What Is Wisdom Tooth Removal?
Wisdom tooth removal (third molar extraction) is one of the most commonly performed oral surgical procedures worldwide, with approximately 10 million wisdom teeth removed annually in the US alone. Wisdom teeth — the last permanent teeth to erupt, usually between ages 17–25 — frequently become impacted (unable to fully emerge into normal occlusal position) due to insufficient space in the alveolar arch because the jaws have already reached adult proportions before these teeth attempt eruption. Impacted wisdom teeth can be partially erupted (partially through the gum, with a flap of gum tissue partially overlying the crown — the operculum) or fully embedded within the jawbone, visible only on X-ray or CBCT scan. Third molars may be vertically, mesio-angularly, horizontally, or disto-angularly impacted, describing the angulation of the tooth relative to the adjacent second molar — mesio-angular impaction is most common (43%) and generally easiest to remove. Simple erupted wisdom teeth are removed straightforwardly with forceps, while impacted teeth require surgical elevation of a mucosal flap, bone removal, and often tooth sectioning for atraumatic extraction. Modern surgical techniques including coronectomy for high inferior alveolar nerve risk cases provide safe alternatives that preserve neurovascular integrity while eliminating the pathological consequences of the symptomatic tooth.
Who Needs This Procedure?
Wisdom tooth removal is recommended by NICE (UK) and AAOMS (USA) guidelines when the third molar is causing, or has caused, a disease state or clinical problem. Evidence-based indications include pericoronitis (acute or recurrent infection of the gum flap overlying a partially erupted wisdom tooth, causing swelling, trismus, and pain); dental caries in the wisdom tooth itself or the adjacent second molar (which is difficult to clean effectively and restore); external root resorption of the adjacent second molar root from pressure by the wisdom tooth crown; cyst or tumour formation around an unerupted wisdom tooth (dentigerous cyst, odontogenic keratocyst); persistent unexplained pain attributable to an erupting or impacted wisdom tooth; and before cancer radiotherapy to the jaws or immunosuppressive treatment where post-operative healing would be severely compromised. Prophylactic removal of completely asymptomatic, disease-free impacted wisdom teeth is not recommended by current guidelines from NICE and AAOMS — the risk-benefit calculation does not support routine extraction in the absence of pathology, though patient preferences and clinical judgement may apply in individual cases.
How the Procedure Is Performed
Wisdom tooth removal is performed in the dental chair under local anaesthesia for most patients, or under intravenous sedation (midazolam with or without an opioid analgesic) or general anaesthesia for anxious patients, complex impactions, or multiple simultaneous extractions. After thorough local anaesthetic infiltration into the buccal and lingual gum, periosteum, and inferior alveolar nerve block (for lower teeth), the field is tested for profound anaesthesia. For surgical removal of an impacted tooth, a crescent-shaped mucoperiosteal flap is elevated using a periosteal elevator, reflecting the gum away from the bone to expose the crown and overlying alveolar bone. A dental surgical bur removes the specific bone obstructing the crown and roots, with copious sterile saline irrigation throughout to prevent bone necrosis from heat. The wisdom tooth may be sectioned into two or three pieces with a bur or piezoelectric saw to allow removal in smaller segments without excessive bone removal or force on the adjacent second molar. Each section is elevated with dental elevators using controlled lever movements and delivered from the socket with forceps. The socket is irrigated, sharp bone edges smoothed, and the mucosal flap repositioned and sutured with 3–0 or 4–0 resorbable sutures. All four wisdom teeth can be removed in a single session. Operating time varies from 10 minutes for a simple erupted tooth to 45–60 minutes for a deeply impacted horizontal lower wisdom tooth requiring extensive bone removal and tooth sectioning.
Results & Success Rates
Surgical removal of a problematic wisdom tooth permanently eliminates the source of infection, dental decay, root resorption, or cystic disease in over 95% of cases, resolving the acute and recurrent symptoms that prompted referral. Pericoronitis does not recur once the causative wisdom tooth is removed. Second molar decay and root resorption are halted, preserving the function and longevity of the adjacent tooth. Postoperative recovery is predictable and manageable for the vast majority of patients following clear post-operative instructions. The long-term oral health benefit from eliminating an inaccessible and pathologically significant tooth outweighs the short-term discomfort of the recovery period in patients meeting the evidence-based indications for surgery. Patients who require pre-radiotherapy or pre-immunosuppression clearance of wisdom teeth benefit from reduced subsequent risk of osteoradionecrosis (a serious jaw complication from radiation therapy) or post-extraction healing failure during chemotherapy.
Risks & Complications
Dry socket (alveolar osteitis) — loss of the protective blood clot from the extraction socket, exposing bare bone — occurs in 5–20% of lower wisdom tooth removals and is the most common post-operative complication, causing severe throbbing pain beginning at day 2–4 that is not relieved by standard analgesics and may radiate to the ear. Risk is higher in smokers (3-fold increase), oral contraceptive users, and patients who rinse forcefully or use straws during the first 72 hours. Treatment is placement of a medicated dressing (alvogyl, bismuth iodoform paste) in the socket, providing immediate relief within hours. Inferior alveolar nerve (IAN) injury — the nerve supplying sensation to the lower lip, chin, and teeth — is the most serious risk of lower wisdom tooth surgery: temporary numbness occurs in 5–8% and persistent numbness beyond 6 months (permanent injury) in 0.5–1%, most commonly when the tooth root apex is intimately related to the inferior alveolar canal (assessed on pre-operative panoramic X-ray or CBCT). Lingual nerve injury causing altered tongue sensation occurs in 0.1–1%. Other risks include secondary haemorrhage at 5–10 days from clot displacement, wound infection requiring antibiotics, jaw fracture (extremely rare, associated with deep impaction in older patients), and damage to the adjacent second molar during extraction.
Recovery & Aftercare
A gauze pad is bitten firmly against the extraction socket for 30–45 minutes immediately post-operatively to promote haemostasis by clot formation. Swelling is anticipated and peaks at 48–72 hours, then gradually resolves over 5–7 days. Ice packs applied in a cloth (20 minutes on, 20 minutes off) for the first 24 hours significantly reduce post-operative swelling. Warm compresses from day 3 onwards promote resolution of residual swelling. Sutures — if resorbable — dissolve in 7–14 days without removal. Gentle saline mouth rinses (half teaspoon of salt in a glass of warm water) should begin 24 hours after surgery and continue after every meal for 7–10 days to keep the socket clean. Vigorous rinsing, spitting, straws, smoking, and alcohol are strictly avoided for the first 72 hours to prevent dislodging the healing blood clot. A soft, cool diet (yoghurt, scrambled eggs, soup, soft pasta) is maintained for 5–7 days. Pain is most effectively managed by alternating paracetamol (1g every 6 hours) with ibuprofen (400mg every 6 hours) rather than taking either alone. Most patients require analgesics for 3–5 days. Sedentary work can resume after 2–3 days; physical work after 5–7 days.
