It is consistently one of the most prevalent pain conditions in the general population: dental caries (tooth decay) affecting 2.5 billion people globally is the underlying driver of the majority of toothache presentations, and dental pain is the most common reason for emergency dental attendance and one of the most frequent causes of unplanned GP visits and antibiotic prescribing. Severity ranges from mild, intermittent sensitivity to specific stimuli (cold or sweet food — characteristic of early reversible pulpitis or dentine hypersensitivity) through to severe, continuous, throbbing, spontaneous pain that is disproportionately worse at night and resistant to analgesics (irreversible pulpitis, requiring root canal treatment or extraction). Toothache is never a minor or self-limiting symptom to be managed indefinitely with analgesics alone — it universally indicates underlying dental pathology that will progress and worsen without definitive dental treatment. The spectrum of pathologies responsible ranges from relatively straightforward (reversible pulpitis from early caries, managed by removing decay and placing a composite filling) to serious and potentially life-threatening (spreading dental abscess causing facial cellulitis, Ludwig's angina — bilateral submandibular space infection with airway compromise, or cavernous sinus thrombosis — septic venous thrombosis of the cavernous sinus causing intracranial infection). Dental pain can also arise from non-dental orofacial causes that must be distinguished: referred pain from maxillary sinusitis (upper molar-like pain from maxillary sinus floor inflammation), temporomandibular joint (TMJ) disorder, trigeminal neuralgia (electric shock-like lancinating pain in the distribution of a trigeminal branch), and rarely cardiac ischaemia — in which jaw pain or left lower molar pain may be the only presenting symptom of myocardial infarction and should be suspected in any patient with unexplained jaw pain and cardiovascular risk factors.
Causes & Risk Factors
Dental caries progressing to pulpitis is the most common cause: bacterial acids (primarily from Streptococcus mutans fermenting dietary sugars) demineralise the enamel and underlying dentine; once caries penetrates the dentine, bacterial toxins and inflammation reach the dental pulp; reversible pulpitis occurs when pulp inflammation is mild (treatable by removing caries and restoring the tooth — pulp can recover); irreversible pulpitis occurs when inflammation is severe or bacteria invade the pulp directly (pulp cannot recover — requires root canal treatment or extraction). Periapical abscess: bacteria from a necrotic pulp spread through the root canal and exit at the root apex into the periapical bone, causing a localised collection of pus in the periapical tissues — the most common cause of dental emergency attendance; the abscess can be contained (localised periapical abscess with tenderness on biting and percussion) or can spread into adjacent soft tissue spaces causing facial cellulitis, swelling, and systemic sepsis. Cracked tooth syndrome: an incomplete longitudinal fracture extending from the crown towards the root — typically through enamel, dentine, and towards the pulp; causes by biting forces, bruxism (night grinding), large occlusal restorations weakening tooth structure, or trauma; produces a characteristic sharp, shooting pain specifically on biting (especially releasing the bite), which is highly localised to the cracked tooth — differentiating it from pulpitis; often very difficult to diagnose as the crack is invisible on X-ray. Periodontitis: chronic bacterial infection of the gum (periodontal) tissues causing progressive loss of the alveolar bone supporting the tooth; causes a dull, persistent ache and sensitivity to biting; acute necrotising ulcerative periodontitis or acute periodontal abscess (localised pus collection in the gum pocket) causes more severe pain. Pericoronitis: bacterial infection in the operculum (gum flap) overlying a partially erupted lower wisdom tooth (third molar); extremely common in late teens and early twenties; causes severe pain, trismus (difficulty opening the mouth), swelling, and characteristic bad taste and bad breath; can spread to the adjacent submandibular or masticator spaces rapidly. Dentinal hypersensitivity: exposed dentine from gum recession, erosion, or abrasion causing sharp brief pain to cold, sweet, acid, or air stimuli through hydrodynamic fluid movement in the dentine tubules stimulating pulp nerve endings — not a structural emergency but significantly impacts quality of life. Other causes: dental trauma (tooth fracture or avulsion); periapical cyst or granuloma; temporomandibular joint disorder (referred preauricular and temple pain); acute maxillary sinusitis (referred upper posterior tooth pain); and, rarely, cardiac ischaemia (left jaw or lower molar pain — a referred pain pattern of acute myocardial infarction requiring immediate cardiac assessment).
