Vitamins and supplements are nutritional compounds used to prevent or correct deficiencies in essential micronutrients and to support specific health functions and medical conditions. The major categories include: fat-soluble vitamins (vitamin A — retinol; vitamin D — cholecalciferol/ergocalciferol; vitamin E — tocopherols; vitamin K — phylloquinone/menaquinone), which are absorbed with dietary fat and stored in adipose tissue and the liver; water-soluble vitamins (the B-complex vitamins — B1/thiamine, B2/riboflavin, B3/niacin, B5/pantothenic acid, B6/pyridoxine, B7/biotin, B9/folate/folic acid, B12/cobalamin — and vitamin C/ascorbic acid), which are not stored in significant quantities and require regular dietary intake; essential minerals (calcium, iron, magnesium, potassium, zinc, selenium, iodine, copper, chromium); and omega-3 polyunsaturated fatty acids (EPA and DHA — from oily fish or algal sources). Supplements are the most widely self-used health products globally, with an estimated industry value exceeding $150 billion annually. However, supplements are not regulated with the same rigour as licensed medicines in most countries — quality, potency, and labeling accuracy vary significantly between manufacturers. Routine supplementation in the absence of a diagnosed deficiency or specific medical indication provides no proven health benefit for most micronutrients and may carry risk at high doses. Evidence-based supplementation targeted at identified deficiencies or high-risk populations provides clear clinical benefit. Always obtain blood tests and medical assessment before starting high-dose supplementation.
Medical Uses & Indications
Evidence-based clinical uses of vitamins and mineral supplements are specific and well-defined. Vitamin D and calcium: prevention and treatment of deficiency, osteoporosis, and rickets/osteomalacia — particularly important in elderly adults, housebound individuals, dark-skinned individuals living at high latitudes, those who cover most of their skin for cultural reasons, and those with fat malabsorption syndromes (Crohn's disease, coeliac disease, post-bariatric surgery). Vitamin D is also important for immune function. Iron: treatment of iron-deficiency anemia — the most common nutritional deficiency globally — in menstruating women, pregnant women, infants, and those with malabsorption or chronic blood loss. Folic acid (folate/B9): 400 mcg/day is recommended for all women planning pregnancy and through the first 12 weeks of pregnancy to reduce the risk of neural tube defects (spina bifida, anencephaly) by approximately 70%; 5 mg/day is recommended for high-risk women (diabetes, epilepsy, BMI over 30, previous NTD-affected pregnancy). Vitamin B12: in vegans and strict vegetarians (animal products are the only natural dietary source); in elderly adults with reduced gastric acid production impairing B12 absorption; in those on long-term metformin (which reduces B12 absorption); and in those with pernicious anemia (intrinsic factor deficiency — requiring IM injection or very high-dose oral supplementation). Omega-3 fatty acids (EPA + DHA): for hypertriglyceridemia (high-dose prescription omega-3 — icosapentaenoic acid/EPA-only Vascepa — reduces cardiovascular events in high-risk patients); and for reducing cardiovascular risk as an adjunct to statins in specific high-risk groups. Zinc and vitamin C: modest evidence for reducing severity and duration of the common cold when taken early. Iodine: for thyroid function support in populations with iodine-poor diets. Magnesium: for correction of deficiency (common in those on loop diuretics or PPIs).
How It Works
Each vitamin and mineral class acts through distinct biochemical mechanisms as essential cofactors, coenzymes, or structural components in fundamental metabolic processes. Vitamin D acts as a steroid hormone after activation in the liver (to 25-hydroxyvitamin D — the form measured in blood tests) and kidneys (to 1,25-dihydroxyvitamin D/calcitriol — the active form). Calcitriol binds to the vitamin D receptor (VDR) in intestinal cells, bones, kidneys, and immune cells, regulating calcium and phosphorus absorption and bone mineral deposition (remodeling), and modulating immune function. Iron is an essential component of hemoglobin (in red blood cells — transports oxygen from lungs to tissues) and myoglobin (in muscle). Iron is also part of cytochrome enzymes critical for mitochondrial energy production. Without sufficient iron, red blood cells are small, pale, and carry less oxygen — causing iron-deficiency anemia with fatigue, pallor, breathlessness, and reduced exercise tolerance. Folic acid (vitamin B9) is a precursor to tetrahydrofolate (THF), a critical coenzyme in single-carbon transfer reactions essential for the synthesis of purines and thymidylate — building blocks of DNA. It is therefore essential for cell division and DNA replication during rapid growth, particularly in the rapidly developing neural tube of the early embryo (3–4 weeks post-conception — often before pregnancy is even confirmed). Vitamin B12 (cobalamin) is essential for the conversion of methylmalonyl-CoA to succinyl-CoA (important in energy metabolism) and for the remethylation of homocysteine to methionine (required for myelin synthesis in the nervous system and DNA methylation). B12 deficiency causes megaloblastic anemia (large dysfunctional red blood cells) and irreversible neurological damage (subacute combined degeneration of the spinal cord). Omega-3 fatty acids (EPA and DHA) are incorporated into cell membrane phospholipids and are precursors to anti-inflammatory prostaglandins, thromboxanes, and leukotrienes (series-3 and -5), competing with arachidonic acid-derived pro-inflammatory mediators, thereby reducing systemic inflammation and improving vascular function.
