Overview: Oropharyngeal Cancer
Oropharyngeal cancer arises from the squamous epithelium of the tonsils, base of tongue, soft palate, uvula, and posterior pharyngeal wall. It is the most rapidly increasing head and neck cancer in developed countries, driven by an epidemic of HPV-16 infection transmitted through sexual contact, particularly in younger non-smoking males. Globally, oropharyngeal cancer now accounts for the largest proportion of HPV-attributable malignancies. In the United States, the incidence of HPV-positive oropharyngeal SCC has increased by approximately 225% since the 1980s. Two biologically and prognostically distinct forms exist: HPV-positive oropharyngeal SCC (predominantly affecting younger, non-smoking adults with excellent prognosis and 5-year survival exceeding 80%) and HPV-negative SCC (tobacco- and alcohol-driven, with behavior resembling other head and neck SCCs and poorer prognosis of approximately 40-50% at 5 years). The AJCC 8th edition (2017) separately stages these two distinct diseases, reflecting their fundamentally different biological behavior.
Causes & Risk Factors
HPV-16 infection is the dominant carcinogen in HPV-positive oropharyngeal SCC, responsible for approximately 70-80% of oropharyngeal cancers in the United States and other high-income nations. HPV is transmitted through oral sexual contact, and increasing numbers of sexual partners are a recognized risk factor. HPV-positive oropharyngeal cancer represents a distinct epidemiological phenomenon from tobacco-related SCC, affecting a younger, healthier patient population. For HPV-negative oropharyngeal SCC, tobacco smoking (especially heavy and prolonged use) and heavy alcohol consumption remain the primary carcinogens with a well-established synergistic effect. Chewing tobacco and betel quid contribute to risk in South Asian populations. Immunosuppression increases susceptibility to persistent HPV infection. Smoking history worsens prognosis even in HPV-positive disease, with each pack-year of tobacco use associated with incremental mortality increase.
Symptoms & Signs
Oropharyngeal cancer is frequently first identified by the patient or a clinician as a painless neck mass representing metastatic cervical lymphadenopathy — a presentation that is characteristic particularly of HPV-positive disease, where cystic or necrotic tonsillar nodal metastases may be large at initial presentation with a clinically occult primary. Other common presenting symptoms include persistent sore throat disproportionate to visible infection, unilateral otalgia (referred ear pain via the Jacobson nerve), voice change or muffled voice, difficulty swallowing or the sensation of a lump in the throat (globus), and blood in saliva. Trismus suggests pterygoid muscle invasion. Weight loss and fatigue accompany locally advanced disease. Patients with an unexplained unilateral cystic neck mass — especially a young non-smoking male — should be evaluated for HPV-positive oropharyngeal cancer as the primary diagnosis.
Diagnosis & Staging
Oropharyngeal cancer is diagnosed by biopsy of the primary site or an accessible neck node with FNA cytology. p16 immunohistochemistry (a surrogate marker for transcriptionally active HPV infection) is standard practice and required for AJCC 8th edition staging — p16-positive tumors are staged using a more favorable staging system reflecting their better prognosis. Confirmatory HPV testing by in situ hybridization or PCR is recommended when p16 result is ambiguous. CT and MRI of the neck define primary tumor extent, skull base involvement, and nodal characteristics. PET/CT is standard for locoregional and distant staging. Examination under anesthesia (EUA) with directed biopsies is performed for occult primary workup. AJCC 8th edition staging separately classifies p16-positive (HPV-associated) and p16-negative tumors.
Treatment Options
For HPV-positive oropharyngeal cancer, concurrent cisplatin-based chemoradiotherapy with intensity-modulated radiation therapy (IMRT at 70 Gy) or transoral robotic surgery (TORS) followed by adjuvant therapy based on pathological findings are the two established standard treatment pathways — selected based on tumor volume, anatomical site, and patient functional status. TORS achieves excellent functional outcomes (swallowing, speech) with lower long-term toxicity for selected T1-T2 tumors. ECOG 3311 and ORATOR trials investigate treatment de-escalation for HPV-positive disease, including reduced radiation doses (50-60 Gy) and substitution of less toxic agents for cisplatin. HPV-negative oropharyngeal SCC is managed with surgery followed by adjuvant chemoradiation for high-risk pathological features, or definitive chemoradiation with high-dose cisplatin (100 mg/m² every 21 days). Immunotherapy (pembrolizumab, nivolumab) is approved for recurrent/metastatic disease after platinum failure.
Prevention & Screening
HPV vaccination with Gardasil 9 (covering HPV types 16, 18, 31, 33, 45, 52, and 58) before sexual debut provides robust protection against the HPV types responsible for most oropharyngeal cancers. ACIP recommends routine vaccination at ages 11-12 with catch-up through age 26; adults aged 27-45 may receive vaccination after shared decision-making with their physician. Expanding HPV vaccination coverage in adolescent populations is expected to significantly reduce future oropharyngeal cancer incidence. Tobacco cessation reduces risk of HPV-negative oropharyngeal SCC and improves outcomes even in HPV-positive disease. No established population-level screening program exists for oropharyngeal cancer. Patients with HIV or other immunosuppressive conditions should maintain optimal immune function and receive HPV vaccination when eligible.
When to See a Doctor
A painless lump in the neck lasting more than two to three weeks — especially in a person aged 35-60 — requires urgent referral to an ENT or head and neck surgeon for evaluation, even without other throat symptoms. Persistent sore throat, difficulty swallowing, or unexplained one-sided ear pain for more than three weeks without an obvious cause warrants prompt specialist assessment. Voice change, muffled voice, or the sensation of something stuck in the throat not improving with conservative measures should also be evaluated. Patients who are young, non-smokers, or lack typical risk factors should not allow this to delay evaluation — HPV-positive oropharyngeal cancer predominantly affects this demographic. Any suspicious finding should include HPV status testing, as it fundamentally changes staging, treatment planning, and prognosis counseling.
Prognosis & Outlook
HPV-positive oropharyngeal cancer: 5-year survival approximately 80-85%, significantly better than HPV-negative tumors (~40-50%). De-escalation strategies aim to maintain excellent outcomes while reducing treatment toxicity. Smoking history worsens prognosis even in HPV-positive disease. Recurrent or metastatic disease is managed with pembrolizumab or nivolumab, with some patients achieving durable responses.
