What Is Varicose Vein Treatment?
Varicose vein treatment encompasses a range of minimally invasive endovenous and surgical procedures to eliminate or permanently close enlarged, dilated, and tortuous superficial veins of the lower limbs — most commonly the great saphenous vein (GSV) and small saphenous vein (SSV) and their tributaries — which develop when venous valves within the superficial venous system fail, causing retrograde blood pooling and progressive venous hypertension. Varicose veins affect approximately 30-40% of the adult Western population and are classified by the CEAP system (Clinical-Aetiology-Anatomy-Pathophysiology): C2 (varicose veins alone), C3 (oedema), C4 (skin changes including lipodermatosclerosis and pigmentation), C5 (healed venous ulcer), and C6 (active venous ulcer). Modern endovenous thermal ablation techniques — endovenous laser ablation (EVLA) and radiofrequency ablation (RFA/ClosureFAST procedure) — have largely replaced conventional open surgery (Trendelenburg procedure with stripping) because they achieve equivalent or superior clinical outcomes with less post-operative pain, faster return to normal activity, lower complication rates, and the ability to be performed under local tumescent anaesthesia as a day-case procedure without general anaesthesia. Non-thermal alternatives include ultrasound-guided foam sclerotherapy (UGFS) and cyanoacrylate glue closure (VenaSeal). The appropriate modality for each patient is selected based on vein anatomy, diameter, tortuosity, and presence of saphenofemoral or saphenopopliteal junction incompetence on duplex Doppler ultrasound mapping.
Who Needs This Procedure?
Treatment is indicated for symptomatic varicose veins causing significant aching, heaviness, throbbing, itching, restless legs, or night cramps that impair daily activities or quality of life, and have not adequately responded to conservative measures (graduated compression stockings worn consistently, regular walking exercise, limb elevation when seated). Patients with CEAP class C4-C6 disease — skin changes, lipodermatosclerosis, atrophie blanche, or active or healed venous ulcers — have a compelling medical indication for treatment to heal ulceration and prevent recurrence; venous leg ulcers are associated with substantially impaired quality of life and carry significant healthcare costs. NICE guidelines recommend that patients with symptomatic varicose veins and reflux confirmed on duplex Doppler ultrasound should be offered endovenous ablation as the primary treatment. Patients with predominantly superficial tortuous tributaries without significant truncal reflux (GSV diameter under 5 mm, no SFJ incompetence) are best treated with ultrasound-guided foam sclerotherapy or direct-vision liquid sclerotherapy. Asymptomatic varicose veins without skin changes and with normal quality of life do not require treatment; conservative management with compression hosiery and lifestyle modification is appropriate.
How the Procedure Is Performed
Endovenous Laser Ablation (EVLA): Under ultrasound guidance, the GSV or SSV is punctured with a needle below the knee and a laser fibre advanced to the sapheno-femoral or sapheno-popliteal junction. Tumescent local anaesthesia (a large volume of dilute lidocaine and adrenaline solution) is infiltrated around the entire length of the vein under ultrasound guidance to compress it, protect surrounding tissues from thermal injury, and provide anaesthesia. The laser fibre is then withdrawn at a steady rate while delivering thermal energy that collapses the vein wall permanently. The procedure takes 45–60 minutes per leg and is performed entirely through a needle puncture with no surgical incision. Radiofrequency Ablation (RFA) uses the same principle but delivers radiofrequency current through a segmental catheter. Following truncal ablation, visible surface tributary varicosities are treated with phlebectomy (avulsion of varicosities through tiny 2mm stab incisions) or foam sclerotherapy injections. Ultrasound-Guided Foam Sclerotherapy (UGFS) alone is used when truncal thermal ablation is not suitable, injecting a detergent foam (polidocanol or sodium tetradecyl sulphate) under ultrasound vision to chemically ablate the vein. Cyanoacrylate glue closure (VenaSeal) is performed without tumescent anaesthesia by injecting a small volume of medical-grade cyanoacrylate adhesive into the lumen of the GSV through a catheter, causing immediate wall adhesion and occlusion without thermal energy, and requires no compression stocking post-procedure in some protocols.
Benefits & Outcomes
Endovenous thermal ablation (EVLA and RFA) achieves primary GSV occlusion rates of 90-95% at 5 years and 80-85% at 10 years in published registry data, with faster recovery and significantly less post-operative bruising and pain compared to conventional surgical stripping. NICE guidance confirms that EVLA is the preferred first-line treatment for truncal GSV reflux, with superior patient experience and equivalent or better long-term efficacy. Successful treatment of incompetent truncal veins leads to regression of visible varicosities in 80-90% of cases — the majority of visible varicose tributaries resolve without requiring separate treatment once the feeding incompetent trunk is ablated. Venous ulcer healing rates improve dramatically after treatment of underlying venous incompetence: the EVRA trial (NEJM 2018) demonstrated that early endovenous ablation of superficial venous incompetence in patients with active leg ulcers accelerated ulcer healing significantly compared to standard compression bandaging alone. Patient-reported outcomes including pain, cosmetic appearance, swelling, functional capacity, and quality-of-life scores improve substantially and are durable at 5-year follow-up in published prospective cohort data.
Risks & Complications
Superficial thrombophlebitis (inflammation along the thermally ablated vein segment) is the most common complication of EVLA/RFA, occurring in 10–20% of cases and causing painful cord-like hardening along the treated area for 2–6 weeks before resolving. Skin burns from thermal energy are rare with adequate tumescent anaesthesia. Post-procedure bruising, tenderness, and tight cord sensation along the treated vein are expected and settle within 2–4 weeks. Deep vein thrombosis (DVT) with endothermal heat-induced thrombosis (EHIT) extending from the treated superficial vein into the deep system occurs in less than 1% of cases. Saphenous nerve numbness below the knee is possible (5–10%) following GSV ablation. Foam sclerotherapy risks include transient visual disturbances, headache (particularly in patients with patent foramen ovale), matting (fine red surface vessels), skin staining, and ulceration from perivenous injection. Recurrent varicosities develop in 15–25% at 5 years.
Recovery & Aftercare
After EVLA or RFA, patients are fitted with a class II compression stocking (18–24 mmHg) worn continuously for 1 week then during the day for a further week. Patients walk for at least 30 minutes daily from the day of treatment to maintain deep venous flow and reduce DVT risk. Normal office work resumes the next day; physical jobs may resume after 5–7 days. Swimming and strenuous exercise are avoided for 2 weeks. A duplex ultrasound at 6 weeks confirms vein closure and excludes DVT. After conventional surgical stripping, 2–3 weeks off work is typical with a more pronounced recovery period. After foam sclerotherapy, compression bandaging for 48 hours followed by stockings for 2–3 weeks is recommended, with a follow-up injection session offered at 6–8 weeks if residual varicosities remain.
