About Ibuprofen
Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) belonging to the propionic acid derivative class. It is one of the most widely used medications globally, available over the counter in low doses (200–400 mg) in most countries under brand names including Nurofen, Advil, and Motrin, and at higher prescription doses for inflammatory conditions. Ibuprofen was first developed in the 1960s and has been on the WHO Model List of Essential Medicines since 1984. It has three primary therapeutic effects: analgesic (pain relieving), antipyretic (fever reducing), and anti-inflammatory — making it versatile for a wide range of conditions. At OTC doses it is appropriate for self-treatment of mild-to-moderate pain and fever. At prescription doses (1600–3200 mg/day) it is used for chronic inflammatory conditions including rheumatoid arthritis and ankylosing spondylitis. FDA-approved and widely available, ibuprofen is also available as a topical gel (for localized musculoskeletal pain with fewer systemic effects) and as an intravenous formulation for hospital use (e.g. patent ductus arteriosus closure in premature neonates).
Medical Uses & Indications
Ibuprofen is used for a wide range of on-label pain, fever, and inflammatory conditions. Common OTC indications include headache, migraine (combined with caffeine), toothache, dental pain, menstrual cramps (primary dysmenorrhea — particularly effective due to its anti-prostaglandin effect), muscle pain (myalgia), back pain, minor sports injuries (sprains, strains), post-exercise soreness, and fever reduction in adults and children from 3 months (weight-based dosing). At prescription doses, ibuprofen treats inflammatory arthritis conditions including rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, juvenile idiopathic arthritis, gout flares, and soft tissue rheumatism. Topical ibuprofen gel (e.g. Nurofen Gel) provides localized anti-inflammatory and analgesic benefit for musculoskeletal injuries with significantly less systemic absorption than oral formulations. Ibuprofen is recommended by GPs, rheumatologists, dentists, and orthopedic surgeons across a range of clinical contexts. It is not effective for neuropathic (nerve) pain.
How It Works
Ibuprofen works by non-selectively and reversibly inhibiting cyclooxygenase enzymes COX-1 and COX-2. These enzymes catalyze the conversion of arachidonic acid (released from cell membranes during tissue injury or infection) to prostaglandins, prostacyclin, and thromboxane A2 — a family of lipid mediators that drive pain sensitization, inflammation, and fever. By blocking prostaglandin synthesis at both the site of injury (peripheral action) and within the central nervous system (central action), ibuprofen reduces peripheral pain sensitization (hyperalgesia), diminishes the inflammatory cascade (reducing vasodilation, edema, and redness), and lowers the fever set point in the hypothalamus. COX-1 inhibition also reduces thromboxane A2 production in platelets, impairing platelet aggregation and increasing bleeding time — unlike aspirin's irreversible effect, ibuprofen's platelet inhibition is reversible, typically resolving 12–24 hours after the last dose. COX-1 normally produces protective prostaglandins that maintain the gastric mucus lining — its inhibition by ibuprofen is why gastrointestinal damage is a key side effect. COX-2 is primarily induced at sites of inflammation, and newer selective COX-2 inhibitors (celecoxib, etoricoxib) spare COX-1 to reduce GI side effects, though they carry elevated cardiovascular risk.
Dosage & Administration
OTC adult dose: 200–400 mg orally every 4–6 hours as needed, with a maximum of 1200 mg per 24 hours. Prescription dose for inflammatory conditions: 400–800 mg every 6–8 hours with food, maximum 3200 mg per 24 hours under medical supervision. Always take ibuprofen with a full glass of water and with food, milk, or an antacid to protect the stomach lining. For children's fever and pain: dose is strictly weight-based — approximately 5–10 mg/kg every 6–8 hours, not recommended under 3 months of age or for premature infants except under medical supervision; use calibrated pediatric oral syringe. Topical ibuprofen gel: apply 4–10 cm of gel to the affected area up to 3–4 times daily, rubbing in gently — do not apply to broken or inflamed skin, eyes, or mucous membranes. Ibuprofen IV (hospital use only): dosing is weight-based and determined by medical staff. For OTC self-treatment, use the lowest effective dose for the shortest duration necessary. If symptoms persist beyond 5 days for pain or 3 days for fever without improvement, consult a doctor.
Side Effects & Precautions
Ibuprofen is well tolerated for short-term use at OTC doses in healthy individuals, but carries important side effect risks particularly with regular or high-dose use. Gastrointestinal side effects are the most common: stomach upset, nausea, indigestion, and heartburn affect approximately 10–20% of users, particularly when taken on an empty stomach. More serious GI risks include peptic ulcer disease and gastrointestinal bleeding — risk is significantly elevated in those with a history of peptic ulcer, those taking aspirin or anticoagulants simultaneously, elderly patients, and with prolonged regular use. Concurrent use of a proton pump inhibitor (omeprazole, lansoprazole) substantially reduces GI toxicity risk in high-risk patients. Kidney effects: NSAIDs reduce renal prostaglandin synthesis, which normally dilates renal afferent arterioles — in patients who are dehydrated, volume-depleted, elderly, or have pre-existing kidney or heart disease, ibuprofen can precipitate acute kidney injury (AKI). Fluid retention and edema can worsen heart failure and hypertension. Cardiovascular risk: regular ibuprofen use is associated with a modest but significant increased risk of heart attack and stroke — FDA black-box warning applies to all NSAIDs. The risk is dose-dependent and duration-dependent, and is highest in patients with pre-existing cardiovascular disease. Ibuprofen should not be used in the third trimester of pregnancy (causes premature closure of the ductus arteriosus and fetal kidney damage). Rare: severe hypersensitivity reactions, Stevens-Johnson syndrome. Ibuprofen should be avoided in aspirin-sensitive patients with NSAID-exacerbated respiratory disease (AERD/Samter's triad).
Important Warnings & Drug Interactions
Ibuprofen carries FDA and MHRA black-box warnings for serious gastrointestinal bleeding and ulceration, and for increased cardiovascular risk (heart attack and stroke). Use the lowest effective dose for the shortest period necessary. Avoid ibuprofen in patients with: active peptic ulcer or GI bleeding; history of aspirin or NSAID-induced asthma or urticaria; severe chronic kidney disease (eGFR below 30); decompensated heart failure; third trimester of pregnancy; and aspirin-sensitive asthma. Ibuprofen blunts the cardioprotective platelet-inhibiting effect of low-dose aspirin (75–100 mg/day) when taken simultaneously — take low-dose aspirin at least 30 minutes before ibuprofen, or use paracetamol instead for pain relief in patients on cardioprotective aspirin. Key drug interactions: warfarin (NSAIDs increase bleeding risk substantially — avoid combination or monitor INR very closely); lithium (NSAIDs raise lithium blood levels — risk of toxicity); ACE inhibitors and ARBs (the 'triple whammy' combination with diuretics significantly increases acute kidney injury risk); methotrexate (NSAIDs raise methotrexate levels — risk of toxicity); SSRIs (combined use increases GI bleeding risk by approximately 3-fold). In patients who need both regular ibuprofen and gastric protection, add omeprazole 20 mg daily.
