Dizziness is a broad symptom umbrella encompassing three distinct sensory experiences: vertigo — a false sensation that the patient or their surroundings are spinning, rotating, or tilting; lightheadedness — a near-faint sensation of spatial disconnection often described as floating or wooziness; and disequilibrium — a sense of unsteadiness or imbalance while walking without a specific illusory sensation. It is among the most common complaints in medical practice, accounting for up to 4% of all emergency department visits and affecting approximately 30% of adults over 65 significantly enough to limit activity. The importance of dizziness lies not only in its impact on quality of life and daily function — dizziness is a leading cause of falls and fall-related injury in older adults, contributing to significant morbidity and mortality — but also in the fact that a minority of presentations represent symptoms of serious neurological, cardiovascular, or metabolic emergencies requiring immediate intervention. Distinguishing between peripheral (inner ear) and central (brainstem or cerebellum) causes of vertigo is the critical clinical decision, as central causes may indicate transient ischaemic attack (TIA) or stroke. The timing, duration, triggers, and associated symptoms of dizziness are the key features guiding accurate diagnosis and appropriate urgency of response.
Common Causes of Dizziness
Peripheral vestibular (inner ear) conditions cause the majority of vertigo cases. Benign paroxysmal positional vertigo (BPPV) is the most common cause: calcium carbonate crystals (otoconia) become dislodged from the utricle and migrate into a semicircular canal, producing brief but intense rotational vertigo (lasting less than 1 minute) triggered predictably by specific head position changes — lying down, rolling over in bed, looking up, or bending forward. Vestibular neuritis (or labyrinthitis if hearing is affected) is caused by viral inflammation of the vestibular nerve, producing a prolonged, severe episode of vertigo lasting days to weeks with nausea and vomiting, often with a recent viral illness. Meniere's disease causes recurrent discrete episodes of vertigo lasting 20 minutes to several hours, combined with fluctuating hearing loss, tinnitus (ringing), and aural fullness, believed to be caused by endolymphatic hydrops. Cardiovascular causes are important: orthostatic hypotension — a drop of more than 20 mmHg systolic on standing — causes lightheadedness or near-faint within seconds of rising, common in elderly patients, in those on antihypertensives, and in the dehydrated. Cardiac arrhythmias including paroxysmal atrial fibrillation cause episodic lightheadedness and palpitations. Migraine-associated vertigo (vestibular migraine) is an increasingly recognised cause of recurrent vertigo with or without headache, often in those with a personal or family history of migraine. Central causes — TIA or stroke affecting the posterior circulation (brainstem, cerebellum) — produce vertigo with neurological accompaniments including diplopia, dysarthria, dysphagia, facial numbness, or limb ataxia. Metabolic causes include hypoglycaemia, anaemia, dehydration, and medication side effects.
Warning Signs & When It's Serious
Call emergency services immediately for dizziness accompanied by any of the following neurological warning signs, collectively representing the FAST acronym and its extensions: sudden unilateral facial drooping or asymmetry; arm or leg weakness, heaviness, or numbness; slurred, muddled, or absent speech; sudden severe headache described as the worst of your life (possible subarachnoid haemorrhage); or double vision (diplopia). These signs indicate a posterior circulation stroke or TIA — a time-critical neurological emergency where every minute of delayed treatment results in additional irreversible brain damage. Also seek emergency care for dizziness with acute new hearing loss in one ear occurring suddenly, suggesting inner ear vascular event or autoimmune labyrinthitis; dizziness with chest pain, severe palpitations, or syncope (loss of consciousness), suggesting a cardiac arrhythmia; dizziness following head trauma, which may indicate intracranial haemorrhage; and prolonged severe vertigo with inability to walk in a person with cardiovascular risk factors (risk of posterior circulation stroke is underappreciated). Important: central causes of vertigo from stroke sometimes produce only dizziness and gait ataxia without the classic FAST signs — maintain a high index of suspicion in those with vascular risk factors.
