What Is Discectomy?
Discectomy is the surgical removal of a herniated, ruptured, or degenerated intervertebral disc fragment that is compressing a spinal nerve root or the spinal cord, causing radicular pain (sciatica in the lumbar spine, arm pain in the cervical spine), numbness, tingling, or limb weakness. The procedure directly removes the source of mechanical and chemical nerve irritation. Microdiscectomy — the current gold-standard technique — performs the same disc removal through a much smaller incision (2–3 cm) using magnification from an operating microscope or loupe system, allowing precise dissection with minimal disturbance of paraspinal muscles, reducing post-operative pain, shortening hospital stay, and accelerating return to function compared with traditional open discectomy. Discectomy is the most commonly performed spinal surgical procedure globally, with excellent evidence of superiority over continued conservative management for radicular symptoms that have failed a minimum 6-week trial of non-operative treatment. Lumbar L4/5 and L5/S1 are the most frequently operated levels; cervical discectomy with fusion (ACDF) is used for cervical radiculopathy. Lumbar microdiscectomy is one of the most commonly performed spinal operations globally, with approximately 500,000 procedures per year in the United States and 15,000 per year in the NHS.
Who Needs This Procedure?
Discectomy is indicated when conservative management — rest modification, physiotherapy, NSAIDs, and epidural steroid injections — has failed to provide adequate relief after a minimum 6 weeks for lumbar disc herniation causing sciatica or radiculopathy. Specific indications include herniated disc confirmed on MRI causing nerve root compression concordant with clinical symptoms (dermatomal pain, myotomal weakness, reduced reflexes), progressive neurological deficit (worsening weakness in a limb), and failure to return to work or normal activities due to uncontrolled radicular pain. Cauda equina syndrome — acute compression of the cauda equina nerve roots causing bladder and bowel dysfunction, perineal numbness, and bilateral leg weakness — is a surgical emergency requiring discectomy within hours to prevent permanent neurological damage. Age is not a barrier: both adolescents with disc herniations and older patients benefit, though degenerated multilevel disc disease in older patients is evaluated differently. Patients with primarily back pain (axial pain) without radiculopathy are poor surgical candidates; discectomy is specifically effective for nerve compression symptoms, not discogenic back pain.
How the Procedure Is Performed
Microdiscectomy is performed under general anaesthesia with the patient positioned prone (face-down) on a Jackson table or Wilson frame to open the posterior elements. A 2–3 cm midline or paramedian skin incision is made over the affected vertebral level, confirmed by intraoperative fluoroscopy. The paraspinal muscles are gently retracted (not cut) to expose the lamina. Using the operating microscope at 4–10x magnification, a small window is created in the ligamentum flavum and, if necessary, the inferior margin of the cephalad lamina is partially removed (laminotomy) using a Kerrison rongeur to provide access to the disc space. The traversing nerve root is gently retracted medially, exposing the disc herniation. The herniated nucleus pulposus fragment is removed using pituitary rongeurs, and any loose disc material within the disc space is curetted. The nerve root is confirmed to be fully decompressed with a dissector before closure. The wound is closed in anatomical layers with absorbable sutures. In cervical disc herniation, the anterior cervical discectomy and fusion (ACDF) approach through the front of the neck is preferred, removing the disc and placing a cage with bone graft to maintain disc height and decompress the nerve. Total operative time for a single-level microdiscectomy is typically 45–90 minutes.
Benefits & Success Rates
Microdiscectomy achieves 80–90% good-to-excellent outcomes for the relief of radicular (leg or arm) pain — superior to continued conservative management in patients with documented nerve compression. The Maine Lumbar Spine Study and SPORT trial (Spine Patient Outcomes Research Trial) demonstrated that surgery provides significantly faster and greater relief of leg pain, better functional status, and greater patient satisfaction at 1–2 years compared with non-operative treatment, though conservative outcomes eventually approach surgical outcomes at 4–10 years in compliant patients. Leg pain (sciatica) responds faster than back pain — many patients experience immediate leg pain relief upon awakening from anaesthesia as the compressed nerve decompresses. Neurological deficits (foot drop, leg weakness) also recover, though motor recovery may take weeks to months depending on the duration and severity of compression. Operating time for uncomplicated microdiscectomy is 45–90 minutes. Hospital stay of 1–2 days versus days-to-weeks of hospital admission for uncontrolled radicular pain represents a significant advantage. Return to light work within 2–4 weeks and full activity within 6–12 weeks allows faster economic and functional recovery.
Risks & Complications
Microdiscectomy has a low overall complication rate. Recurrent disc herniation at the same level is the most common long-term complication, occurring in 5–10% within 2 years; risk factors include early return to heavy manual work, obesity, and smoking. Dural tear with cerebrospinal fluid (CSF) leak occurs in approximately 1–3% and is usually identified and repaired intraoperatively — post-operative bed rest and monitoring prevent complications; re-exploration for persistent CSF fistula is rare. Nerve root injury causing new neurological deficit occurs in less than 1% with meticulous technique. Wound infection occurs in 0.5–2%; diabetics and smokers are at higher risk. Epidural haematoma — post-operative bleeding compressing the nerve roots — is rare (0.1–0.3%) but requires urgent surgical evacuation if it causes progressive neurological deficit. Failed back surgery syndrome — persistent or recurrent pain after technically successful surgery — affects 5–10% of patients, often related to psychosocial factors, adjacent segment disease, or scar formation (epidural fibrosis). Discectomy does not prevent future disc degeneration or herniation at adjacent levels.
Recovery & Aftercare
Most microdiscectomy patients are ambulant on the day of surgery or the following morning and are discharged within 1–2 days once pain is controlled with oral analgesics and independent mobilisation is confirmed. Post-operative instructions: avoid bending, twisting, and lifting more than 5 kg for 6 weeks. Patients may sit, stand, and walk immediately. Return to desk work is typically possible at 2–3 weeks; light manual work at 4–6 weeks; heavy manual labour at 8–12 weeks. A physiotherapy programme beginning at 2–4 weeks post-operatively — focused on core muscle activation, postural correction, and progressive mobilisation — significantly reduces recurrence risk and optimises long-term functional outcomes. Driving is safe once comfortable sitting for 45 minutes and able to perform an emergency stop, typically 2–3 weeks post-surgery (with a manual gearbox, somewhat longer). Swimming is one of the earliest exercise modalities recommended at 4–6 weeks once the wound is healed. The prognosis for radicular pain relief is excellent — the vast majority of patients achieve permanent benefit, though about 1 in 10 experience recurrence within 2 years requiring consideration of repeat discectomy or spinal fusion.