Dosage & Administration
Evidence-based dosing for key supplements. Vitamin D: prevention/general health in adults — 400–2000 IU (10–50 mcg) per day; treatment of deficiency (serum 25-OH vitamin D below 30 nmol/L): 20,000–50,000 IU per week for 8–12 weeks (prescription), then maintenance of 400–2000 IU/day. Maximum safe long-term daily dose: 4000 IU/day for adults. Iron for iron-deficiency anemia: 100–200 mg elemental iron daily in divided doses (e.g. ferrous sulfate 200 mg contains 65 mg elemental iron — 1–3 tablets daily); to be continued for 3–6 months after hemoglobin normalizes to fully replenish body stores. Take on an empty stomach or with vitamin C for optimal absorption; take 2 hours apart from antacids, dairy, calcium, and thyroid medication. Folic acid: 400 mcg/day for preconception and first trimester; 5 mg/day for high-risk groups; 5 mg daily for megaloblastic anemia. Vitamin B12 for deficiency: 1000 mcg oral daily (cyanocobalamin or methylcobalamin) for confirmed dietary deficiency or malabsorption — oral high-dose B12 is as effective as intramuscular injections for most patients except those with severe malabsorption; pernicious anemia: hydroxocobalamin 1 mg IM injection every 3 months for life. Omega-3 fatty acids: 1–4 g of combined EPA + DHA daily (prescription icosapentaenoic acid — EPA-only 4 g/day for proven cardiovascular risk reduction in hypertriglyceridemia). Calcium: 1000–1200 mg/day total intake (dietary + supplement) for osteoporosis prevention in adults over 50. Magnesium glycinate or citrate: 200–400 mg/day. Always consult your doctor before starting supplements at therapeutic or high doses.
Side Effects & Precautions
Side effects of vitamins and supplements vary critically depending on whether they are fat-soluble (potential for accumulation and toxicity) or water-soluble (excess excreted in urine, lower toxicity risk). Fat-soluble vitamins can accumulate to toxic levels. Vitamin A (retinol) toxicity: chronic intake above 3000 mcg/day (10,000 IU/day) causes headache, nausea, liver damage, bone pain, and hair loss. Critically, vitamin A supplements (NOT beta-carotene) are highly teratogenic and must be strictly avoided in pregnancy — even standard doses in retinol-based supplements can cause birth defects. Vitamin D toxicity (hypervitaminosis D): occurs with chronic intake exceeding 10,000 IU/day over months; causes hypercalcemia presenting as weakness, nausea, vomiting, excessive thirst, polyuria, confusion, and kidney stone formation. Vitamin K in excess may interfere with anticoagulant therapy (see interactions). Vitamin E at doses above 400 IU/day has been associated in some studies with increased all-cause mortality and hemorrhagic stroke — avoid supplemental vitamin E above 400 IU/day without medical indication. Water-soluble vitamins are generally safer in excess. High-dose vitamin C (above 2000 mg/day) causes osmotic diarrhea and significantly increases kidney stone (oxalate stones) risk in susceptible individuals. High-dose vitamin B6 (pyridoxine — above 100 mg/day for prolonged periods) causes peripheral sensory neuropathy. High-dose niacin (above 1000–3000 mg/day) causes flushing, nausea, liver toxicity, and hyperglycemia — the flush can be reduced by taking aspirin 30 minutes before. Iron: common side effects at therapeutic doses include constipation (very common — add a stool softener or dietary fiber), dark-colored or black stools (expected — inform patients), nausea, and abdominal cramping. Acute iron overdose is the leading cause of fatal poisoning in children under 6 years — iron tablets must be stored safely out of reach of children, as they look like sweets. Omega-3: fishy aftertaste, fish burps (use enteric-coated formulations to minimize this), loose stools. At high doses (above 3–4 g/day): increased bleeding time.
Important Warnings & Drug Interactions
Supplements are NOT regulated with the same rigour as pharmaceutical medicines in most countries (EU, UK, USA, India, Australia) — they do not require proof of efficacy or safety before sale, and product quality (correct dose, absence of contaminants, bioavailability) varies greatly. Choose third-party tested products certified by independent quality assurance bodies (e.g. USP, NSF International, ConsumerLab in the USA; or pharmaceutical-grade licensed products in the UK). Do not self-diagnose nutritional deficiencies based on symptoms — get a blood test (25-OH vitamin D, serum ferritin, B12, folate, zinc, magnesium) to confirm deficiency before starting high-dose supplementation. Many supplement deficiency symptoms (fatigue, hair loss, mood changes) are non-specific and have multiple causes. Critical drug interactions: vitamin K (from food or supplements) is the principal antagonist of warfarin (vitamin K antagonist anticoagulant) — consistent dietary vitamin K intake is important for INR stability; high-dose vitamin K supplements can render warfarin ineffective and cause dangerous clot formation. Iron significantly reduces the absorption of levothyroxine (thyroid replacement) and bisphosphonates (osteoporosis drugs) — always take iron at least 4 hours away from these medications. Calcium reduces the absorption of iron, levothyroxine, fluoroquinolone antibiotics, and tetracyclines — separate by at least 2 hours. High-dose vitamin E (above 400 IU/day) increases bleeding risk and may potentiate the effect of anticoagulants. High-dose omega-3 supplements (above 3 g/day) have mild antiplatelet activity — discuss with your doctor before surgery or if on anticoagulants. Zinc at doses above 50 mg/day chronically depletes copper stores — causing copper-deficiency anemia and neuropathy. St. John's Wort (a herbal supplement) is a powerful inducer of CYP3A4 and P-glycoprotein, significantly reducing blood levels of many prescription medicines including warfarin, oral contraceptives, SSRIs, antiretrovirals, ciclosporin, digoxin, and statins — avoid with all prescription medicines. Always inform your doctor and pharmacist of all supplements you take — including herbal and 'natural' products.
