Globally, musculoskeletal conditions — led by osteoarthritis — are a leading cause of disability and reduced quality of life, with over 500 million people affected by osteoarthritis alone. Joint pain may be articular (arising from within the joint structure itself, including cartilage, synovium, or bone) or periarticular (originating from surrounding structures such as tendons, bursae, or ligaments). This distinction is clinically important because causes and treatments differ fundamentally between these two categories. Pain may affect a single joint (monoarthritis) or multiple joints simultaneously (polyarthritis), and whether onset is acute or gradual — each pattern suggesting a different diagnostic pathway. Accurate classification of joint pain character, distribution, and associated features drives effective, targeted management and appropriate specialist referral.
Common Causes of Joint Pain
Articular causes include osteoarthritis (degenerative wear-and-tear joint disease, the most common overall cause affecting over 500 million people globally), rheumatoid arthritis (chronic autoimmune inflammatory disease causing symmetrical small joint involvement and prolonged morning stiffness), and gout (caused by monosodium urate crystal deposition — classically affecting the big toe with sudden, severe, red, hot pain). Septic arthritis (bacterial joint infection from Staphylococcus aureus, Streptococcus, or Neisseria gonorrhoeae) is a true emergency. Periarticular causes include tendinitis, bursitis, and ligament injuries from overuse or trauma. Systemic inflammatory causes include psoriatic arthritis, ankylosing spondylitis, systemic lupus erythematosus (SLE), and reactive arthritis following gastrointestinal or urogenital infections. Seek emergency care immediately for a single hot, swollen, intensely painful, red joint accompanied by fever — this clinical picture strongly suggests septic arthritis, a bacterial joint infection that can permanently destroy cartilage within 24-48 hours if not drained and treated with intravenous antibiotics. Any fever above 38.5°C with an acutely swollen joint warrants same-day assessment. Also seek urgent care for sudden severe joint pain and swelling following trauma — fracture, haemarthrosis (blood in the joint), or complete tendon or ligament rupture must be excluded promptly. Joint instability or visible deformity following an injury requires urgent imaging. Seek urgent review for joint pain accompanied by a widespread rash, high fever, and systemic illness — this pattern suggests infectious or systemic inflammatory disease including reactive arthritis or viral arthritis. Morning stiffness lasting over 60 minutes with multiple swollen joints suggests inflammatory arthritis (rheumatoid arthritis or related condition) requiring early specialist referral to prevent irreversible joint destruction. Joint pain with neurological symptoms such as weakness or numbness warrants urgent evaluation.
How Joint Pain Is Diagnosed
Physical examination assesses joint swelling, warmth, range of motion, tenderness pattern, and symmetry — determining whether the pain is articular or periarticular. Joint aspiration (arthrocentesis) analyses synovial fluid for white cell count (infection), crystals under polarised light (gout vs pseudogout), and culture to confirm septic arthritis.
Treatment & Management
Osteoarthritis is managed conservatively with exercise (particularly aquatic and strength training programmes), physiotherapy, and weight management — each kilogram of weight loss reduces knee load by approximately four kilograms. Analgesia follows a stepwise approach: paracetamol or topical NSAIDs first-line, then oral NSAIDs with gastric protection, then weak opioids for moderate-severe pain. Intra-articular corticosteroid injections provide temporary but significant relief for acute flares. Total joint replacement (arthroplasty) is highly effective for end-stage osteoarthritis of the knee or hip. Avoid repetitive high-impact joint loading and use appropriate protective equipment in contact sports. For gout prevention: reduce red meat, organ meats, alcohol (especially beer and spirits), and high-fructose corn syrup intake; maintain good hydration with at least 2 litres of water daily to dilute serum urate. For inflammatory arthritis: early initiation of DMARDs is the most effective prevention of joint destruction — do not delay specialist referral. Treat underlying infections promptly to prevent reactive arthritis. Calcium and vitamin D supplementation helps maintain bone density around joints in older adults.

This represents the most urgent joint pain presentation and must not be delayed. Seek urgent same-day or next-day care for significant joint pain, deformity, or instability following trauma — fracture or ligament rupture must be excluded by clinical assessment and imaging. Arrange an urgent routine appointment within one to two weeks for joint pain in multiple joints with morning stiffness lasting over 30 minutes, suggesting inflammatory arthritis — early rheumatology referral is critical to preventing joint damage. Also see a doctor promptly for joint pain with systemic features such as fever, rash, weight loss, or fatigue; joint pain unresponsive to paracetamol and rest after two weeks; or recurrent episodes of acute joint swelling and pain. Routine review is appropriate for mild joint aching from suspected osteoarthritis without red flag features. Children with any persistent joint pain warrant early paediatric rheumatology referral, as juvenile idiopathic arthritis requires specialist management.
