Swelling (oedema) occurs when excess fluid accumulates in the interstitial spaces of body tissues, causing visible and palpable enlargement of the affected area. It most commonly affects the ankles, feet, and lower legs — gravity causes fluid to pool in the most dependent parts of the body — and characteristically worsens through the day and improves overnight with leg elevation. Oedema is clinically classified by two key characteristics. Pitting oedema leaves a visible indentation when firm digital pressure is applied to the swollen area for five to ten seconds and released — it indicates elevated capillary hydrostatic pressure (as in heart failure or venous insufficiency) or reduced plasma oncotic pressure (as in hypoalbuminaemia from nephrotic syndrome or cirrhosis). Non-pitting oedema leaves no indentation on pressing and indicates lymphatic obstruction (lymphoedema), thyroid disease (myxoedema — caused by mucopolysaccharide deposition, not fluid), or lipid accumulation in lipoedema. The distribution of swelling — bilateral (systemic cause) versus unilateral (local cause), and whether it involves the face and hands (suggesting renal or allergic cause) — is equally diagnostically important. New, unexplained, or progressively worsening oedema should always be medically investigated rather than attributed to lifestyle factors.
Common Causes of Swelling
Benign and physiological causes include prolonged standing or sitting (orthostatic or gravitational oedema), pregnancy (physiological fluid retention and uterine compression of the iliac veins), premenstrual fluid retention (cyclical bilateral ankle and breast swelling), prolonged heat exposure causing peripheral vasodilation, and minor limb injury or sprain. Cardiovascular causes include congestive heart failure (bilateral pitting ankle and leg oedema worsening through the day, associated with breathlessness and orthopnoea — the most important systemic cause to identify), chronic venous insufficiency from varicose veins or post-thrombotic syndrome (aching, heaviness, and skin discolouration in the lower legs), and deep vein thrombosis (DVT — acute, unilateral, painful swelling with redness and warmth). Renal causes include chronic kidney disease (reduced sodium and water excretion) and nephrotic syndrome (massive urinary protein loss causing severe hypoalbuminaemia — bilateral generalised pitting oedema, facial puffiness, and frothy urine). Hepatic causes include liver cirrhosis causing both ascites (abdominal fluid) and peripheral pitting oedema from hypoalbuminaemia and portal hypertension. Lymphatic causes include lymphoedema (primary — congenital abnormality of lymphatics; or secondary from cancer treatment, infection including filariasis, or trauma) causing non-pitting chronic swelling. Medications commonly causing oedema include calcium channel blockers (amlodipine, nifedipine — up to 30% of users), corticosteroids, NSAIDs, thiazolidinediones, and oestrogen-containing preparations. Allergic causes include angioedema — sudden facial and laryngeal swelling from ACE inhibitors or allergens, which can be life-threatening.
Warning Signs & When It's Serious
Seek emergency care immediately for sudden swelling of the face, lips, tongue, or throat — this is angioedema, which can cause complete airway obstruction. Call emergency services. If epinephrine (adrenaline) auto-injector is available, use it immediately. Also seek emergency assessment for sudden unilateral leg swelling with pain, redness, and warmth — the classic presentation of deep vein thrombosis (DVT), which carries a significant risk of potentially fatal pulmonary embolism. Unilateral leg swelling in a patient with known cancer — particularly pelvic cancer — raises concern for malignant iliac vein or inferior vena cava compression and requires same-day assessment.
How Swelling Is Diagnosed
Clinical assessment establishes the distribution (unilateral versus bilateral), character (pitting versus non-pitting), severity, onset (acute versus chronic, and whether intermittent or progressive), and associated systemic features. The pitting test is performed by pressing firmly on the medial malleolus or pretibial area for ten seconds — depth and duration of indentation is graded 1+ (barely detectable) to 4+ (deep indentation persisting over two minutes). Blood tests are central to the workup: kidney function (serum creatinine, eGFR), serum albumin (low in nephrotic syndrome, cirrhosis, malnutrition), 24-hour urinary protein or spot protein-to-creatinine ratio (nephrotic syndrome — greater than 3.5 g per day), liver function tests, full blood count, and B-type natriuretic peptide (BNP or NT-proBNP) — the most sensitive and specific blood test for heart failure, proportional to severity and prognostic in monitoring response to treatment. Thyroid function tests (TSH, free T4) exclude hypothyroidism — a common, easily missed cause of non-pitting oedema. Doppler ultrasound of the leg veins assesses for DVT with high sensitivity (over 95%) and specificity. Echocardiogram assesses left ventricular systolic function (ejection fraction), diastolic function, valve disease, and pulmonary pressures when heart failure is suspected. Abdominal ultrasound evaluates for ascites, hepatosplenomegaly, and inferior vena cava compression.
Treatment & Management
Treatment of oedema must always target the underlying cause rather than the fluid retention alone. Heart failure oedema is treated with loop diuretics (furosemide — oral or intravenous, doses titrated to achieve daily urine output of approximately one to two litres to achieve net fluid loss), combined with mineralocorticoid receptor antagonists (spironolactone, eplerenone) for additional diuresis and cardiac remodelling benefit, sodium restriction to below two grams per day, and optimised guideline-directed heart failure medications (ACE inhibitors, beta-blockers, SGLT2 inhibitors). DVT is treated with direct oral anticoagulants (DOACs — rivaroxaban or apixaban preferred) for three to six months minimum; below-knee graduated compression stockings (30-40 mmHg) reduce post-thrombotic syndrome risk when worn for two years after proximal DVT. Chronic venous insufficiency is managed with graduated compression stockings (20-30 mmHg for mild, 30-40 mmHg for moderate-severe disease), regular leg elevation above heart level, and daily walking to activate the calf muscle venous pump. Lymphoedema requires complete decongestive therapy: manual lymphatic drainage by trained physiotherapist, compression bandaging, graduated compression garments (24-hour compression), skin care, and prescribed exercise. Medication-induced oedema often responds significantly to dose reduction or switching to an alternative medication class (e.g., switching from amlodipine to ramipril for hypertension). Nephrotic syndrome oedema is managed with diuretics and treatment of the underlying glomerular disease.

Elevate the legs above heart level for at least 30 minutes, two to three times daily when resting — gravity drains interstitial fluid back into the venous circulation. Use a pillow or wedge to keep legs elevated while sleeping. Exercise regularly — walking, swimming, and cycling all activate the calf muscle venous pump, which is the primary mechanism for returning venous blood from the legs to the heart against gravity. Reduce dietary sodium intake to below two grams per day (avoid processed foods, tinned soups, and table salt) to limit dietary-driven fluid retention. Maintain a healthy body weight as obesity significantly worsens all forms of dependent oedema by increasing venous pressure and lymphatic load. Avoid extreme heat for prolonged periods, particularly with known venous insufficiency.

Never dismiss new swelling as trivial without proper assessment — oedema frequently indicates significant underlying cardiovascular, renal, or hepatic disease requiring prompt diagnosis and treatment optimisation. Seek specialist review with a vascular surgeon, nephrologist, or hepatologist depending on the suspected underlying cause.
