What Is a Liver Biopsy?

A liver biopsy is a procedure in which a small core of liver tissue — typically one to two centimetres in length and one to two millimetres in diameter, weighing approximately ten to fifty milligrams — is extracted from the liver parenchyma using a hollow needle and submitted for histopathological examination under the microscope by a specialist pathologist. It is the reference standard ('gold standard') investigation for grading the severity of hepatic inflammation (necroinflammatory activity, graded A0–A3 by the METAVIR system) and staging the degree of fibrosis (F0–F4, where F4 represents established cirrhosis) in chronic liver diseases including chronic viral hepatitis B and C, non-alcoholic fatty liver disease (NAFLD, including its progressive form NASH — non-alcoholic steatohepatitis), autoimmune hepatitis, primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC), alcoholic liver disease, haemochromatosis, Wilson's disease, and alpha-1 antitrypsin deficiency. Biopsy also diagnoses space-occupying lesions including hepatocellular carcinoma (HCC), liver metastases, and hepatic adenoma where imaging characteristics are equivocal. The three technical approaches are: percutaneous biopsy (blind or ultrasound-guided) — the most common approach; transjugular liver biopsy (TJLB) — used when coagulopathy or ascites contraindicates the percutaneous route; and laparoscopic or surgical biopsy — used when simultaneous surgical exploration is required or a wedge biopsy for surface lesions is needed.

Who Needs This Procedure?

Liver biopsy is indicated when histopathological information will meaningfully change management and cannot be adequately obtained by non-invasive tests. Primary indications include: chronic hepatitis C to determine the degree of fibrosis in patients where antiviral treatment decisions depend on disease stage — though direct-acting antiviral (DAA) availability has substantially reduced this indication; chronic hepatitis B to assess activity and fibrosis in indeterminate viraemia scenarios guiding treatment initiation; NAFLD and NASH where advanced fibrosis (F2 or above) changes management and non-invasive markers (FIB-4 index, liver stiffness by FibroScan/elastography) are discordant or in the intermediate zone; autoimmune hepatitis (AIH) for diagnosis (interface hepatitis, plasma cells, rosette formation are diagnostic histological features) and monitoring of treatment-induced remission; assessment of liver transplant dysfunction and rejection; evaluation of unexplained hepatomegaly, unexplained transaminase elevation, or cholestasis; and liver mass biopsy for lesions of uncertain nature on imaging. Contraindications include uncorrectable coagulopathy (INR above one point five to two, platelet count below fifty to sixty thousand), severe ascites (use transjugular route instead), suspected haemangioma in the needle path, uncooperative patient, and inability to identify a safe biopsy path by imaging.

How the Procedure Is Performed

Before percutaneous liver biopsy, coagulation tests (INR, platelet count) are reviewed and corrected if abnormal. Aspirin and NSAIDs are stopped five to seven days before; clopidogrel seven to ten days before; warfarin three to five days before with bridging heparin if indicated. Pre-procedure ultrasound defines the optimal biopsy site — typically the right lobe of the liver accessed from the right intercostal approach — and identifies large vessels and bile ducts to be avoided. The patient lies supine or in left lateral decubitus position. The right flank is cleaned with antiseptic solution. Local anaesthetic (lignocaine two percent) is injected through skin, subcutaneous tissue, and intercostal muscles to the liver capsule. Real-time ultrasound guidance is strongly recommended (reduces complication rate by sixty percent compared to blind technique) to confirm needle position before firing. A Menghini aspiration needle (fourteen to sixteen gauge) or semi-automated Tru-Cut biopsy gun (sixteen to eighteen gauge, spring-loaded) is advanced to the liver capsule under ultrasound guidance. The patient is instructed to hold their breath (end-expiration) and the needle is advanced into the liver parenchyma and a tissue core is extracted in one to two seconds. Two passes are typically made to obtain an adequate core length (greater than fifteen to twenty millimetres, containing more than ten portal tracts) for reliable fibrosis staging. The biopsy site is observed by ultrasound for immediate post-procedure haematoma. A pressure dressing is applied and the patient rests on their right side for two hours.

