Surgical menopause following bilateral oophorectomy causes immediate onset of severe vasomotor symptoms due to abrupt oestrogen withdrawal. Menopause is a universal physiological transition in women — approximately 1.3 million women enter menopause each year in the United States — but its timing, symptom severity, and long-term health consequences vary substantially between individuals and populations. Causes & Risk Factors
Natural menopause is caused by the irreversible depletion of ovarian primordial follicles — a process beginning before birth and continuing throughout reproductive life. As follicular reserves decline, the remaining follicles become resistant to gonadotrophin stimulation, oestradiol production falls, and FSH rises compensatorily. Premature ovarian insufficiency (POI) affecting women under 40 has diverse causes: autoimmune (the most common identifiable cause in developed countries — anti-ovarian and anti-adrenal antibodies, often associated with autoimmune thyroid disease or Addison's disease), chromosomal (Turner syndrome 45,X; Fragile X premutation carriers — FMR1 gene; chromosomal mosaicism), iatrogenic (chemotherapy — particularly alkylating agents such as cyclophosphamide, which destroys primordial follicles in a dose-dependent, age-dependent manner; pelvic radiotherapy above 2-5 Gy causes permanent ovarian failure), surgical oophorectomy, and infiltrative diseases (tuberculosis, haemochromatosis). Approximately 75% of POI cases have no identifiable cause. Surgical menopause — bilateral oophorectomy — produces immediate, abrupt oestrogen withdrawal causing severe vasomotor and genitourinary symptoms typically more severe than natural menopause. Symptoms & Signs

Psychological and cognitive symptoms: anxiety, irritability, mood instability, and depressive symptoms in perimenopause (independent of prior psychiatric history); cognitive difficulties ('brain fog', poor concentration, verbal memory impairment); sleep disruption from night sweats causing secondary fatigue and mood disturbance. Musculoskeletal symptoms: joint pain, muscle stiffness, and increased fracture risk from accelerating bone loss. Physical changes include skin thinning, hair loss, and redistribution of body fat towards visceral adiposity.
Diagnosis & Tests
Menopause is a clinical diagnosis in women over 45 with typical symptoms (irregular periods, vasomotor symptoms) and does not routinely require blood tests to confirm. In the UK, NICE recommends against routine FSH measurement in women over 45 with typical menopausal symptoms. FSH measurement is indicated in: women under 45 (to confirm POI — FSH above 25 IU/L on two occasions 4-6 weeks apart with appropriate symptoms); women who have had a hysterectomy without oophorectomy (cannot use menstrual pattern for diagnosis); and women using hormonal contraception making periods and symptoms difficult to interpret (FSH unreliable while on combined OCP or progestogen-only methods). Serum oestradiol levels are unreliable for diagnosis due to the wide variation in perimenopausal secretion. Anti-Müllerian hormone (AMH) and antral follicle count (AFC) are measures of ovarian reserve but not validated diagnostic tools for menopause per se. Thyroid function tests (TSH, free T4) are recommended to exclude hypothyroidism, which closely mimics menopausal symptoms (fatigue, weight gain, cognitive difficulties, irregular periods). DEXA scan for bone mineral density assessment is indicated in women under 45 (POI) or women with significant osteoporosis risk factors — fracture risk assessment using FRAX tool guides decisions on bisphosphonate prophylaxis alongside HRT.
