A breast biopsy is a definitive diagnostic procedure in which a sample of breast tissue is removed and examined under a microscope by a pathologist to determine whether a breast abnormality is benign or malignant. It is the only test that can conclusively confirm or exclude breast cancer — imaging studies including mammography, ultrasound, and MRI can identify suspicious lesions but cannot provide tissue diagnosis. Several biopsy techniques exist: image-guided core needle biopsy (most common), vacuum-assisted biopsy (VAB), fine needle aspiration cytology (FNAC), and surgical excision biopsy. Image-guided core needle biopsy using ultrasound or stereotactic (mammographic) guidance is now the international standard of care for the initial assessment of breast lesions, replacing surgical biopsy in the majority of cases. The biopsy is performed as an outpatient procedure under local anaesthesia, produces minimal discomfort, and provides sufficient tissue for full histological, hormone receptor (ER/PR), and HER2 testing — information essential for treatment planning in the event of a malignant diagnosis. Mammography-guided stereotactic biopsy and MRI-guided biopsy target lesions visible only on those specific modalities. Breast biopsy is performed by radiologists, breast surgeons, and specialist breast care nurses. Who Needs a Breast Biopsy?

Breast biopsy is indicated for any suspicious breast finding that requires histological characterisation. The primary indications are: BI-RADS category 4 or 5 lesions identified on mammography or ultrasound (representing findings with 15–95% probability of malignancy); a palpable breast lump that is new, growing, or has features suspicious for malignancy; nipple discharge (particularly unilateral, spontaneous, bloody, or clear/watery discharge); skin changes over the breast including peau d'orange, redness, or skin thickening; inflammatory changes that do not resolve with antibiotics; and suspicious enhancement on breast MRI in high-risk surveillance patients. BI-RADS 3 lesions (probably benign, <2% malignancy risk) are generally followed with short-interval imaging (6 months) rather than immediate biopsy unless the patient is high-risk or requests definitive assessment. All suspicious axillary lymph nodes in the context of a breast lesion should also be sampled. Biopsy should not be postponed in the presence of classic clinical or radiological features of malignancy, as delay directly impacts stage and survival outcomes.

How Breast Biopsy Is Performed

The most commonly performed technique is ultrasound-guided core needle biopsy. The patient lies supine with the arm raised. The radiologist or breast surgeon performs a focused ultrasound to localise the target lesion. The biopsy site is cleaned with antiseptic and local anaesthetic (lidocaine 1–2%) is infiltrated into the skin and around the target. Breast Biopsy Accuracy and Benefits

Ultrasound-guided core needle biopsy achieves a sensitivity of 97–99% for breast cancer detection and a specificity exceeding 95%, making it the most accurate non-surgical tissue sampling technique available. The diagnostic accuracy of vacuum-assisted biopsy (VAB) for microcalcifications approaches 99%, significantly superior to FNAC. The major advantage of core needle biopsy over surgical excision biopsy is that it is performed under local anaesthesia as an outpatient procedure, leaving a 5 mm or smaller wound, no general anaesthesia exposure, and same-day return to normal activities. Core biopsy provides sufficient tissue for full immunohistochemistry including oestrogen receptor (ER), progesterone receptor (PR), HER2, and Ki-67 index — enabling neoadjuvant chemotherapy planning before definitive surgery. Accurate preoperative diagnosis reduces the number of surgical procedures a patient requires; most patients with core biopsy-confirmed breast cancer proceed directly to wide local excision or mastectomy without a preceding diagnostic surgical biopsy. The titanium marker clip placed at biopsy enables accurate surgical localisation even if the tumour becomes impalpable after neoadjuvant treatment.

Risks and Complications of Breast Biopsy

Breast core needle biopsy is a safe procedure with a low complication rate. Bruising (ecchymosis) around the biopsy site occurs in 5–15% of patients and resolves over 1–2 weeks. Haematoma formation (blood collection beneath the skin) requiring clinical review occurs in less than 2% of cases. Infection is rare (<0.5%) with standard sterile technique and is treated with oral antibiotics. Needle track seeding — the theoretical risk of cancer cells being displaced along the biopsy needle path — is an extremely rare event with no proven clinical significance in properly performed ultrasound-guided biopsy. False-negative results occur in 1–3% of core biopsies, typically due to technical sampling error, and are identified by radiological-pathological correlation: if the biopsy result (e.g., benign fibrocystic change) does not fit the imaging appearance of the lesion (e.g., an irregular spiculated mass), repeat biopsy or surgical excision is recommended. Vacuum-assisted biopsy of mammographic microcalcifications carries a small risk of complete lesion removal at biopsy, necessitating accurate clip placement for subsequent surgical localisation.

Recovery After Breast Biopsy

Recovery from core needle breast biopsy is rapid. Most patients return home immediately after the procedure and resume normal daily activities including driving and desk work within a few hours. A firm pressure dressing is applied over the biopsy site and should be kept in place for 24 hours to minimise bleeding risk. Ice packs applied over the dressing for 20 minutes at a time reduce bruising and soreness. Mild breast tenderness and bruising are normal and resolve within 5–7 days. Paracetamol or ibuprofen provide adequate analgesia; aspirin should be avoided for 24 hours as it may worsen bleeding. Strenuous upper body exercise (racket sports, weightlifting, swimming) should be avoided for 48 hours. Contact sports and bra-less activity are avoided for 24–48 hours. Patients should report any signs of infection (redness, warmth, increasing pain, discharge from the biopsy site) to their breast care team. Pathology results are typically available within 3–7 working days, and the breast team will contact the patient to discuss findings and arrange follow-up planning.
