Who Is a Candidate
Medical treatment of sinusitis is appropriate for all patients with symptomatic sinonasal disease. Acute sinusitis is managed with saline irrigation, analgesics, and nasal decongestants; antibiotics are reserved for confirmed bacterial cases meeting criteria. Risks & Potential Complications
Medical treatment of sinusitis is generally safe, with the main risks being intranasal corticosteroid epistaxis (nasal bleeding from directed spray technique — using correct angulation away from the septum minimises this), and risks of systemic corticosteroid use (hyperglycaemia, insomnia, mood changes, adrenal suppression with prolonged high-dose courses). FESS complications range from minor to serious. Major complications occur in less than 0.5% of cases in experienced hands: orbital haematoma (an emergency requiring immediate lateral canthotomy if vision is threatened), CSF leak with risk of meningitis, and injury to the carotid artery (extremely rare, less than 0.01%). Balloon sinuplasty — inflation of a small balloon catheter in the sinus ostium to dilate it — is a less invasive alternative to FESS for selected patients with predominantly ostial obstruction without significant polyp disease. It can be performed under local anaesthesia in the clinic or as a brief procedure under sedation, with limited tissue trauma and faster recovery. However, balloon sinuplasty is appropriate only for limited ostial disease and cannot address polyps, significant mucosal disease, or structural pathology requiring FESS. For acute sinusitis, steam inhalation and warm facial compresses provide symptomatic relief of facial pressure, and oral decongestants (pseudoephedrine, xylometazoline) provide temporary relief of nasal congestion without addressing underlying infection.
