Common abdominal conditions represent some of the most frequent diagnoses in medicine: acute appendicitis (approximately 250,000 hospitalizations per year in the US), inguinal and ventral hernias (over 700,000 repairs annually in the US), irritable bowel syndrome (IBS, affecting 10-15% of adults globally), cholelithiasis and gallstone disease (affecting approximately 20 million Americans), and peritonitis (a life-threatening emergency requiring immediate surgical intervention). Additional important abdominal disorders include peptic ulcer disease, inflammatory bowel disease (Crohn's disease and ulcerative colitis), diverticular disease, bowel obstruction, and mesenteric ischemia. Abdominal symptoms — including pain, distension, nausea, altered bowel habits, and gastrointestinal bleeding — account for a substantial proportion of emergency room visits, primary care consultations, and surgical admissions worldwide. Causes and Risk Factors

Peritonitis carries significant mortality of 10-30% overall depending on the underlying cause and time to surgical intervention; survivors may develop adhesions causing future bowel obstruction. Factors that adversely affect prognosis include delayed diagnosis, age above 70, comorbid diabetes or immunosuppression, and the underlying cause of peritonitis. Gallstone disease resolved by cholecystectomy is generally curative — over 90% of patients are symptom-free long-term. Long-term monitoring for bowel obstruction from post-peritonitis adhesions, hernia recurrence, and IBS-related quality of life is important for comprehensive patient follow-up and ongoing management.
Prevention
Maintaining a healthy body weight reduces the risk of abdominal hernias, as obesity increases intra-abdominal pressure on the abdominal wall. Avoiding chronic constipation through adequate dietary fibre and hydration prevents excessive straining that predisposes to hernias and diverticular disease. For gallstone prevention, reducing refined carbohydrates, maintaining physical activity, and avoiding rapid weight loss (which promotes bile supersaturation) are beneficial. IBS symptoms can be reduced by identifying and avoiding personal dietary triggers, managing stress through cognitive-behavioural therapy, and establishing regular meal patterns. Regular screening colonoscopy from age 45 (or earlier with family history) prevents colorectal pathology. Prompt medical attention for abdominal symptoms prevents dangerous complications such as perforation and peritonitis.
When to See a Doctor
Seek immediate emergency care for severe sudden-onset abdominal pain ('thunderclap'), a rigid board-like abdomen, persistent vomiting, fever with severe abdominal pain, rectal bleeding, or any abdominal trauma. Right lower quadrant pain that progressively worsens over hours should be evaluated urgently to exclude appendicitis. A painful abdominal bulge that cannot be pushed back (non-reducible hernia) is a surgical emergency requiring same-day evaluation.
