Overview
Hair thinning — the gradual reduction in hair shaft diameter (miniaturization), density, and coverage — is one of the most common dermatological complaints worldwide, affecting millions of people across all ages and genders. It ranges from mild diffuse reduction in hair volume to severe, visible scalp exposure. The two most common causes are androgenetic alopecia (AGA, pattern hair loss — a genetically programmed, androgen-mediated miniaturization of hair follicles) and telogen effluvium (TE — diffuse shedding triggered by a systemic physiological stressor causing premature shift of follicles from anagen growth phase to telogen resting phase). AGA affects approximately 50% of men by age 50 and 40% of women by age 70, following the Hamilton-Norwood scale in men and Ludwig scale in women. While hair thinning rarely indicates life-threatening disease, it causes significant psychological distress, reduced self-esteem, and social anxiety. Thorough evaluation is important to exclude treatable underlying causes (thyroid disease, iron deficiency, nutritional deficiencies, medications) before committing to long-term treatment strategies.
Causes and Risk Factors
The most common causes of hair thinning are: (1) Androgenetic alopecia (AGA): genetically determined sensitivity of hair follicles to dihydrotestosterone (DHT), causing follicular miniaturization over years. The 5-alpha reductase enzyme converts testosterone to DHT; AR (androgen receptor) gene polymorphisms on the X chromosome increase susceptibility. Familial clustering is strong; both maternal and paternal inheritance contribute. (2) Telogen effluvium (TE): acute or chronic shedding triggered by thyroid dysfunction (hypothyroidism or hyperthyroidism), iron deficiency or iron deficiency anemia (ferritin below 30–70 ng/mL is a common and treatable cause), protein malnutrition, significant weight loss, crash dieting, post-partum hormonal change (postpartum alopecia, typically 3–6 months after delivery), major surgery, severe illness, fever, or chronic psychological stress. (3) Medications: hair-thinning drugs include retinoids, anticoagulants (heparin, warfarin), antithyroid drugs, anticonvulsants, ACE inhibitors, beta-blockers, and occasionally hormonal contraceptives. (4) Nutritional deficiencies: iron, ferritin, zinc, biotin, selenium, and vitamin D deficiencies are associated. (5) Autoimmune and inflammatory: alopecia areata (patchy rather than diffuse); lichen planopilaris and frontal fibrosing alopecia in cicatricial (scarring) patterns. (6) Endocrine: PCOS with hyperandrogenism causes female pattern thinning.
Symptoms
Symptoms of hair thinning vary by underlying cause. Androgenetic alopecia in men presents as a receding frontal hairline and bitemporal recession, followed by vertex thinning, progressing over years to decades (Hamilton-Norwood grade I–VII). In women, AGA presents as diffuse thinning over the crown and top of the scalp with preservation of the frontal hairline (Ludwig grade I–III) — often noticed as a widening central parting. Telogen effluvium classically presents as diffuse hair shedding — increased hairs on the pillow, in the shower, and on the brush — typically 2–4 months after the triggering event. The hair pull test (grasping 50 hairs and pulling gently) yields >6 telogen (club) hairs in active TE. Scalp trichoscopy (dermoscopy of the scalp) is a non-invasive bedside tool revealing follicular units, hair shaft diameter variation (>20% variability is consistent with AGA), perifollicular erythema, or yellow dots. Symptoms of underlying causes may accompany hair thinning: fatigue and cold intolerance in hypothyroidism; excessive sweating and palpitations in hyperthyroidism; pallor and breathlessness in iron deficiency anemia; acne, irregular periods, and hirsutism in PCOS.
Diagnosis
Diagnosis requires clinical history (duration, pattern, shedding vs. thinning, associated symptoms, medication review, family history, recent stressors), trichoscopic examination (dermoscopy of the scalp), and targeted laboratory investigations to exclude systemic causes. Laboratory workup for diffuse thinning includes: full blood count (CBC), serum ferritin (key — levels below 30–70 ng/mL implicate iron deficiency as a contributor), thyroid function tests (TSH, free T4), zinc, vitamin D, and B12 levels. In women with features of hyperandrogenism (irregular periods, hirsutism, acne), additional tests include free and total testosterone, DHEAS, prolactin, and LH/FSH ratio. Scalp biopsy (4 mm punch, preferably with horizontal sections stained with H&E and special stains) is the gold standard diagnostic tool when the diagnosis is uncertain — it quantifies the terminal:vellus hair ratio (normal >7:1; AGA <4:1), identifies the presence or absence of fibrosis, and detects inflammation consistent with alopecia areata or cicatricial alopecia. Trichogram (microscopic analysis of plucked hairs) and trichoscan (phototrichogram, automated hair density quantification) are used in specialist centers for objective monitoring.
