Overview: Vitiligo

Vitiligo is a chronic acquired autoimmune disorder characterised by selective destruction of melanocytes — the pigment-producing cells of the skin, hair, and mucous membranes — resulting in well-demarcated, depigmented (chalk-white or milk-white) macules and patches that are completely devoid of pigmentation. It affects 0.5-2% of the global population regardless of race or ethnicity; however, the visible impact is much more pronounced in individuals with darker skin tones, where the contrast between affected and unaffected skin causes significant psychological distress and social stigma. Vitiligo affects men and women equally; onset most commonly occurs between 10-30 years of age, although it can develop at any age. Vitiligo is classified into two main types with distinct clinical behaviour: non-segmental vitiligo (NSV — also called generalised or bilateral vitiligo; accounts for 85-90% of cases) — characterised by bilateral, often symmetric, patches that tend to spread progressively over the years, preferentially affecting acral sites (hands, feet), perioral and periorbital areas, elbows, and knees; and segmental vitiligo (SV — 10-15% of cases) — characterised by unilateral, band-like or dermatomal distribution, rapid progression for 1-2 years then stable arrest, less commonly associated with other autoimmune diseases, and better surgical outcomes. Vitiligo is strongly associated with other autoimmune conditions — particularly autoimmune thyroid disease (20-30%), type 1 diabetes, alopecia areata, rheumatoid arthritis, and Addison's disease. The first specifically approved targeted therapy — ruxolitinib cream (Opzelura, a topical JAK inhibitor) — received FDA approval in 2022 and NICE approval in 2024.

Causes & Risk Factors

The primary mechanism is autoimmune CD8+ cytotoxic T-cell mediated destruction of melanocytes — specific autoreactive CD8+ T cells targeting melanocyte antigens (MART-1, gp100, tyrosinase) via the IFN-gamma and CXCL10 signalling pathway; IFN-gamma released by CD8+ T cells induces CXCL10 production by keratinocytes, which recruits further cytotoxic T cells in a self-perpetuating inflammatory loop selectively destroying melanocytes. JAK-STAT signalling is central to this pathway, explaining why topical JAK inhibitors (ruxolitinib 1.5% cream) are effective therapeutically. Genetic susceptibility: HLA-A*02:01 antigen presentation of melanocyte peptides to autoreactive T cells; HLA-DR4 and DQB1 associations; NLRP1 inflammasome gene loss-of-function mutations (cause familial vitiligo); and PTPN22 polymorphisms (a major autoimmune susceptibility gene shared with rheumatoid arthritis, type 1 diabetes, and lupus). Positive family history in approximately 30% of cases — first-degree relatives have a substantially elevated lifetime risk. Associated autoimmune conditions: autoimmune thyroid disease (Hashimoto's thyroiditis and Graves' disease — 20-30% of vitiligo patients), type 1 diabetes mellitus, Addison's disease, alopecia areata, pernicious anaemia, rheumatoid arthritis, and systemic lupus erythematosus. Environmental triggers: the Koebner phenomenon (isomorphic response — vitiligo appearing at sites of skin trauma, sunburn, friction, tattoos, or surgical wounds — documented in 20-50% of patients); oxidative stress from reactive oxygen species (damaged keratinocytes release HMGB1 danger signals activating innate immunity); acute sun exposure or sunburn; and significant physical or psychological stress preceding disease onset or new relapse.

