weight loss (malignant obstruction), and pain (biliary colic from gallstones). Painless progressive jaundice in an older person is pancreatic cancer until proven otherwise. Fever, jaundice, and right upper quadrant pain (Charcot's triad) indicate ascending cholangitis — a medical emergency requiring immediate antibiotics and biliary decompression.

Diagnosis: LFTs, Ultrasound & ERCP

Liver function tests (LFTs): ALT and AST (hepatocellular markers — markedly elevated in viral/toxic hepatitis); ALP and GGT (cholestatic markers — elevated in biliary obstruction and PBC); bilirubin (total, direct/conjugated, and indirect/unconjugated — fractionation guides classification); albumin and prothrombin time (markers of synthetic function — impaired in significant hepatocellular disease). FBC: anaemia and reticulocytosis (haemolysis), thrombocytopenia (portal hypertension). Abdominal ultrasound: first-line imaging — identifies bile duct dilatation (indicates post-hepatic obstruction), gallstones, hepatic masses, liver texture changes (cirrhosis). CT abdomen and pelvis (pancreatic protocol): detects pancreatic and biliary malignancy with staging. MRCP (MR cholangiopancreatography): non-invasive gold standard for imaging the biliary tree — identifies stones, strictures, and anatomical variants. ERCP (endoscopic retrograde cholangiopancreatography): therapeutic — removes common bile duct stones, places biliary stents for strictures, and takes brush cytology for diagnosis. EUS (endoscopic ultrasound): superior for detecting small pancreatic masses, periampullary lesions, and bile duct stones. Viral serology, autoimmune liver panel, and genetic tests (Wilson's, haemochromatosis) for appropriate causes. Liver biopsy for definitive histological diagnosis when needed.

Treatment by Underlying Cause

Treatment is directed at the underlying cause. Pre-hepatic haemolytic jaundice: treat the specific cause — immunosuppression for autoimmune haemolytic anaemia (prednisolone, rituximab); hydroxycarbamide for sickle cell; splenectomy for hereditary spherocytosis; antimalarials for malaria. Gilbert's syndrome requires no treatment — reassurance that it is benign. Hepatic jaundice: direct-acting antivirals for hepatitis C (cure 95%+); antivirals (tenofovir, entecavir) for hepatitis B; corticosteroids for autoimmune hepatitis and severe alcoholic hepatitis; N-acetylcysteine for paracetamol overdose (within 24 hours — highly effective if given early); alcohol abstinence for alcoholic hepatitis; and liver transplantation for acute liver failure and end-stage cirrhosis. Cholestatic jaundice: ERCP with stone extraction for choledocholithiasis — urgent if ascending cholangitis is suspected. Biliary stenting for malignant obstruction (provides palliation). Surgery (Whipple's pancreaticoduodenectomy for pancreatic head cancer or cholangiocarcinoma). Ursodeoxycholic acid (UDCA) for PBC (slows progression). Complications

The complications of jaundice arise from the underlying condition causing bilirubin elevation rather than bilirubin accumulation per se, though very high bilirubin levels contribute to their own toxicity. Untreated biliary obstruction leads to acute cholangitis — bacterial infection ascending obstructed bile ducts causing sepsis with Charcot's triad (fever, rigors, right upper quadrant pain) or Reynold's pentad (adding hypotension and confusion) — a life-threatening emergency requiring urgent ERCP biliary drainage and IV antibiotics; mortality without drainage approaches 100% in suppurative cholangitis.
