Antrostomy is a surgical procedure that creates or enlarges an opening in the wall of the maxillary sinus (antrum) to improve drainage and ventilation. The maxillary sinuses are the largest paranasal sinuses, located in the cheekbone area on either side of the nose. When the natural ostium (drainage opening) of the maxillary sinus becomes blocked by inflammation, polyps, or anatomical abnormalities, mucus accumulates and recurrent or chronic infections develop. Antrostomy restores normal sinus drainage and function.
Chronic rhinosinusitis (CRS) is one of the most prevalent chronic diseases, affecting approximately 10-12% of the adult population in developed countries. It significantly impacts quality of life, productivity, and healthcare utilization, with direct healthcare costs exceeding $8 billion annually in the United States alone. When medical therapy — including nasal corticosteroids, saline irrigations, and appropriate antibiotics — fails to adequately control symptoms after a minimum of 8-12 weeks, surgical intervention becomes a consideration.
Modern antrostomy is most commonly performed as part of functional endoscopic sinus surgery (FESS), a minimally invasive technique introduced in the 1980s that uses nasal endoscopes and specialized instruments to restore normal sinus drainage through the natural pathways. This represents a fundamental shift from older radical procedures, preserving sinus mucosa and mucociliary function while effectively addressing the underlying obstruction.

Antrostomy is indicated for a variety of conditions affecting the maxillary sinus that have failed to respond to adequate medical management. The procedure addresses the underlying mechanical obstruction that perpetuates sinus disease.

- Antrochoanal polyp — a solitary polyp originating in the maxillary sinus that extends through the ostium into the nasal cavity and nasopharynx
- Odontogenic (dental-origin) sinusitis — sinus infection arising from dental pathology such as periapical abscess, oroantral fistula, or displaced dental implants
- Sinus biopsy or tumor removal — diagnostic access to the maxillary sinus for tissue sampling or removal of benign tumors such as inverted papilloma
Who Is a Candidate
Candidates for antrostomy are patients with documented maxillary sinus disease who have failed an adequate trial of medical therapy. Guidelines recommend a minimum of 8-12 weeks of appropriate medical management — including topical nasal corticosteroids, saline irrigations, and a course of culture-directed antibiotics if bacterial infection is confirmed — before considering surgical intervention. Documentation with CT scanning showing persistent sinus opacification despite medical therapy is standard.
Ideal candidates include patients with anatomical obstruction of the maxillary sinus ostium, recurrent infections (four or more per year), nasal polyposis with maxillary sinus involvement, or fungal sinusitis requiring surgical clearance. Patients should be assessed for underlying conditions that contribute to sinus disease, including allergic rhinitis, asthma, aspirin-exacerbated respiratory disease (Samter's triad), immune deficiency, and ciliary dyskinesia.
Relative contraindications include uncontrolled bleeding disorders, active vasculitis, and significant medical comorbidities that increase anesthetic risk. Anatomical considerations such as prior orbital or facial surgery, variant anatomy of the internal carotid artery or optic nerve (assessed on CT), and active cocaine abuse require special caution. Children are generally managed more conservatively, with adenoidectomy often attempted before sinus surgery, though FESS can be safely performed in pediatric patients with refractory disease.
Treatment Options & Techniques
The most commonly performed technique is endoscopic middle meatal antrostomy (MMA), in which the natural ostium of the maxillary sinus is identified and enlarged using through-cutting instruments, microdebriders, or backbiting forceps. This approach works with the natural mucociliary drainage pathway rather than against it, allowing the cilia to continue sweeping mucus toward the enlarged opening. The procedure is performed entirely through the nostril with no external incisions.
The size of the antrostomy is tailored to the underlying pathology. A standard MMA enlarges the ostium to approximately 1-2 cm in diameter, while a mega-antrostomy or wide maxillary antrostomy (extending posteriorly and inferiorly) may be performed for fungal disease, recalcitrant infection, or to facilitate ongoing access for topical medication delivery. Image-guided navigation (surgical GPS) systems are increasingly used to enhance safety and precision, particularly in revision cases or complex anatomy.
The Caldwell-Luc procedure is an older technique that approaches the maxillary sinus through an incision under the upper lip, creating an opening through the anterior wall of the maxillary sinus. While largely supplanted by endoscopic techniques, it remains useful for specific indications including removal of large benign tumors, closure of oroantral fistulas, removal of displaced dental implants or root tips, and cases where endoscopic access is insufficient.
Balloon sinuplasty is a newer, less invasive alternative in which a catheter with a balloon is threaded into the sinus ostium and inflated to dilate the opening without removing tissue. It is suitable for uncomplicated sinusitis without polyps but is not appropriate for fungal disease, tumors, or extensive polyposis.
Benefits & Expected Outcomes
Endoscopic antrostomy achieves significant improvement in sinus symptoms in 80-90% of patients. Studies using validated quality-of-life instruments such as the Sino-Nasal Outcome Test (SNOT-22) consistently demonstrate substantial and durable improvements in nasal obstruction, facial pressure and pain, discharge, and smell disturbance. The greatest improvement is typically seen in the first 3-6 months following surgery.
