Overview: Lymphedema

Symptoms & Signs

Diurnal variation is characteristic in early stages — swelling typically worse by evening and improves overnight with limb elevation. Lymphedema causes profound psychosocial impact: depression, anxiety, body image distress, and social isolation are common from the visible, chronic nature of the condition.

Diagnosis & Tests

Lymphedema is primarily a clinical diagnosis based on history (cancer treatment, family history, onset pattern) and examination (characteristic oedema distribution, positive Stemmer's sign, skin changes, ISL staging). Limb volume measurement using sequential circumferential measurements (using a perometer optical volumetry device or tape-measure calculated with the truncated cone formula) establishes baseline and monitors treatment response — a 10% volume difference between limbs is clinically significant. Imaging investigations to confirm diagnosis or assess lymphatic anatomy: lymphoscintigraphy (nuclear medicine scan using technetium-99m labelled colloid injected intradermally) — the gold standard for confirming lymphatic system dysfunction, demonstrating dermal backflow, absent or delayed lymphatic uptake, absent lymph nodes, and lymphatic collateral pathways; indocyanine green (ICG) near-infrared fluorescence lymphography — a sensitive, real-time imaging modality visualising superficial lymphatic vessels and identifying pre-clinical functional impairment; MRI lymphangiography — provides detailed anatomical information on lymphatic vessel and lymph node morphology for surgical planning (lymph node transfer, lymphaticovenous anastomosis); duplex ultrasound — excludes deep vein thrombosis as a cause of acute limb swelling (important differential diagnosis in cancer patients). Treatment Options

Lymphedema has no cure, but complex decongestive therapy (CDT) — the gold standard treatment — significantly reduces limb volume, prevents progression, and substantially improves quality of life. Complications of Lymphedema

Acute and recurrent cellulitis is the most serious and common complication of lymphedema — the protein-rich lymphatic fluid provides an ideal bacterial culture medium, and loss of normal lymphatic immune surveillance removes a critical infection barrier; each episode of cellulitis further damages lymphatic vessels, worsening lymphedema severity and creating a self-perpetuating cycle. Cellulitis typically presents as a rapidly spreading area of erythema, warmth, swelling, and systemic features (fever, rigors, tachycardia) — requiring prompt antibiotic treatment (IV flucloxacillin or amoxicillin for mild-moderate; piperacillin-tazobactam for severe) and temporary compression reduction. Regular gentle exercise such as swimming or walking helps pump lymph fluid and keeps the affected limb mobile.
