About Prof. Ferro
Prof. José Ferro is one of Europe's most distinguished cerebrovascular neurologists, holding the professorial chair of neurology at the University of Lisbon Faculty of Medicine and serving as a senior consultant at Hospital de Santa Maria — Portugal's largest and most comprehensive university hospital. His career, spanning more than 32 years, has been defined by a singular focus on the epidemiology, pathophysiology, and treatment of stroke and related cerebrovascular conditions.
He is globally recognised as the world's leading authority on cerebral venous thrombosis (CVT) — a relatively rare but serious form of stroke in which the brain's venous drainage system becomes blocked. The International Study on Cerebral Vein and Dural Sinus Thrombosis (ISCVT), which he designed and led, enrolled over 600 patients from 21 countries across Europe and South America and provided the most comprehensive natural history and outcome data for CVT ever assembled. Published in the New England Journal of Medicine, this landmark study transformed the understanding and management of CVT worldwide and remains one of the most cited papers in cerebrovascular neurology.
At Hospital de Santa Maria, Prof. Ferro leads the stroke unit and cerebrovascular disease programme, providing specialist care for patients with ischemic stroke, hemorrhagic stroke, TIA, CVT, and vascular cognitive impairment. He is the principal architect of Portugal's national stroke network, contributing to the design of telemedicine-based acute stroke care coverage across Portuguese hospitals.
Prof. Ferro is a member of the ESO (European Stroke Organisation) and the WSO (World Stroke Organization), having contributed extensively to international guideline development and stroke education. He has trained many of Portugal's current leading neurologists and is a universally respected figure in European neurovascular medicine.
Education & Training
Prof. Ferro completed his medical degree at the University of Lisbon Faculty of Medicine and pursued specialist neurology training at Hospital de Santa Maria, where he has remained throughout his career. His doctoral research (PhD) examined the mechanisms and clinical manifestations of cerebrovascular disease, laying the foundation for a research career centred on stroke biology and outcomes.
He completed advanced training in stroke neurology and cerebrovascular medicine at leading European centres — including time in the United Kingdom and France — where he was exposed to the emerging field of thrombolysis and organised stroke unit care. He holds the Fellowship of the European Stroke Organisation (FESO) and is a founding contributor to ESO's educational and guideline frameworks. He is a full professor of neurology at the University of Lisbon and has trained successive generations of Portuguese neurologists in cerebrovascular medicine.
Clinical Expertise & Procedures
Prof. Ferro's clinical expertise encompasses the full spectrum of acute and chronic cerebrovascular disease. In the acute setting, he oversees the management of ischemic stroke including intravenous thrombolysis (tPA) and endovascular thrombectomy assessment, hemorrhagic stroke management, and the diagnosis and anticoagulation treatment of cerebral venous thrombosis.
His diagnostic procedures include comprehensive clinical neurological examination, brain MRI and CT interpretation (including diffusion-weighted imaging, MR venography, and CTA), carotid and vertebral artery duplex ultrasound, and transcranial Doppler (TCD) for embolic signal detection and patent foramen ovale (PFO) assessment. In the outpatient setting, he manages stroke prevention through risk factor control, antiplatelet and anticoagulant therapy, and the management of atrial fibrillation as a stroke risk factor. He has a particular subspecialty interest in vascular cognitive impairment and vascular dementia — the cognitive consequences of cumulative cerebrovascular disease.
Research & Publications
Prof. Ferro has authored and co-authored over 300 peer-reviewed publications, with major contributions in Stroke, Lancet Neurology, the New England Journal of Medicine, JAMA Neurology, and the European Journal of Neurology. The ISCVT study — his landmark investigation of cerebral venous thrombosis — is among the most cited papers in cerebrovascular neurology and has been the basis for ESO and AHA/ASA management guidelines on CVT.
He has contributed to European Stroke Organisation guideline writing committees, including guidelines on ischemic stroke management, intracerebral haemorrhage, and cerebral venous thrombosis. His epidemiological research has established the incidence, risk factors, and outcomes of stroke in Portugal, contributing to national public health policy. He has presented at ESO, WSO, and the European Academy of Neurology (EAN) congresses for many years and serves as associate editor of the journal Stroke.
International Patient Services
Hospital de Santa Maria's stroke and cerebrovascular programme at the University of Lisbon is one of Southern Europe's most comprehensive, and regularly provides specialist second-opinion consultations for patients from across Europe, Brazil, Angola, and the wider Portuguese-speaking world. Prof. Ferro consults in Portuguese, English, and Spanish, making him accessible to a wide range of international patients.
For patients who have had a stroke or TIA and are seeking specialist evaluation of their risk factors, imaging findings, or treatment strategy, video consultations are available for case review. EU patients can use their EHIC for eligible treatments under cross-border healthcare directive arrangements. Lisbon is easily accessible by direct flight from across Europe, North America, and Africa, and the hospital's location in the city centre simplifies logistics for patients and accompanying family members.
Awards & Recognition
Prof. Ferro has received the highest honours available in Portuguese and European neurology. The Sociedade Portuguesa de Neurologia (SPN) awarded him its Lifetime Achievement Award for his transformative contributions to neurology in Portugal and internationally. The European Stroke Organisation made him an Honorary Member — one of the most prestigious distinctions in European stroke medicine — in recognition of his lifetime contributions to stroke research and guideline development. The World Stroke Organization recognised him with its Distinguished Service Award for his contributions to global stroke advocacy and education. He is a Fellow of the European Stroke Organisation (FESO) and has served on the ESO's scientific advisory board and guideline writing panels for many years.
