What Is the Tilt Table Test?

The tilt table test (head-up tilt test, HUTT) is a non-invasive diagnostic procedure used to investigate unexplained recurrent syncope (fainting) and presyncope by provoking orthostatic haemodynamic changes under controlled, monitored conditions. The patient is secured to a motorised table and tilted from horizontal (supine) to a 60-80 degree upright angle while heart rate and blood pressure are continuously monitored. This passive orthostatic stress challenges autonomic cardiovascular reflexes and can reproduce reflex (vasovagal) syncope or demonstrate orthostatic hypotension, postural orthostatic tachycardia syndrome (POTS), or carotid sinus hypersensitivity in susceptible individuals.

The standard protocol (Westminster protocol, widely used in UK) tilts the patient to 60–70 degrees for 20 minutes in the passive phase, followed by administration of isoproterenol (isoprenaline 1–3 mcg/min IV) or sublingual glyceryl trinitrate (GTN 400 mcg spray) as a pharmacological provocateur if the passive phase is negative, then a further 15–20 minutes of monitoring. GTN is now more widely used than isoproterenol in many European centres as it is simpler to administer and avoids dose titration. POTS (postural orthostatic tachycardia syndrome) — characterised by a heart rate rise of more than 30 beats per minute without syncope on tilting — is a distinct diagnosis identified during the tilt table test.

Who Needs This Procedure?

The tilt table test is indicated for patients with recurrent unexplained syncope or presyncope in whom a cardiac cause has been excluded by echocardiography and 12-lead ECG, and where clinical history suggests a reflex (vasovagal or situational) mechanism. It is also used to evaluate orthostatic hypotension, POTS (particularly in young women with chronic fatigue and tachycardia on standing), carotid sinus hypersensitivity, and autonomic neuropathy due to diabetes or Parkinson's disease. It is not required after a first single syncopal episode with a classical vasovagal prodrome in a young patient with a normal ECG and echocardiogram.

Tilt table testing is most valuable when vasovagal syncope is suspected clinically (young patient, typical prodrome of nausea/sweating/warmth before loss of consciousness, rapid recovery, benign episode) but cannot be confirmed diagnostically, and where confirmation is needed to guide management (e.g., for professional drivers, pilots, or patients with occupational implications). When a structural or primary arrhythmic cause has been identified on preliminary workup (2D echo, Holter monitoring, ECG), the tilt table test is usually unnecessary. Implantable loop recorder (ILR) is preferred for monitoring infrequent (less than monthly) unexplained syncope.

How the Procedure Is Performed

The patient fasts for 4-6 hours before the test and IV access is secured. Continuous 12-lead ECG, beat-to-beat blood pressure (Finapres device or invasive arterial line), pulse oximetry, and symptom monitoring are established. The patient lies supine for 5 minutes to establish baseline values, then the table is tilted to 60-70 degrees for 20-45 minutes (passive phase). If no syncopal response occurs, pharmacological provocation is applied — sublingual glyceryl trinitrate (400 mcg) or intravenous isoproterenol — and tilting continued for a further 15-20 minutes. A positive result is reproduction of syncope or presyncope with the characteristic pattern of bradycardia, hypotension, or both. The table is immediately returned to horizontal when syncope occurs.

The test is performed in a quiet, dimly lit room to minimise sympathetic arousal. Baseline blood pressure, heart rate, and 12-lead ECG are recorded supine for 10 minutes. The tilt is executed mechanically or electronically over 10 seconds. The cardiologist or physiologist observes the patient throughout, monitoring for prodromal symptoms (nausea, sweating, pallor, yawning) that precede vasovagal syncope by 30–120 seconds, during which the patient should be encouraged to perform physical counterpressure manoeuvres (leg crossing, hand gripping) which may abort the episode and demonstrate a useful management strategy.

