Chest X-ray is the minimum imaging in all patients — identifies lung mass, lymphadenopathy, and pleural effusion. CT of the chest, abdomen, and pelvis (CT-CAP) with intravenous contrast is the cornerstone imaging for identifying malignancy, lymphadenopathy, and visceral abnormalities. Upper gastrointestinal endoscopy and colonoscopy evaluate the gastrointestinal tract. PET-CT is used when CT is non-diagnostic but clinical suspicion remains high — particularly in lymphoma. Bone marrow biopsy, tissue biopsy of abnormal lymph nodes or masses, and specialist endoscopy (capsule endoscopy for small bowel) are performed based on findings.
Treatment & Management
Treatment is entirely directed at the identified underlying cause — there is no generic treatment for unexplained weight loss itself. Malignancy requires oncology evaluation with multidisciplinary team-directed management including surgery, chemotherapy, radiotherapy, immunotherapy, or targeted biological therapy depending on tumour type, stage, and molecular characteristics. Hyperthyroidism responds to antithyroid medications (carbimazole, propylthiouracil), radioiodine ablation, or thyroidectomy, with weight recovery typically occurring over six to twelve weeks as the metabolic rate normalises. Coeliac disease reverses completely with strict, lifelong gluten-free diet — villous architecture and nutritional absorption typically recover within six to twelve months of strict adherence. Tuberculosis requires the standard WHO six-month four-drug (HRZE) regimen — isoniazid, rifampicin, pyrazinamide, ethambutol — achieving cure in over 95% of drug-sensitive TB. HIV is treated with combination antiretroviral therapy (cART) — suppression of viral load allows immune reconstitution and weight gain. Any unexplained weight loss exceeding 10% of body weight, regardless of associated symptoms, warrants comprehensive investigation with blood tests, chest X-ray, and CT imaging on an urgent basis. Routine assessment within one to two weeks is appropriate for five to ten percent weight loss over six months without obvious red flag features — this still requires systematic investigation. Never accept a reassurance that unexplained weight loss is benign without a full workup, including imaging where appropriate. Early cancer detection through prompt investigation dramatically improves five-year survival in virtually all tumour types.
