About Chronic Fatigue
Chronic fatigue is defined as persistent, overwhelming tiredness lasting 6 months or more that is not relieved by sleep or rest and that significantly impairs a person's ability to carry out daily physical, cognitive, and social activities. It must be distinguished from normal tiredness after exertion or a busy period, which resolves with adequate rest. Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is the most widely recognised and debilitating form — estimated to affect 17–24 million people worldwide — but chronic fatigue is also a cardinal feature of many common, highly treatable medical conditions including anaemia, hypothyroidism, diabetes, depression, and obstructive sleep apnoea. A critical principle in the management of chronic fatigue is that treatable medical and psychiatric causes must be systematically excluded before a diagnosis of ME/CFS is made. The condition is more common in women and typically peaks between the ages of 20 and 45, though it can affect people of any age, including children and adolescents. Chronic fatigue is associated with significant economic and social burden, including job loss, relationship difficulties, and impaired educational attainment. Early, accurate diagnosis and cause-specific treatment can dramatically improve outcomes.
Common Causes of Chronic Fatigue
Anaemia — particularly iron deficiency anaemia — causes profound fatigue by reducing the oxygen-carrying capacity of the blood; ferritin deficiency without overt anaemia also causes significant tiredness. Hypothyroidism (underactive thyroid) is one of the most common and easily reversible causes — even mildly low thyroid function produces fatigue, weight gain, cold intolerance, and low mood. Type 2 diabetes causes fatigue through chronic hyperglycaemia, cellular energy inefficiency, and disrupted sleep. Clinical depression is characterised by fatigue in over 80% of cases, mediated through neurobiological changes affecting serotonin, dopamine, and energy metabolism. Anxiety disorders cause chronic physical fatigue through sustained sympathetic nervous system activation. Post-viral syndromes — including post-COVID syndrome (long COVID), post-Epstein-Barr virus fatigue, and post-influenza fatigue — produce fatigue through immune dysregulation and autonomic nervous system disruption. Warning Signs & When It's Serious
Seek urgent medical evaluation if persistent fatigue is accompanied by any of the following red flag features that suggest serious underlying disease. Unexplained weight loss of more than 5% of body weight over 3–6 months combined with fatigue warrants investigation for malignancy, inflammatory bowel disease, or diabetes. Drenching night sweats soaking through pyjamas and bedclothes with fatigue may indicate lymphoma, tuberculosis, or other serious infection. Tender swollen lymph nodes — particularly cervical, axillary, or inguinal — alongside fatigue need assessment for haematological malignancy or infection. Persistent fever above 38°C for more than two weeks without an obvious infectious cause requires investigation. New progressive neurological symptoms including cognitive changes, confusion, focal weakness, or visual disturbance alongside fatigue may indicate a neurological condition. Shortness of breath at rest or on minimal exertion with fatigue raises concern for cardiac or pulmonary disease. Fatigue with significant unrefreshing sleep and witnessed apnoeic episodes suggests severe obstructive sleep apnoea requiring prompt sleep study referral. Sudden onset of severe fatigue, particularly following a viral illness, also warrants prompt medical assessment.
How Chronic Fatigue Is Diagnosed
A comprehensive blood panel is the first step in investigating chronic fatigue and typically includes: full blood count (to detect anaemia, leukaemia, or infection); thyroid stimulating hormone (TSH) to screen for thyroid dysfunction; fasting blood glucose and HbA1c for diabetes; ferritin and serum iron for iron deficiency; vitamin B12 and folate; vitamin D (25-hydroxyvitamin D); C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) for systemic inflammation; liver function tests and kidney function (creatinine and eGFR); and urinalysis. Additional targeted tests are added based on clinical context — these may include HIV serology, hepatitis B and C antibodies, coeliac disease antibodies (tTG IgA), antinuclear antibodies (ANA) and rheumatoid factor for autoimmune disease, cortisol for adrenal insufficiency, and fasting calcium for hyperparathyroidism. A polysomnography (overnight sleep study) or home sleep apnoea test is recommended when obstructive sleep apnoea is clinically suspected based on snoring, witnessed apnoeas, or excessive daytime sleepiness. Validated depression and anxiety screening tools — PHQ-9 and GAD-7 — should be administered as part of every chronic fatigue workup. ME/CFS is diagnosed clinically after excluding all other causes, using the 2015 Institute of Medicine diagnostic criteria requiring post-exertional malaise, unrefreshing sleep, and either cognitive impairment or orthostatic intolerance.