Symptoms & Signs

The hallmark feature of vitiligo is the appearance of well-demarcated, chalk-white (milk-white or porcelain-white) macules and patches — flat lesions with no surface changes (no scales, no elevation, no induration) — on the skin. The lesions represent areas of complete absence of functioning melanocytes and melanin. Distribution patterns in non-segmental vitiligo: periorbital (around the eyes — one of the most common and cosmetically distressing sites); perioral (around the mouth, extending to the lips — mucosal vitiligo); acral sites (dorsal hands, fingers, and feet — around the knuckles and nails); elbows, knees, and ankles (pressure and friction sites — Koebner phenomenon sites); axillae, groins, and genitalia; and around body orifices. Vitiligo affects mucous membranes (lips, inside the mouth, genital mucosa) in a significant proportion of patients. Leukotrichia (depigmentation of hair within vitiligo patches — white hairs growing from affected skin): indicates that follicular melanocyte stem cells have been destroyed — a poor prognostic indicator for repigmentation with medical treatment, as hair follicles are the reservoir from which melanocytes repopulate during treatment. Vitiligo is asymptomatic — no itch, pain, or sensation abnormality within patches — but depigmented skin is highly sensitive to sunburn due to the absence of melanin UV protection. Disease activity assessment: rapidly spreading vitiligo with hyperpigmented borders and new lesions appearing at sites of skin trauma (Koebner-positive — stress, scratches, friction) indicates 'active' disease requiring systemic treatment. The Vitiligo Area Scoring Index (VASI) and Vitiligo Extent Score (VES) quantify the percentage of body surface area affected.

Diagnosis & Tests

Vitiligo is primarily a clinical diagnosis based on characteristic depigmented macules with typical distribution — investigations are directed at confirming the diagnosis in atypical cases and screening for associated autoimmune comorbidities. Wood's lamp examination (long-wave ultraviolet A light at 365 nm): vitiligo patches fluoresce as bright chalky-blue or blue-white, markedly more visible than with normal lighting — particularly useful in fair-skinned individuals where depigmented patches may not be obvious in natural light, and for distinguishing complete depigmentation (vitiligo) from partial depigmentation (post-inflammatory hypopigmentation, pityriasis alba). Skin biopsy (punch biopsy — rarely needed): shows absence of melanocytes on Melan-A/MART-1 immunohistochemical staining (the definitive histological feature) with preserved keratinocytes and mild inflammatory infiltrate at the active border. Baseline screening for autoimmune comorbidities (annually): thyroid function tests (TSH, free T4, anti-TPO antibodies — thyroid disorders in 20-30%); fasting blood glucose and HbA1c (type 1 diabetes); full blood count (pernicious anaemia — B12 deficiency from autoimmune gastritis); ANA and anti-dsDNA (SLE); and 9 am cortisol (Addison's disease in those with symptoms). Ophthalmology referral if uveitis or iritis is suspected.

Treatment Options

Topical treatments for localized disease: potent corticosteroids (mometasone, clobetasol — 3-4 month trial) or topical calcineurin inhibitors (tacrolimus 0.1% ointment — preferred for face/flexures, avoids skin atrophy). Complications

Sunburn susceptibility: depigmented skin lacks melanin — the primary UV-absorbing photoprotective pigment — making vitiligo patches highly vulnerable to acute sunburn (erythema, blistering) and increasing cumulative UV-related skin damage; chronic UV exposure on depigmented skin may increase the risk of squamous cell carcinoma and actinic keratosis in severely affected areas. Psychosocial complications are the most significant and disabling aspect of vitiligo for most patients: depression affects 10-30% of people with vitiligo; anxiety and social withdrawal are prevalent; romantic and sexual relationship difficulties; stigmatisation and discrimination (particularly in South Asian, Middle Eastern, and African communities where vitiligo carries social stigma, historically with misconceptions conflating it with leprosy); reduced occupational functioning; and body image disturbance — vitiligo ranks among the skin conditions with the highest impact on health-related quality of life (DLQI scores comparable to psoriasis). Dermatology Life Quality Index (DLQI) should be assessed in all patients. Associated autoimmune comorbidities: untreated hypothyroidism (Hashimoto's) from undetected thyroid autoimmunity; Addison's disease (adrenal insufficiency — potentially life-threatening if unrecognised); pernicious anaemia from autoimmune gastritis. Ocular involvement: autoimmune uveitis (inflammation of the uveal tract causing eye pain, photophobia, and visual disturbance) occurs in 1-5% of vitiligo patients — requires prompt ophthalmology review and treatment with topical corticosteroids.

Prevention & Management

Photoprotection is the single most important daily management measure for all vitiligo patients: apply SPF 50+ broad-spectrum (UVA and UVB) sunscreen to all exposed depigmented areas every morning and reapply every 2 hours during outdoor sun exposure; wear protective clothing (long sleeves, wide-brimmed hat) and avoid direct sun exposure during peak UV intensity (11 am-3 pm).
