Overview: Gallbladder Cancer
Gallbladder cancer is the most common biliary tract malignancy, with approximately 5,000 new US cases annually and an estimated 210,000 cases globally. It is an adenocarcinoma arising from the gallbladder epithelium and carries a poor overall prognosis, with a 5-year survival of less than 20% across all stages due to a tendency for late presentation. Geographic incidence is highly variable: rates are highest in Chile, Bolivia, northern India (particularly the Gangetic plain), South Korea, and Eastern Europe — regions with elevated gallstone prevalence and exposure to specific environmental carcinogens. Women are affected 2–3 times more frequently than men, likely reflecting higher gallstone prevalence. Most cases are discovered incidentally during or after laparoscopic cholecystectomy performed for presumed symptomatic gallstone disease — a scenario that carries important implications for the need for re-resection. Molecular profiling identifies actionable alterations in FGFR2, IDH1, ERBB2, KRAS, and BRCA1/2 that guide systemic therapy selection.
Causes & Risk Factors
Gallstones (cholelithiasis) are present in approximately 90% of gallbladder cancer patients and represent the dominant risk factor, likely through chronic mucosal irritation and inflammation promoting carcinogenesis. Large gallstones (greater than 3 cm diameter) confer a 10-fold higher risk than smaller stones. Additional independent risk factors include gallbladder polyps greater than 1 cm (particularly sessile or rapid-growing polyps), porcelain gallbladder (associated with cancer in a subset of patients, particularly in the setting of selective mucosal calcification), anomalous pancreaticobiliary ductal junction (APBDJ) causing bile-pancreatic juice reflux into the gallbladder, chronic typhoid carrier state (Salmonella typhi in the bile), primary sclerosing cholangitis, obesity, and exposure to industrial carcinogens in rubber, textile, and petrochemical industries. Adenomatous polyps with dysplastic changes, rather than simple cholesterol polyps, carry the highest malignant potential.
Symptoms & Signs
Most early gallbladder cancers are asymptomatic and discovered incidentally during cholecystectomy for what was presumed to be benign gallstone disease. When symptomatic, right upper quadrant abdominal pain (often initially attributed to biliary colic) is the most common presenting symptom. Progressive jaundice, nausea and vomiting, anorexia, and significant unintentional weight loss indicate more advanced disease with biliary obstruction or peritoneal spread. A palpable right upper quadrant mass suggests locally advanced disease. In patients with advanced disease obstructing the common bile duct or hepatic hilum, painless progressive jaundice with dark urine and pale stools develops. Acute cholangitis from biliary obstruction causes fever, rigors, and jaundice. Peritoneal carcinomatosis causes ascites.
Diagnosis & Staging
Abdominal ultrasound is the initial imaging modality, revealing gallbladder wall thickening, mucosal irregularity, a polypoid intraluminal mass, or gallstones with associated mass. CT abdomen with contrast characterizes local extension, hepatic invasion, and regional lymphadenopathy. MRI with MRCP provides superior assessment of biliary involvement and vascular encasement. Elevated serum CA 19-9 and CEA support the diagnosis but lack specificity. ERCP or PTC allows biliary drainage and brush cytology in cases with biliary obstruction. AJCC 8th edition TNM staging: T1a (invasion of lamina propria), T1b (invasion of muscular layer), T2 (invasion of perimuscular connective tissue), T3 (perforation of visceral peritoneum or direct invasion of liver or one adjacent structure), T4 (invasion of main portal vein or hepatic artery or two or more extrahepatic structures). Molecular tumor profiling — FGFR2 fusions, IDH1/2 mutations, ERBB2/HER2, MSI/MMR, BRCA1/2 — guides systemic therapy in unresectable or metastatic settings.
Treatment Options
Surgical resection is the only curative treatment. Simple cholecystectomy is curative for T1a tumors (lamina propria invasion only) discovered incidentally, achieving greater than 95% 5-year survival. T1b tumors are managed with simple cholecystectomy at most centers, though extended resection is considered at some high-volume hepatobiliary centers. T2 tumors require extended cholecystectomy with 2 cm hepatic margin resection plus regional (hepatoduodenal ligament) lymphadenectomy to address the perimuscular connective tissue and draining lymph nodes. T3–T4 disease may require major hepatectomy. Port-site excision is recommended for cases where laparoscopic cholecystectomy was performed before gallbladder cancer was known. Adjuvant capecitabine improves survival after resection. Unresectable or metastatic disease: gemcitabine plus cisplatin plus durvalumab (TOPAZ-1, median OS approximately 12.9 months). FGFR2 inhibitors, IDH1 inhibitor (ivosidenib), and trastuzumab-based combinations are available for molecularly selected tumors.
Prognosis & Outlook
Five-year survival by AJCC T-stage: T1a greater than 95%, T1b approximately 80%, T2 approximately 50–70%, T3 approximately 25–40%, T4 less than 10%, and metastatic disease less than 5%. Median survival for unresectable disease is approximately 11–13 months with gemcitabine-cisplatin-durvalumab. Nodal involvement substantially worsens prognosis — N1 disease reduces 5-year survival by approximately 30–40% compared to node-negative disease. Incidentally discovered T1a/b cancer has the best outcome; advanced disease discovered because of symptoms carries very poor prognosis due to the natural history of minimal early symptoms. Timely cholecystectomy for symptomatic cholelithiasis eliminates the primary gallbladder cancer risk factor. Dietary modification to reduce gallstone formation — reducing saturated fat intake, increasing dietary fiber, achieving and maintaining a healthy weight — decreases gallstone incidence. Prophylactic cholecystectomy is recommended for gallbladder polyps larger than 10 mm (high malignant potential), gallbladder polyps with associated gallstones, any polyp showing rapid growth on serial ultrasound, and all symptomatic polyps. Anomalous pancreaticobiliary junction (APBDJ) warrants prophylactic cholecystectomy and biliary reconstruction. Treatment of chronic Salmonella typhi (typhoid) carriers with fluoroquinolones eliminates biliary colonization. In high-incidence populations (northern India, Chile), ultrasound surveillance programs for high-risk individuals (women over 40 with gallstones, positive family history) may improve stage at detection.
When to See a Doctor
Seek prompt evaluation for right upper quadrant abdominal pain that is new, progressive, or distinct from previous biliary colic episodes — particularly if accompanied by weight loss, appetite changes, or jaundice. Progressive yellowing of the skin or eyes combined with dark urine and pale stools indicates biliary obstruction and requires urgent imaging and liver function testing. Fever with jaundice and right upper quadrant pain (Charcot's triad) is a medical emergency indicating acute cholangitis, requiring hospitalization and urgent biliary decompression. Patients found to have a gallbladder polyp on ultrasound should have its size documented and follow-up imaging scheduled — polyps greater than 10 mm require surgical evaluation. Any rapid growth in a known gallbladder polyp on serial ultrasound examinations necessitates prompt surgical consultation. Incidental gallbladder cancer findings at pathology after cholecystectomy require immediate review by a hepatobiliary surgeon to determine whether re-resection is needed for T1b or greater disease.