The tilt table test identifies the cause of syncope in 40-75% of patients with unexplained recurrent syncope who have had inconclusive preliminary investigations. It safely reproduces vasovagal syncope in a controlled environment with immediate resuscitation capability, allowing classification of syncope type (vasodepressor, cardioinhibitory, or mixed Vasovagal International Study [VASIS] classification) to guide treatment decisions. Identification of POTS or orthostatic hypotension enables targeted lifestyle and pharmacological interventions. The non-invasive nature avoids the need for electrophysiology study in the majority of cases. Results directly guide management decisions including lifestyle modification, compression stockings, fludrocortisone, midodrine, or cardiac pacemaker in cardioinhibitory syncope.

For patients with POTS, the tilt table test provides an objective diagnosis, distinguishes POTS subtypes (neuropathic, hyperadrenergic, hypovolaemic), guides treatment selection (fludrocortisone, ivabradine, beta-blockers, increased salt and fluid intake), and provides a baseline for monitoring treatment response. Avoiding the tilt table test in favour of 'wait and watch' may result in prolonged diagnostic uncertainty, unnecessary driving restrictions, recurrent syncopal episodes with injury risk, and delayed institution of effective preventive therapy.

Risks & Complications

The tilt table test is generally very safe. Risk of prolonged syncope or asystole during the passive phase is under 0.1% and immediately reversed by returning to horizontal. The pharmacological provocation phase carries a higher risk of bradyarrhythmia, including prolonged sinus pause or complete heart block (rare). Intravenous isoproterenol can provoke supraventricular or ventricular arrhythmia. Resuscitation equipment (defibrillator, atropine, adrenaline) must be immediately available throughout. The false-positive rate for passive tilt alone is approximately 10% in asymptomatic healthy individuals but falls to 2-5% with GTN provocation. The false-negative rate is 15-30%, meaning a negative test does not exclude vasovagal syncope.

Patients with significant haemodynamic instability, severe aortic stenosis, recent MI, severe LV dysfunction (EF less than 30%), or known severe coronary artery disease should not undergo pharmacological provocation protocols. GTN provocation can cause severe symptomatic hypotension, headache, and flushing lasting 5–15 minutes, which are distressing but self-resolving in the horizontal position. Atropine and IV fluid are immediately available throughout the procedure for emergency management.

Recovery & Aftercare

If syncope is induced, the patient is returned to horizontal and recovers within 2-5 minutes as cerebral perfusion is rapidly restored. An observation period of 30-60 minutes after the test allows heart rate and blood pressure to stabilise before discharge. Oral fluids and a light snack are provided. Driving is advised against for 24 hours after a positive test. The results are reviewed with the referring cardiologist or neurologist to determine the syncope mechanism. Management is personalised based on findings: vasovagal syncope is managed with lifestyle education, increased fluid and salt intake, physical counterpressure manoeuvres, and selected pharmacotherapy; POTS responds to fluid loading and beta-blockers; cardioinhibitory syncope may prompt consideration of dual-chamber pacemaker implantation.

Symptom diary and Holter monitoring results are reviewed alongside tilt table findings at a dedicated syncope clinic appointment. Vasovagal syncope management involves counselling on recognition of prodromal symptoms, avoiding triggering situations (prolonged standing, heat, dehydration, painful stimuli), physical counterpressure manoeuvres, increased fluid and salt intake, and in medically refractory cases, fludrocortisone, midodrine, or implantable cardiac pacing for cardioinhibitory vasovagal syncope with documented asystole greater than 3 seconds. Driving cessation advice is provided according to the DVLA guidelines for syncope.

POTS (postural orthostatic tachycardia syndrome) causes a sustained heart rate rise of over 30 bpm on standing without blood pressure drop, producing chronic lightheadedness, fatigue, and palpitations without loss of consciousness. Beta-blockers reduce heart rate response and may blunt vasovagal reflex, causing false-negative results. Fludrocortisone increases blood volume and may suppress orthostatic hypotension. A negative tilt table test does not exclude vasovagal syncope — the false-negative rate is 15-30%. If syncope remains unexplained, further investigation with an implantable loop recorder (ILR) inserted subcutaneously under local anaesthesia provides continuous ECG monitoring for up to 3 years and is the most sensitive method for correlating heart rhythm with spontaneous syncopal episodes.