Diagnostic Value & Benefits

Liver biopsy provides irreplaceable histological information that blood tests and imaging cannot fully replicate. It is the only test that simultaneously grades inflammation, stages fibrosis, identifies the pattern of injury (steatosis, lobular inflammation, ballooning degeneration, interface hepatitis), detects copper or iron deposition, and identifies bile duct changes — enabling a specific aetiological diagnosis in many cases. In NAFLD, biopsy distinguishes simple steatosis (NAFLD activity score 0–2) — a benign condition with low fibrosis progression risk — from NASH (NAS 3–8) with significant inflammation and ballooning, which carries a substantially higher fibrosis progression rate and liver-related mortality. This distinction cannot be made reliably by non-invasive means. In autoimmune hepatitis, histology is required for diagnosis, treatment decisions, and confirmation of histological remission before considering treatment withdrawal. A well-performed biopsy with adequate sample size has a reported diagnostic accuracy for cirrhosis of eighty-five to ninety-five percent. The information obtained from biopsy guides treatment choice, determines urgency, enables prognostication, and provides a baseline for monitoring treatment response on repeat biopsy.

Risks & Complications

Liver biopsy is a generally safe procedure when performed by experienced operators using ultrasound guidance, but carries a defined complication profile. Pain at the biopsy site — typically mild to moderate right upper quadrant or right shoulder tip discomfort — occurs in approximately thirty percent of patients and is managed with paracetamol. Significant bleeding is the most serious complication: intra-abdominal haemorrhage requiring transfusion occurs in approximately zero point five to one percent of procedures, and approximately one in two thousand to five thousand biopsies requires intervention (embolisation or surgery) for haemorrhage. Haemobilia (bleeding into the bile duct from a fistula between a vessel and a duct) is rare (one in two thousand procedures) and presents with biliary colic, obstructive jaundice, and gastrointestinal bleeding two to five days post-biopsy. Bile leakage from bile duct puncture occurs in less than one percent and may cause biliary peritonitis. Pneumothorax from intercostal approach occurs in less than one percent. The overall major complication rate is approximately one to three percent, and mortality is approximately one in ten thousand to thirty thousand procedures. Needle track seeding of malignant cells during biopsy of HCC is a historical concern; the current estimated rate is below one percent with modern fine needles and adequate technique. Risk is minimised using the smallest adequate gauge needle with ultrasound guidance.

Recovery & Aftercare

After the biopsy, the patient rests on the right lateral decubitus position in the procedure unit or ward for two to four hours. Vital signs (blood pressure, pulse, pain score) are monitored every thirty minutes for the first two hours, then hourly. Intravenous access is maintained throughout the observation period in case intervention for bleeding is required. Most patients are discharged after four to six hours of uneventful observation with stable observations. Patients must be accompanied home by a responsible adult and must not drive on the day of biopsy. Instructions include: avoid strenuous exercise, heavy lifting, and sports for seven days; avoid aspirin and NSAIDs for five days; paracetamol is safe and recommended for post-biopsy discomfort; and return immediately to hospital if they experience severe right upper quadrant or shoulder pain, increasing abdominal distension, pallor, fainting, fever above thirty-eight degrees, or haemoptysis within forty-eight hours. Biopsy results are typically available within five to seven working days for standard haematoxylin and eosin and Masson's trichrome fibrosis staining, and ten to fourteen days if additional special stains (iron, copper, PAS-D) or immunohistochemistry are required. Results are discussed with the patient by the hepatologist or gastroenterologist in a follow-up appointment.

FibroScan (transient elastography) and MR elastography are now recommended as first-line non-invasive assessments of liver fibrosis for chronic viral hepatitis and NAFLD in major guidelines (EASL, AASLD). They are highly accurate for detecting cirrhosis (F4) and advanced fibrosis (F3–F4), reducing the need for biopsy in patients with clearly normal or clearly abnormal elastography. However, FibroScan cannot distinguish the intermediate fibrosis stages F1–F2 reliably, cannot assess inflammation or steatosis comprehensively, and cannot diagnose autoimmune hepatitis or rare metabolic conditions — so liver biopsy retains an important clinical role for selected patients.
Transjugular liver biopsy (TJLB) is performed by interventional radiologists via a catheter introduced through the internal jugular vein, advanced into the right hepatic vein, and a biopsy needle deployed through the hepatic vein wall into the liver parenchyma. Any bleeding enters the vascular system rather than the peritoneal cavity, making TJLB safe in patients with uncorrectable coagulopathy (INR above 1.5) or significant ascites who cannot safely undergo percutaneous biopsy. Hepatic venous pressure gradient measurement can be performed simultaneously, providing information about portal hypertension severity.
Most patients describe the procedure as uncomfortable rather than severely painful. The local anaesthetic injection produces a brief stinging sensation, after which the skin and subcutaneous tissues are numb.
