Dr. Karma Wangchuk

Dr. Karma Wangchuk is a consultant physician at Jigme Dorji Wangchuck National Referral Hospital (JDWNRH) in Thimphu — Bhutan's main national referral hospital. With 13 years of internal medicine experience, he provides comprehensive adult medical care within Bhutan's universal health system.

Dr. Karma trained at Khesar Gyalpo University of Medical Sciences of Bhutan before obtaining his MD and MRCP (UK). Non-communicable diseases including diabetes and hypertension are rapidly rising in Bhutan as the country urbanises, and his subspecialty focus reflects this evolving health burden.

His practice covers the full range of adult internal medicine, including both the chronic disease management typical of Thimphu's urban population and the infectious disease challenges (tuberculosis, respiratory infections) more common in rural areas. He is one of Bhutan's senior physicians and contributes to postgraduate medical training at JDWNRH.

For highly complex cases requiring subspecialist investigation or intervention beyond Bhutan's capacity, he coordinates referrals to India — primarily Vellore, Delhi, and Kolkata.

Dr. Karma earned his MBBS from Khesar Gyalpo University of Medical Sciences of Bhutan — the country's national medical university, established in 2012 to reduce dependence on overseas medical training for Bhutanese doctors. He completed his MD in Internal Medicine through postgraduate training at JDWNRH in Bhutan and at affiliated institutions in India, gaining comprehensive training across the spectrum of adult internal medicine.

He obtained the MRCP (UK) — Membership of the Royal Colleges of Physicians — a rigorous international examination that recognizes advanced clinical competency in internal medicine. Achieving MRCP reflects exceptional academic dedication for a Bhutanese physician who must prepare largely through self-directed study. He has participated in continuing medical education through Bhutan Medical and Health Council programs, WHO NCD management modules, and international partner organizations supporting NCD programme development in Bhutan.

Dr. Karma's practice covers the full spectrum of adult internal medicine relevant to both Bhutan's rapidly urbanising population and the ongoing infectious disease challenges in rural areas. In chronic disease management, he provides structured diabetes mellitus care — including insulin initiation and intensification, HbA1c monitoring, and diabetic foot assessment and wound care — alongside hypertension management, dyslipidaemia, and hypothyroidism as part of comprehensive NCD care.

He manages chronic kidney disease with conservative management strategies and coordinates referrals to India for patients requiring dialysis initiation — Bhutan has very limited renal replacement therapy capacity. He manages pulmonary tuberculosis — which remains an important diagnosis in Bhutan — and community-acquired pneumonia. His procedural skills include diagnostic and therapeutic pleural tap, ascitic paracentesis, lumbar puncture for meningitis diagnosis, and bone marrow aspiration for haematological assessment.

Dr. Karma has contributed to the Bhutan Health Journal and participates in Bhutan Medical and Health Council continuing education programs. His interests include diabetes epidemiology in Bhutan's changing population and NCD management in resource-limited settings.

Dr. Karma sees international patients and tourists at JDWNRH in Thimphu. Bhutan attracts high-value international tourism from visitors who pay a daily sustainable development fee, and some international tourists and trekkers require medical attention during their stay — from altitude illness assessment to exacerbations of pre-existing conditions or acute infections. Dr. Karma is experienced in assessing and managing these presentations.

He consults in Dzongkha, English, and Hindi — making him accessible to the significant numbers of Indian tourists and South Asian visitors who travel to Bhutan. Video consultations are available for follow-up and for visitors who have returned home after receiving initial treatment at JDWNRH. For complex specialist investigations or interventions not available in Bhutan, he coordinates referrals to India.

Dr. Karma has been recognized by JDWNRH for his substantial contributions to internal medicine — both through direct patient care and through his role in training junior physicians in the management of complex adult medical presentations at Bhutan's national referral hospital.