The procedure offers several advantages over older surgical approaches and ongoing medical therapy. It is minimally invasive with no external incisions, preserves normal sinus mucosa and mucociliary function, has a rapid recovery time (most patients return to work within 3-7 days), and can be combined with treatment of other sinus regions, septoplasty, or turbinate reduction in a single surgical session. Complication rates are low, with major complications occurring in less than 1% of cases.
Long-term studies show that the benefits of FESS including antrostomy are sustained in most patients, with 75-85% reporting continued improvement at 5-10 year follow-up. Patients with chronic rhinosinusitis without nasal polyps (CRSsNP) tend to have better long-term outcomes than those with nasal polyps (CRSwNP), as the latter have higher rates of polyp recurrence and may require revision surgery or ongoing biologic therapy.
Risks & Complications
Endoscopic antrostomy is generally safe with a low complication rate. Minor complications occur in 5-10% of cases and include postoperative bleeding requiring nasal packing, synechia (adhesion) formation between the middle turbinate and lateral nasal wall, and temporary periorbital swelling or ecchymosis. Synechiae can be managed with office-based lysis and usually do not affect the surgical outcome if addressed promptly during postoperative debridement visits.
Major complications are rare, occurring in less than 0.5-1% of FESS procedures. These include orbital complications such as orbital hematoma, damage to the extraocular muscles (particularly the medial rectus), or breach of the lamina papyracea causing orbital fat prolapse. Intracranial complications — including cerebrospinal fluid leak and intracranial infection — are extremely rare in isolated maxillary antrostomy but can occur when ethmoid or frontal sinus surgery is performed concurrently. Damage to the nasolacrimal duct at the anterior margin of the antrostomy can cause epiphora (excessive tearing).
Postoperative stenosis or closure of the antrostomy opening (cicatricial stenosis) occurs in 3-10% of cases and may require revision surgery. Risk factors for restenosis include inadequate initial antrostomy size, failure to remove the uncinate process completely, postoperative synechiae formation, and ongoing mucosal inflammation. Persistent or recurrent disease requiring revision FESS is reported in approximately 10-20% of patients over 5-10 years.
Recovery & Follow-Up
Postoperative care following antrostomy focuses on promoting healing, preventing adhesion formation, and managing nasal congestion. Patients begin nasal saline irrigations 24-48 hours after surgery and continue them at least 3-4 times daily for several weeks. High-volume, low-pressure irrigation using a squeeze bottle or neti pot is preferred. Topical nasal corticosteroids are typically resumed 3-7 days after surgery to reduce inflammation and polyp recurrence.
Activity restrictions include avoiding nose blowing, heavy lifting, and strenuous exercise for 1-2 weeks to minimize the risk of postoperative bleeding. Most patients experience nasal congestion, mild facial pressure, and blood-tinged discharge for the first 7-14 days. Return to work or school is generally possible within 3-7 days for sedentary activities. Complete mucosal healing takes 4-8 weeks, during which time the sinus cavity transitions through phases of crusting and mucosal regeneration.
Postoperative endoscopic debridement is a critical component of follow-up. Visits are typically scheduled at 1 week, 3 weeks, and 6-8 weeks after surgery. During these visits, the surgeon uses an endoscope to remove crusts, blood clots, and early adhesions from the surgical cavity. This office-based care significantly improves outcomes and reduces the risk of stenosis. Patients with chronic rhinosinusitis should continue long-term medical therapy including nasal corticosteroids and saline irrigations to maintain the surgical results.
Cost Factors
The cost of antrostomy depends on whether it is performed as an isolated procedure or as part of a more comprehensive FESS involving multiple sinuses, septoplasty, or turbinate reduction. In the United States, isolated endoscopic maxillary antrostomy typically costs $3,000-$8,000, while comprehensive bilateral FESS ranges from $8,000-$25,000. Alternative Treatments
Medical management is the first-line treatment for chronic rhinosinusitis and should be optimized before considering surgery. This includes intranasal corticosteroid sprays (fluticasone, mometasone, budesonide), large-volume saline nasal irrigations, short courses of systemic corticosteroids for acute exacerbations or polyposis, leukotriene receptor antagonists, and culture-directed antibiotics for documented bacterial infection. For patients with CRS with nasal polyps, biologic therapies (dupilumab, omalizumab, mepolizumab) have shown significant efficacy and may delay or eliminate the need for surgery.
Balloon sinuplasty is a less invasive office-based or operating room procedure that dilates the sinus ostia without tissue removal. It is FDA-approved for use in the maxillary, frontal, and sphenoid sinuses and may be appropriate for patients with uncomplicated sinusitis without polyps. However, it does not address polypoid disease, fungal balls, or significant anatomical obstruction and has higher recurrence rates than traditional FESS for more complex disease.
For patients with allergic rhinitis contributing to sinusitis, allergy testing and immunotherapy (allergy shots or sublingual tablets) can reduce the inflammatory burden on the sinuses and decrease the frequency of sinus infections. Aspirin desensitization therapy is beneficial for patients with aspirin-exacerbated respiratory disease. In rare cases of medically and surgically refractory disease, maxillary sinus obliteration or modified endoscopic medial maxillectomy may be considered.
