Dr. Joseph Nahnpei

Dr. Joseph Nahnpei is a senior medical officer at Pohnpei State Hospital, the principal healthcare facility for Pohnpei — the most populous of the Federated States of Micronesia (FSM) — and serves as one of FSM's most experienced general practitioners. With 11 years of clinical experience, Dr. Nahnpei provides comprehensive primary and secondary care for Pohnpei's population across a broad range of medical conditions.

The Federated States of Micronesia faces a severe NCD epidemic: the FSM has some of the world's highest rates of type 2 diabetes, hypertension, and obesity, alongside significant rates of tuberculosis and dengue fever. Dr. Nahnpei's practice addresses both this NCD burden and the communicable disease challenges in a setting where specialist care is extremely limited — most specialist procedures require referral to Guam, Hawaii, or the Philippines.

Dr. Nahnpei is bilingual in English and Pohnpeian, the primary indigenous language of Pohnpei island. His linguistic and cultural connection to the Pohnpeian community makes him an effective and trusted clinician and health educator.

Dr. Nahnpei completed his MD at the University of Hawaii John A. Burns School of Medicine — a common medical training pathway for Micronesian medical graduates, reflecting FSM's Compact of Free Association with the United States. He holds a Diploma in Primary Care and ACLS (Advanced Cardiac Life Support) certification.

He has completed WHO training in tuberculosis management and participated in NCD management training through the Pacific Community (SPC) and FSM Department of Health. He has completed clinical attachments at Guam Regional Medical City and participates in the Pacific Open Learning Health Net (POLHN) continuing medical education programme.

Dr. Nahnpei's clinical expertise centres on the management of NCDs, particularly type 2 diabetes and hypertension, which together represent the largest portion of his clinical workload. He manages diabetes comprehensively, including insulin initiation, diabetic complication monitoring, and the management of diabetic foot disease — a significant cause of limb loss in FSM.

For infectious diseases, Dr. Nahnpei manages tuberculosis (applying FSM national TB protocols), typhoid fever, and dengue fever. He provides emergency stabilisation for acute presentations including chest pain, stroke, and trauma, with transfer coordination to Guam or the Philippines for specialist intervention. He also provides antenatal care and childhood immunisation.

Dr. Nahnpei is dedicated to serving Micronesia's communities and improving health outcomes in a nation where the NCD burden is among the world's highest. He has contributed to the FSM NCD STEPS Survey and presented clinical findings at the FSM Medical Association Annual Meeting and Pacific NCD Forum.

He collaborates with the FSM Department of Health and Social Affairs, WHO FSM Country Office, and the Pacific Community (SPC) on NCD surveillance and prevention. He contributes to FSM's national diabetes and tuberculosis control programmes.

Pohnpei State Hospital serves as the primary referral facility for Pohnpei and receives patients from FSM's outer islands. Specialist care is typically referred to Guam (Guam Regional Medical City or Guam Memorial Hospital), Hawaii, or the Philippines through FSM's medical referral programme, given the limited specialist resources in FSM.