Treatment & Management
Effective treatment depends entirely on identifying and addressing the underlying cause. Iron deficiency — with or without anaemia — is treated with oral iron supplements (ferrous sulphate or ferrous fumarate) for at least 3 months, with dietary advice to increase iron-rich foods and co-administer with vitamin C to enhance absorption. Hypothyroidism is treated with levothyroxine (thyroid hormone replacement), which typically resolves fatigue within 6–12 weeks. Blood glucose control in diabetes reduces fatigue through improved cellular energy metabolism — dietary change, weight loss, and medications (metformin, GLP-1 agonists) all contribute. CPAP (continuous positive airway pressure) therapy for obstructive sleep apnoea produces dramatic improvement in daytime energy, often within days of starting treatment. Treating depression with antidepressants (SSRIs as first-line) and psychotherapy — particularly cognitive behavioural therapy (CBT) — reliably reduces fatigue in 4–8 weeks. For ME/CFS specifically, carefully paced activity management (avoiding post-exertional malaise through energy envelope strategies) is the cornerstone of management; significant overexertion worsens the condition. Sleep hygiene optimisation, symptom management with low-dose amitriptyline for sleep, and careful multidisciplinary coordination are evidence-based ME/CFS approaches. Vitamin D and B12 supplementation correct deficiency-related fatigue. Nutritional rehabilitation is essential in eating disorder-related fatigue.
Prevention & Lifestyle Tips
While not all causes of chronic fatigue are preventable, several lifestyle factors significantly reduce the risk of its most common treatable causes. Prioritise 7–9 hours of consistent quality sleep nightly with a regular bedtime and wake time, avoiding screen exposure in the hour before sleep, keeping the bedroom cool and dark, and addressing insomnia or sleep apnoea proactively. Maintain a nutritionally complete diet with adequate iron (red meat, beans, leafy greens), vitamin B12 (meat, fish, eggs, dairy or B12-fortified foods), and vitamin D (sunlight exposure, oily fish, fortified foods, or supplementation of 400–1000 IU daily particularly in winter). Exercise regularly at a moderate intensity — 150 minutes per week of aerobic activity prevents deconditioning and reduces fatigue in most medical conditions. Limit caffeine to before midday and avoid alcohol as a sleep aid, as both disrupt sleep architecture. Address mental health concerns early — seek professional support for persistent stress, anxiety, or low mood before they become entrenched chronic conditions. Manage chronic diseases (diabetes, thyroid disease, heart disease) proactively with regular reviews and medication adherence.
When to See a Doctor
Seek urgent medical assessment if fatigue is accompanied by unexplained weight loss, drenching night sweats, swollen lymph nodes, persistent fever, significant shortness of breath, or rapidly progressive neurological symptoms — these red flags require same-day or emergency evaluation. Schedule a priority appointment within a week for fatigue that has been present for more than 4 weeks, is worsening, is significantly limiting your ability to work or manage daily activities, or is accompanied by other symptoms such as low mood, increased thirst, cold intolerance, hair loss, or snoring at night. Book a routine appointment if you have noticed gradually increasing tiredness over months that you cannot explain by lifestyle factors alone. Do not dismiss persistent fatigue as simply a consequence of a busy modern lifestyle or a normal part of ageing — it very frequently has a specific, identifiable, and treatable medical or psychological cause. Early investigation and diagnosis leads to much faster and more complete recovery. Fatigue in children, adolescents, and pregnant women always warrants prompt evaluation.