Treatment Options
Hormone replacement therapy (HRT) is the most effective treatment for menopausal symptoms, recommended by NICE, BMS, and the Endocrine Society for most women under 60 or within 10 years of menopause onset in whom benefits outweigh risks. Combined HRT (oestrogen + progestogen) is required in women with an intact uterus — progestogen protects against oestrogen-induced endometrial hyperplasia and cancer; women post-hysterectomy receive oestrogen alone. Local vaginal oestrogen (pessaries, cream, ring) for genitourinary syndrome of menopause provides symptomatic relief without significant systemic absorption and is safe for most women including those with a history of breast cancer (with specialist guidance). Non-hormonal options for vasomotor symptoms in women who cannot or choose not to use HRT: venlafaxine (75-150mg) and escitalopram/paroxetine (SSRIs/SNRIs) — reduce hot flash frequency by 50-60%; gabapentin 300-900mg nocte (useful for night sweats); oxybutynin 2.5-5mg; fezolinetant (Veoza, a neurokinin 3 receptor antagonist) — the first non-hormonal specifically licensed treatment for moderate-severe VMS, approved by EMA and FDA 2023, reducing hot flash frequency by 65%.
Complications
Oestrogen deficiency accelerates bone resorption, causing a 2-3% annual reduction in bone mineral density in the first 5 years after menopause — cumulatively causing osteoporosis in 1 in 3 postmenopausal women and accounting for 50% of lifetime fracture risk. Hip fracture carries 20-30% one-year mortality and permanent disability in over 50% of survivors. Genitourinary syndrome of menopause is progressive and severely impacts sexual function, causing relationship difficulties and reduced quality of life in over 50% of postmenopausal women — yet under 25% seek treatment due to stigma. Depression affects 18-25% of perimenopausal women — the window of vulnerability for new-onset and recurrent depression. Severe night sweats causing chronic sleep disruption are associated with cognitive impairment, cardiovascular risk, and impaired immune function. Untreated POI without HRT confers the full cardiovascular, bone, and cognitive risks of oestrogen deficiency from a much younger age.
Prevention & Management
Regular weight-bearing and resistance exercise preserves bone mineral density (equivalent to 1-2% BMD protection per year), improves cardiovascular health, and reduces vasomotor symptom severity through endorphin-mediated pathways — 150 minutes weekly of moderate-intensity aerobic exercise plus twice-weekly resistance training is recommended. Adequate calcium intake (1000-1200 mg daily from dietary sources — dairy, fortified foods, leafy green vegetables) and vitamin D supplementation (800-1000 IU daily, higher if deficient) are essential to support bone health alongside or instead of HRT. Smoking cessation reduces VMS severity and frequency, slows bone loss, and reduces cardiovascular risk. Alcohol moderation (below 14 units weekly) reduces breast cancer risk and improves sleep quality. Cognitive behavioural therapy (CBT) for hot flashes — delivered via the Mindfulness-Based Cognitive Therapy for Menopause (MBCT-M) programme or structured 6-8 week CBT programme — reduces hot flash frequency by 40-50% and significantly improves sleep, mood, and quality of life in RCTs, providing an effective non-pharmacological option. Mindfulness-based stress reduction and paced respiration are similarly evidence-based. For women with premature ovarian insufficiency, HRT (or combined OCP) continued until at least age 51 (natural menopause age) is strongly recommended to prevent premature cardiovascular disease, osteoporosis, and cognitive decline — the risks of HRT in POI before age 51 are lower than the harms of untreated oestrogen deficiency.

See your GP if menopausal symptoms — hot flashes, night sweats, vaginal dryness, low mood, sleep disruption, brain fog, or joint pains — are significantly affecting your quality of life, work, relationships, or sleep. Effective treatments including HRT are available and recommended for most women; you do not need to endure severe symptoms. NICE recommends offering HRT to most women under 60 or within 10 years of menopause onset after discussing benefits and risks individually. See your GP urgently or request a same-week appointment for: any vaginal bleeding that occurs more than 12 months after your last period (postmenopausal bleeding — this requires urgent investigation to exclude endometrial cancer, even if you are using HRT). Women under 40 who notice new irregular periods, hot flashes, or menopausal symptoms should see their GP promptly — premature ovarian insufficiency (POI) is a serious diagnosis requiring confirmation by repeated FSH measurements and specialist management including fertility counselling. If HRT is not adequately controlling symptoms after 3 months of consistent use, request review of preparation, dose, or route of administration with your GP or a specialist menopause clinic.
