About Atorvastatin

Atorvastatin is a prescription statin medication belonging to the HMG-CoA reductase inhibitor class, available as brand-name Lipitor (Pfizer) and widely as generic atorvastatin calcium. It is one of the most prescribed drugs in the world — in the top 5 globally by prescriptions — and has been FDA-approved since 1996. Atorvastatin works in the liver to significantly lower LDL (low-density lipoprotein, or 'bad') cholesterol and triglycerides while modestly raising HDL (high-density lipoprotein, or 'good') cholesterol. Multiple large randomized controlled trials — including ASCOT-LLA, CARDS, and TNT — have demonstrated that atorvastatin substantially reduces the risk of major cardiovascular events including heart attack, stroke, and cardiovascular death in both primary prevention (people at high risk without prior cardiovascular disease) and secondary prevention (people with established cardiovascular disease). Medical Uses & Indications

Atorvastatin is prescribed for two main purposes: reducing elevated cholesterol levels and preventing cardiovascular events. As a lipid-lowering agent, it treats primary hypercholesterolemia (including heterozygous familial hypercholesterolemia — HeFH), mixed dyslipidemia (elevated total cholesterol, LDL, and triglycerides with low HDL), and homozygous familial hypercholesterolemia (in combination with other LDL-lowering therapies). For cardiovascular risk reduction, atorvastatin is used in primary prevention of cardiovascular disease in adults with multiple risk factors (diabetes, hypertension, smoking), and in secondary prevention following a heart attack (post-MI), stroke, unstable angina, coronary artery disease (CAD), or peripheral artery disease. In secondary prevention it is used at the highest tolerated dose (typically 40–80 mg) regardless of baseline cholesterol. It is also prescribed in type 2 diabetes patients and in those with chronic kidney disease at elevated cardiovascular risk. Prescribers include cardiologists, general practitioners, and endocrinologists.

How It Works

Atorvastatin works by competitively and reversibly blocking HMG-CoA reductase (3-hydroxy-3-methylglutaryl-coenzyme A reductase), the rate-limiting enzyme in the mevalonate pathway — the key biochemical pathway by which the liver synthesizes cholesterol. By inhibiting this enzyme, atorvastatin reduces intrahepatic cholesterol synthesis. In response to this reduced intracellular cholesterol, liver cells upregulate the expression of LDL receptors on their surface — these receptors bind circulating LDL particles from the bloodstream and bring them into the liver for metabolism and excretion. Dosage & Administration

Atorvastatin is taken orally once daily at any time of day, with or without food — unlike some other statins (simvastatin), it does not need to be taken in the evening because of its longer half-life of 14 hours. The typical starting dose is 10–20 mg once daily for primary prevention. For secondary prevention or high cardiovascular risk, higher-intensity statin therapy is recommended: 40–80 mg once daily. For familial hypercholesterolemia: 10–80 mg once daily depending on severity, often combined with ezetimibe. Dose adjustments are made every 4–8 weeks based on LDL-C response and tolerability. Maximum dose is 80 mg once daily. Dose reduction is necessary in patients taking certain interacting drugs (see warnings). In children aged 10–17 years: 10–20 mg once daily with careful monitoring. Atorvastatin is contraindicated in active liver disease and pregnancy. Regular follow-up with lipid panel testing and, initially, liver function testing is required. Side Effects & Precautions

More serious muscle toxicity includes myopathy (muscle inflammation with elevated creatine kinase/CK levels, occurring in less than 1% of users) and rhabdomyolysis (severe muscle breakdown with markedly elevated CK, myoglobinuria — dark brown urine — and acute kidney failure; rare, estimated 1 in 10,000–100,000 patient-years). Risk of muscle toxicity increases with higher doses (80 mg carries significantly more myopathy risk than 10–20 mg), older age, female sex, low body weight, hypothyroidism, alcohol excess, and concurrent use of certain medications. Other common side effects include headache (3–5%), nausea and diarrhea (2–4%), and mildly elevated liver transaminases (AST, ALT — in approximately 1% of patients, usually asymptomatic and reversible). New-onset or worsening type 2 diabetes has been identified as a class effect of statins, particularly at higher doses — risk is relatively small compared to cardiovascular benefit. Rare adverse effects include peripheral neuropathy and cognitive impairment (memory lapses). Atorvastatin is absolutely contraindicated in pregnancy (FDA Pregnancy Category X) due to risk of fetal harm — effective contraception is required in women of childbearing age. It is also contraindicated in breastfeeding. In active liver disease, atorvastatin is contraindicated.

Important Warnings & Drug Interactions

Statins as a class carry a warning for myopathy and rhabdomyolysis — risk is substantially elevated with higher doses, advanced age, hypothyroidism (undiagnosed thyroid disease increases muscle risk), renal impairment, and concurrent use of drugs that increase atorvastatin plasma levels. Report any unexplained muscle pain, aching, weakness, tenderness, or dark brown urine to your doctor immediately — do not wait for the next appointment. Grapefruit juice contains bergamottin and other furanocoumarins that inhibit intestinal CYP3A4, significantly increasing atorvastatin blood levels and therefore the risk of muscle toxicity — avoid grapefruit and grapefruit juice while taking atorvastatin. Major drug interactions that increase atorvastatin levels and myopathy risk: cyclosporine (maximum atorvastatin dose 10 mg), gemfibrozil (avoid combination — use fenofibrate if a fibrate is necessary), clarithromycin and other CYP3A4 inhibitors (ketoconazole, itraconazole, HIV protease inhibitors such as ritonavir — dose limit 20 mg with these agents), colchicine (increased myopathy risk). Rifampicin (rifampin) substantially decreases atorvastatin levels through CYP3A4 induction — may require dose adjustment. Atorvastatin modestly increases digoxin concentrations. Liver function tests (LFTs) should be performed before starting, and if any symptoms of liver problems develop (unusual fatigue, jaundice, abdominal pain, dark urine) during treatment.
