About Chest Pain
Chest pain is among the most serious and potentially life-threatening symptoms a person can experience, and it demands immediate clinical evaluation. The chest contains the heart, lungs, great vessels, oesophagus, and chest wall structures — any of which can be the source of chest pain. Approximately 15% of all emergency department presentations are for chest pain, and among these, approximately 15–25% involve an acute coronary syndrome (heart attack or unstable angina). The central clinical challenge is accurately and rapidly distinguishing between life-threatening cardiac or vascular causes and the much more numerous benign non-cardiac causes, which include musculoskeletal injury, gastro-oesophageal reflux, anxiety, and pleuritis. This distinction is made harder because atypical presentations of myocardial infarction are common — particularly in women, older adults, and diabetic patients, who may present with fatigue, nausea, jaw pain, or epigastric discomfort rather than classic crushing chest pain. The golden rule in medicine is: treat any new, unexplained chest pain as potentially cardiac until proven otherwise. Never self-diagnose chest pain as muscular or indigestion without a clinical assessment.
Common Causes of Chest Pain
Cardiac causes include stable angina — exertional chest pressure relieved within 5 minutes by rest or glyceryl trinitrate — caused by fixed atherosclerotic coronary artery narrowing. Unstable angina occurs at rest or with minimal exertion and is a medical emergency. Myocardial infarction (heart attack) results from complete occlusion of a coronary artery, causing ischaemic necrosis of heart muscle. Pericarditis (inflammation of the pericardial sac) causes sharp pleuritic chest pain that worsens lying flat and improves leaning forward. Aortic dissection causes abrupt tearing or ripping pain between the shoulder blades — a catastrophic vascular emergency. Pulmonary embolism (blood clot in the lungs) causes pleuritic chest pain, sudden breathlessness, and haemoptysis. Pneumothorax (collapsed lung) presents with sudden sharp unilateral chest pain and shortness of breath. Pneumonia causes pleuritic pain with fever and productive cough. Gastro-oesophageal reflux disease (GERD) produces retrosternal burning after meals, when lying flat, or at night. Oesophageal spasm causes severe squeezing substernal pain often indistinguishable from angina. Costochondritis (inflammation of the cartilage connecting ribs to the sternum) causes reproducible chest wall tenderness. Panic disorder frequently causes chest tightness, palpitations, and breathlessness.
Warning Signs & When It's Serious
Call emergency services (999/112/911) immediately without delay for any of the following: crushing, squeezing, pressure, or heaviness in the chest lasting more than a few minutes, which may radiate to the left arm, right arm, jaw, neck, or upper back — the classic warning signs of myocardial infarction; chest pain accompanied by profuse sweating (diaphoresis), nausea or vomiting, breathlessness, or a feeling of impending doom; sudden severe tearing or ripping chest pain between the shoulder blades radiating to the back — a hallmark of aortic dissection, which has up to 1% per hour mortality if untreated; chest pain with coughing up blood (haemoptysis), suggesting pulmonary embolism; chest pain with severe new breathlessness and oxygen desaturation; and acute severe pleuritic chest pain in someone who has recently had surgery, a long-haul flight, or is known to have a clotting disorder (pulmonary embolism risk). In women, older adults, and diabetic patients, atypical presentations including fatigue, jaw pain, epigastric discomfort, and nausea without classic chest pain may be the only sign of myocardial infarction — maintain a high index of suspicion.
How Chest Pain Is Diagnosed
A 12-lead ECG is performed within 10 minutes of presentation to detect ST-segment elevation myocardial infarction (STEMI), which requires immediate coronary intervention, and other arrhythmias or ischaemic changes. Serial troponin blood tests (high-sensitivity troponin I or T) at 0, 1, and 3 hours detect cardiac muscle damage with high sensitivity and specificity — elevation confirms myocardial injury. Chest X-ray assesses lung fields (pneumonia, pneumothorax, pulmonary oedema), cardiac size, and mediastinal width (widening suggests aortic dissection). D-dimer blood test is used to screen for pulmonary embolism when clinical probability is low to moderate — an elevated D-dimer prompts CT pulmonary angiography (CTPA). CTPA is the definitive imaging for pulmonary embolism. Echocardiogram assesses left ventricular function, wall motion abnormalities, pericardial effusion, and valve disease. CT coronary angiography evaluates coronary anatomy non-invasively in stable patients. Coronary angiography (invasive) is gold standard for defining coronary stenoses and guides revascularisation decisions.
Treatment & Management
Cardiac chest pain from STEMI requires the fastest possible coronary reperfusion — primary percutaneous coronary intervention (PPCI) within 90 minutes of first medical contact is the gold standard, reducing mortality by up to 50%. Dual antiplatelet therapy (aspirin plus a P2Y12 inhibitor such as ticagrelor or clopidogrel), anticoagulation (heparin), and oxygen if saturations are below 94% are initiated immediately. Thrombolysis is an alternative when PPCI is not available within 120 minutes. Non-STEMI and unstable angina are managed with risk-stratified medical treatment and early invasive angiography. Stable angina is treated with sublingual glyceryl trinitrate (GTN) for acute attacks, and long-term beta-blockers, nitrates, and calcium channel blockers for prevention. Non-cardiac causes are treated by aetiology: high-dose proton pump inhibitors for GERD; NSAIDs and colchicine for pericarditis; anticoagulation (low-molecular-weight heparin followed by a DOAC) for pulmonary embolism; needle decompression for tension pneumothorax; antibiotics for pneumonia; and cognitive behavioural therapy with anxiolytics for panic disorder.
Prevention & Lifestyle Tips
Cardiovascular risk reduction is the most important prevention strategy for the most serious causes of chest pain. This means not smoking — which halves cardiovascular risk within 1 year of cessation; eating a heart-healthy Mediterranean-style diet rich in fish, olive oil, nuts, legumes, fruits, and vegetables; exercising for at least 150 minutes of moderate-intensity aerobic activity per week; and maintaining a healthy body weight, reducing alcohol consumption, and managing emotional stress. Control modifiable risk factors actively with your doctor: treat hypertension to a target below 130/80 mmHg, optimise LDL cholesterol with statins, and maintain good blood glucose control in diabetes. Take prescribed cardiac medications consistently — statins, aspirin, and antihypertensives all save lives through event prevention. For GERD-related chest pain, avoid large meals, elevate the head of the bed, reduce coffee, alcohol, and fatty foods, and maintain a healthy weight. Pulmonary embolism prevention includes adequate hydration on long flights, compression stockings, early mobilisation after surgery, and appropriate use of prophylactic anticoagulation.
When to See a Doctor
Call emergency services immediately (do not drive yourself) for any new, unexplained chest pain lasting more than a few minutes, even if you feel it may be benign. The single most critical factor in surviving a heart attack is time — every minute of delay in reperfusion increases long-term cardiac damage. Call 999/112/911 for chest pain radiating to the arm, jaw, or neck; chest pain with sweating, nausea, or breathlessness; sudden severe chest pain (possible aortic dissection); or chest pain with coughing up blood (possible pulmonary embolism). Attend the emergency department on the same day for any unexplained chest pain that has now resolved, as you may have experienced unstable angina or a NSTEMI that still requires urgent investigation. Book a priority same-week appointment with your GP for recurrent mild chest pain that you have not previously discussed, known GERD with changed symptoms, or musculoskeletal chest wall pain after injury. Never dismiss mild chest pain without at minimum a clinical consultation and resting ECG. If you have known coronary artery disease and your symptoms have changed in character or frequency, seek urgent review.