Treatment
Treatment is tailored to the underlying cause. For AGA — men: topical minoxidil 5% solution or foam (applied to the scalp once or twice daily) is the first-line treatment, slowing hair loss and stimulating regrowth in 60–70% of users; oral finasteride 1 mg daily (5-alpha reductase inhibitor blocking DHT production) is equally or more effective, with clinical response seen at 6–12 months, and is approved by FDA for male AGA. Low-level laser therapy (LLLT) devices (650 nm, FDA-cleared) provide additional modest benefit. For AGA — women: topical minoxidil 2% or 5% is FDA-approved for female AGA; oral spironolactone (50–200 mg/day, antiandrogen) is widely used off-label for female pattern hair loss; oral minoxidil at low doses (0.25–1 mg/day) has emerging evidence for women. Hormonal therapy (combined OCP with antiandrogen properties, e.g., cyproterone acetate-containing pills) is used in women with hyperandrogenemia. For telogen effluvium: treat the underlying cause — iron supplementation for iron deficiency (target ferritin >70 ng/mL), thyroid hormone replacement or antithyroid therapy, correction of nutritional deficiencies. TE is typically self-limiting within 6–12 months of trigger resolution. Platelet-rich plasma (PRP) therapy — autologous PRP injected into the scalp every 4–6 weeks — has evidence supporting benefit for AGA in multiple randomized trials. Surgical hair transplantation (follicular unit extraction, FUE; or follicular unit transplantation, FUT strip) is the most effective and permanent option for advanced AGA in appropriate candidates.
Prognosis and Outlook
The prognosis for hair thinning depends substantially on the underlying cause and how early treatment is initiated. Telogen effluvium triggered by identifiable and treatable causes — iron deficiency, thyroid disease, postpartum hormonal changes, crash dieting, or medication — carries an excellent prognosis, with spontaneous hair density restoration occurring within 6–12 months after the triggering factor is corrected and nutritional deficiencies addressed. Serum ferritin levels should be targeted above 70 ng/mL and thyroid function fully normalized for optimal hair recovery. Androgenetic alopecia (AGA) is a progressive, lifelong condition without a permanent cure, but early intervention with medical therapy can significantly alter the long-term trajectory. Topical minoxidil stabilizes or reverses early-stage AGA in approximately 60–70% of consistent users; oral finasteride 1 mg achieves stabilization in over 80% of men, with modest regrowth in many, and must be used indefinitely as hair loss resumes upon discontinuation within 12 months. Platelet-rich plasma (PRP) injections provide meaningful adjunctive benefit with a favorable safety profile. Hair transplantation (follicular unit extraction, FUE; or follicular unit transplantation, FUT strip) provides permanent, natural-appearing hair restoration in appropriately selected candidates with sufficient donor hair density — transplanted follicles are androgen-resistant and do not miniaturize. Alopecia areata carries a variable prognosis: spontaneous remission within 12 months occurs in approximately 50% of those with limited patchy disease, while extensive alopecia totalis or universalis has a much lower spontaneous remission rate and requires specialist immunomodulatory therapy. Regular monitoring and adjustment of treatment strategies are recommended, as individual responses to therapy vary and the underlying process is dynamic.
Prevention and Scalp Health
While genetic androgenetic alopecia cannot be prevented, early treatment initiation slows progression and maximizes hair retention — early intervention before significant miniaturization gives superior long-term outcomes. Optimization of nutritional status — maintaining adequate ferritin (>70 ng/mL), vitamin D (>50 nmol/L), and zinc — supports hair health. Protein intake of at least 1 g/kg/day from complete protein sources supports hair shaft synthesis. Avoidance of crash dieting, extreme weight loss, and prolonged caloric restriction prevents telogen effluvium. Gentle hair care practices — avoiding excessive heat styling, chemical treatments (bleaching, perming), tight hairstyles (traction alopecia), and harsh shampooing — reduce mechanical hair shaft damage. For women with PCOS, treatment of the underlying hormonal disorder reduces AGA progression. Stress management through regular exercise, sleep hygiene, and psychological support helps minimize stress-related effluvium. UV protection of the scalp (hats or sunscreen) is important after hair thinning exposes the scalp, preventing UV damage to follicles and skin.

Hair transplant candidacy should be assessed by a specialist dermatologist or trichologist after at least 12 months of stable disease on medical therapy, as premature surgery may not provide lasting results if the underlying process continues.