How Dizziness Is Diagnosed
Accurate history is the most powerful diagnostic tool: the clinician determines whether the sensation is true rotational vertigo (inner ear more likely) versus lightheadedness (cardiovascular, metabolic more likely) versus disequilibrium (central neurological or bilateral vestibular dysfunction). Duration and triggers are essential — BPPV lasts seconds to under 1 minute and is position-triggered; vestibular neuritis causes constant vertigo for days; TIA causes brief vertigo without positional trigger. The Dix-Hallpike manoeuvre is performed to diagnose BPPV: the patient is quickly moved from sitting to lying with the head turned 45 degrees to one side and then 30 degrees lower than horizontal — a positive test produces characteristic geotropic rotary nystagmus (eyes beating toward the affected lower ear) with a latency of a few seconds. Orthostatic blood pressure measurement (lying and then standing at 1 and 3 minutes) detects postural hypotension. ECG assesses cardiac rhythm and identifies arrhythmias. Full blood count detects anaemia. Blood glucose identifies hypoglycaemia. The HINTS examination (Head Impulse test, Nystagmus pattern, Test of Skew) performed at the bedside distinguishes peripheral from central vertigo with higher sensitivity than early MRI for posterior circulation stroke. MRI brain with diffusion-weighted imaging (DWI) is the gold standard for detecting posterior fossa stroke and is indicated when HINTS examination suggests central pathology. Pure-tone audiogram assesses hearing in Meniere's disease. Caloric testing and videonystagmography provide objective vestibular function assessment in complex cases.
Treatment & Management
BPPV is highly effectively treated with the Epley canalith repositioning manoeuvre — a sequence of 4 head and body positions maintained for 30 seconds each, designed to guide the displaced otoconia out of the affected semicircular canal and back into the utricle where they cannot cause symptoms. A single Epley manoeuvre achieves resolution of symptoms in 80–90% of patients, and repeated sessions successfully treat almost all remaining cases. The manoeuvre can be performed in clinic or taught for self-administration at home. Vestibular neuritis is treated with a short course of vestibular suppressants (prochlorperazine or cinnarizine) for severe acute nausea and vertigo lasting only 2–3 days, as prolonged use impairs the brain's natural vestibular compensation process. Vestibular rehabilitation exercises — systematic head movement programmes that recalibrate the vestibulo-ocular reflex — are the primary treatment and accelerate recovery in vestibular neuritis and chronic vestibular disorders. Meniere's disease is managed with dietary sodium restriction (below 2g sodium daily), a thiazide diuretic to reduce endolymphatic pressure, and betahistine; severe cases may require intratympanic gentamicin injections or endolymphatic sac surgery. Orthostatic hypotension is managed with postural advice (rising slowly in stages from lying to sitting to standing), adequate hydration, compression stockings, reducing or adjusting antihypertensive medications, and in refractory cases, fludrocortisone or midodrine. Vestibular migraine responds to standard migraine preventive treatments including propranolol, amitriptyline, and CGRP antagonists. Central causes (stroke, TIA) require urgent neurovascular management.
Prevention & Lifestyle Tips
Prevent orthostatic dizziness by rising from lying or sitting positions in stages — pause on the edge of the bed or chair for 30–60 seconds before fully standing to allow blood pressure to compensate. Maintain adequate hydration, particularly in hot weather or during exercise, as even mild dehydration causes orthostatic hypotension. If you have recurrent BPPV, learn the modified Epley self-treatment manoeuvre from a vestibular physiotherapist so you can perform it promptly when symptoms recur. Review your medications with your doctor if dizziness developed after starting a new drug — many antihypertensives, antidepressants, antihistamines, and sedatives cause or worsen dizziness, particularly in older adults. Avoid excessive alcohol, which causes both acute intoxication-related unsteadiness and chronic vestibular dysfunction with heavy use. Manage cardiovascular risk factors (hypertension, diabetes, dyslipidaemia, smoking) to reduce the risk of posterior circulation TIA and stroke — the most serious central cause of dizziness. Correct vitamin D and B12 deficiency, both associated with impaired proprioception and balance in older adults. Install grab rails and remove trip hazards in the home if recurrent dizziness or balance problems increase fall risk.
When to See a Doctor
Call emergency services immediately for dizziness with sudden facial drooping, arm or leg weakness or numbness, slurred or incomprehensible speech, sudden severe headache, or double vision — these are stroke warning signs requiring immediate brain imaging and thrombolysis or thrombectomy assessment within minutes to hours. Seek emergency care for dizziness with chest pain, palpitations, and near-faint or loss of consciousness, suggesting a cardiac arrhythmia. Seek urgent same-day assessment for a first episode of prolonged severe vertigo lasting hours in someone over 50 with cardiovascular risk factors (possible cerebellar stroke even without classic neurological signs), dizziness with acute unilateral hearing loss (possible inner ear vascular event), or dizziness with fever and ear pain (possible mastoiditis or inner ear infection). Schedule an urgent GP appointment within a few days for new persistent dizziness lasting more than 24 hours, recurrent episodes of vertigo with hearing changes or tinnitus (possible Meniere's disease), or dizziness significantly affecting walking, driving, or daily function. Dizziness in older adults causing falls or fear of falling always warrants medical review and falls risk assessment. Recurrent positional vertigo lasting seconds, triggered by lying down or rolling over, is almost certainly BPPV and responds very well to physiotherapy referral for Epley manoeuvre.