Symptoms & Signs
Reversible pulpitis symptoms: sharp, brief pain in response to specific stimuli — most characteristically cold (cold food, cold drink, cold air) and sweet stimuli; the pain is immediate and sharp, lasting only seconds (typically 5-10 seconds after the stimulus is removed); the tooth is not spontaneously painful; percussion of the tooth (tapping with a metal instrument) is not usually painful; reversible pulpitis responds to caries removal and restoration alone without root canal treatment, as the pulp retains its capacity for healing. Irreversible pulpitis symptoms: the most severe and distressing dental pain pattern; spontaneous, persistent, severe, throbbing pain that arises without any trigger and is not proportionate to any identifiable stimulus; characteristic of irreversible pulpitis is pain that is dramatically worse at night when lying down (increased intracranial and pulpal pressure in horizontal position causing vascular congestion in the enclosed pulp chamber — the patient cannot sleep and often walks around or hangs their head off the side of the bed for temporary relief); pain lingers for a prolonged period (30 seconds to several minutes) after the stimulus is removed — contrasting with the brief seconds of reversible pulpitis; heat causes more pain than cold (characteristic in late irreversible pulpitis where cold may temporarily relieve the inflammation — a clinically useful diagnostic distinction); paracetamol alone provides little relief. Periapical abscess symptoms: constant, severe, pulsating pain localised to one tooth; the tooth is exquisitely tender on biting or percussion (tapping the tooth with a finger or dental mirror causes severe pain — the most reliable clinical sign of periapical infection); there may be visible gum swelling with fluctuance (an abscess pointing at the buccal sulcus gingival tissue ready to spontaneously drain); facial cellulitis (diffuse tender swelling of the cheek, submandibular region, or infraorbital region); fever, malaise, and regional lymphadenopathy in spreading infections; trismus (restricted mouth opening from masseteric or pterygoid space involvement — a sign of spreading infection requiring urgent treatment). Cracked tooth syndrome: sharp, shooting pain specifically provoked by biting onto something hard — characteristically on releasing the bite rather than during biting; the pain is brief, immediately severe, and highly localised to one specific tooth; diagnosis is often delayed as the crack is invisible on standard periapical X-ray and requires careful clinical testing (bite test, transillumination, or use of tooth sleuth diagnostic instrument). Dentinal hypersensitivity: brief, sharp pain to cold, sweet, acid, or air stimuli — with no spontaneous pain and short duration; distinguished from pulpitis by the short duration of pain and absence of spontaneous symptoms.
Diagnosis & Tests
Clinical dental examination: vitality testing (electric pulp test, cold CO2 ice test), percussion test (periapical pathology), periodontal probing (pocket depths). Intraoral periapical X-ray (periapical abscess, caries depth, root resorption, bone loss). CBCT (cone beam CT) for complex diagnosis (root fractures, accessory canals). Palpation of buccal sulcus (swelling/sinus tract). Transillumination (cracked tooth). A systematic diagnostic approach that carefully combines a thorough clinical history, detailed physical examination findings, and appropriately targeted laboratory and imaging investigations is essential to accurately distinguish this condition from closely mimicking disorders and to guide safe and effective treatment selection. Early specialist referral — particularly when the diagnosis is uncertain or when initial treatments fail — optimises diagnostic accuracy and reduces unnecessary delays to effective management.
Treatment Options
Reversible pulpitis: remove caries, place sedative dressing (IRM/calcium hydroxide), restore with composite filling. Irreversible pulpitis/periapical abscess: root canal treatment (RCT) — biomechanical preparation and obturation of root canals; or extraction if RCT not possible. Antibiotics (amoxicillin 500 mg TDS or metronidazole 400 mg TDS) only if systemic spread (swelling, fever, trismus — not for localized pulpitis). Incision and drainage (I&D) for fluctuant abscess. NSAIDs (ibuprofen 400 mg + paracetamol 1g alternating) for acute pain. Regular monitoring of treatment response, early detection of side effects, and ongoing assessment of disease progression are essential components of optimising patient outcomes over the long term. Complications
Untreated dental abscess can spread to adjacent spaces: buccal space, sublingual space, Ludwig's angina (bilateral submandibular space infection — life-threatening, airway compromise), pterygomandibular space (trismus), and parapharyngeal space. Cavernous sinus thrombosis (rare but fatal). Osteomyelitis of jaw. Bacteremia and systemic sepsis in immunocompromised patients. Tooth loss and impaired mastication. Long-term specialist follow-up and structured regular review are essential to detect and appropriately manage complications at the earliest possible stage, minimising long-term disability, preserving organ function, and improving the overall prognosis. Patient education about the early warning signs of complications and clear guidance on when to seek urgent medical attention empowers timely help-seeking behaviour and reduces preventable serious adverse outcomes. Psychological impact — including depression, anxiety, and reduced quality of life — should be proactively assessed and addressed as part of comprehensive complication management.
Prevention & Management
Regular dental check-ups every 6 months (or annually for low-risk patients — based on the NICE Clinical Guideline CG19 recall interval framework): professional examination allows detection of early caries (before pulpal involvement), periodontal disease, cracked teeth, and soft tissue abnormalities at a stage when treatment is simpler, cheaper, and less painful than emergency management; routine monitoring also includes professional plaque and calculus (tartar) removal to prevent periodontitis progression. Alternatively, attend an NHS urgent dental care (walk-in) centre. Never place aspirin tablets directly on the gum tissue — this causes chemical (aspirin) burns to the oral mucosa and does not provide effective pain relief.
