Dr. Hélder Macuácua

Dr. Hélder Macuácua is a consultant physician in the Department of Internal Medicine at Hospital Central de Maputo (HCM) — Mozambique's principal public hospital, the country's largest healthcare facility, and the primary tertiary referral centre for all of Southern Mozambique. With 10 years of clinical practice, Dr. Macuácua provides comprehensive internal medicine services in one of sub-Saharan Africa's most resource-challenged healthcare environments.

Mozambique's disease burden is shaped by high prevalence of HIV (approximately 12% in adults nationally, higher in Maputo), tuberculosis — often HIV-associated — malaria, and parasitic diseases. Simultaneously, non-communicable diseases including hypertension, diabetes, and chronic kidney disease are increasing. Dr. Macuácua's internal medicine practice addresses both: managing HIV and opportunistic infections while also attending to the growing burden of metabolic and urological disease.

Urological conditions — including urinary tract infections, urinary retention from prostatic disease, renal failure, and kidney stones — form a significant part of his medical caseload, given the limited availability of dedicated urology specialists in Mozambique. He manages urological medical conditions and performs emergency catheterisation and suprapubic procedures, coordinating with surgical colleagues for cases requiring operative intervention.

Dr. Macuácua completed his medical degree at the Universidade Eduardo Mondlane (UEM) Faculty of Medicine in Maputo — Mozambique's national university and the country's principal medical training institution. He completed his specialty in Internal Medicine at UEM and Hospital Central de Maputo, training under senior physicians in the challenging multimorbid patient environment of Mozambique's largest hospital.

He subsequently undertook advanced training in HIV medicine at Fiocruz (Fundação Oswaldo Cruz) in Brazil — a leading public health and tropical medicine research institution — deepening his expertise in antiretroviral therapy, HIV-associated co-infections, and immunological monitoring. He is registered with the Ordem dos Médicos de Moçambique (OMM) and is a member of the African Federation of Internal Medicine (AFIM).

Dr. Macuácua's internal medicine practice at HCM encompasses the full spectrum of medical conditions presenting in Mozambique's capital city. His HIV medicine expertise includes initiation and monitoring of first- and second-line antiretroviral regimens, management of HIV-related opportunistic infections (Pneumocystis pneumonia, cryptococcal meningitis, CMV disease, toxoplasmosis), and the immune reconstitution inflammatory syndrome (IRIS).

For urological conditions, he manages urinary tract infections — including complicated UTIs in diabetic and immunocompromised patients — urinary retention from prostatic hyperplasia requiring catheterisation, and chronic kidney disease with appropriate supportive management. He performs lumbar puncture for CNS infection diagnosis, thoracentesis for pleural effusion, and ascitic tap for decompensated liver disease. He manages tuberculosis within the national DOTS framework and coordinates HIV-TB co-infected patients with the TB programme.

Hospital Central de Maputo serves Mozambique's domestic population and is the primary referral centre for the country. It is accessible to diplomats, NGO workers, and expatriates based in Maputo. Dr. Macuácua consults in Portuguese (the official language of Mozambique), Shangana, Ronga, and basic English.

For complex conditions — including urological surgery, oncology, or tertiary specialist care not available in Mozambique — Dr. Macuácua coordinates referrals to South Africa (Johannesburg, Durban), which is accessible via direct flights from Maputo and has more comprehensive specialist services.
