Internal hemorrhoids are graded I–IV by the degree of prolapse: Grade III (prolapsing, manually reducible) and Grade IV (irreducible) are the primary surgical indications. External hemorrhoids complicated by acute thrombosis causing severe pain also warrant prompt excision. Conservative management (high-fiber diet, oral hydration, topical preparations, and avoidance of straining) resolves the majority of Grade I–II internal hemorrhoids. Office-based procedures — rubber band ligation, infrared coagulation, and sclerotherapy — are effective for Grade II and selected Grade III hemorrhoids and do not require anesthesia or operating room resources. Surgical hemorrhoidectomy is recommended when conservative measures and office procedures have failed, when hemorrhoids are large Grade III–IV internal hemorrhoids, or when significant external components coexist that are not amenable to banding. Surgery achieves the highest rate of symptomatic cure and lowest long-term recurrence of any hemorrhoid treatment modality.
Multiple surgical techniques are available, ranging from the traditional open (Milligan-Morgan) and closed (Ferguson) excisional hemorrhoidectomy to newer approaches including stapled hemorrhoidopexy (PPH), transanal hemorrhoidal dearterialization (THD/DGHAL), laser hemorrhoidectomy, and laser ablation. Technique selection depends on hemorrhoid grade and extent, patient anatomy, surgeon expertise, and patient preference regarding postoperative pain and recovery duration. All approaches achieve comparable long-term symptom control in appropriately selected patients, though they differ substantially in postoperative pain levels, return-to-work intervals, and risk of specific complications.

- Prolapsed, thrombosed mixed hemorrhoids (gangrenous hemorrhoids): An acute complication requiring emergency hemorrhoidectomy to prevent sepsis; rare but limb/life-threatening.
Who Is a Candidate
Suitable candidates for hemorrhoidectomy include patients with Grade III or IV symptomatic internal hemorrhoids, those with mixed hemorrhoidal disease that has failed office procedures, and patients with acutely thrombosed external hemorrhoids presenting within 72 hours of onset. Patients should be medically fit for their chosen anesthetic modality (spinal, regional, or general). A complete anorectal examination — including anoscopy and, in patients over 45 or with alarm symptoms, colonoscopy — must be performed to exclude other anorectal pathology (colorectal cancer, inflammatory bowel disease, anal fissure, fistula) that could explain or complicate the hemorrhoidal symptoms.
Relative contraindications and precautions: Anticoagulated patients require careful perioperative medication management; warfarin is typically bridged or held, and direct oral anticoagulants (DOACs) held for appropriate washout periods depending on the agent and renal function. Patients with Crohn's disease have substantially elevated risk of poor wound healing, chronic pain, and fistula formation after hemorrhoidectomy and should generally be managed conservatively. Immunocompromised patients (HIV with low CD4 count, transplant recipients, active chemotherapy) have increased infection risk. Pregnancy is a contraindication to elective hemorrhoidectomy; the majority of hemorrhoids associated with pregnancy resolve postpartum.
Treatment Options & Techniques
Open hemorrhoidectomy (Milligan-Morgan technique): The traditional technique performed predominantly in the UK. Each hemorrhoidal complex is excised using scissors, scalpel, or electrocautery, with wounds left open to heal by secondary intention. The open technique has the lowest recurrence rate of any approach but is associated with the most postoperative pain (due to open wounds in a highly sensitive perianal region) and the longest wound healing time (4–6 weeks). It remains the gold standard against which other techniques are compared.
Closed hemorrhoidectomy (Ferguson technique): The most widely used technique in North America. Hemorrhoidal tissue is excised and wounds are primarily closed with absorbable sutures. Closure reduces wound healing time and is associated with slightly less postoperative pain than the open technique, with equivalent long-term recurrence rates. Primary closure does carry a small risk of wound dehiscence and abscess if significant contamination occurs.
Stapled hemorrhoidopexy (Procedure for Prolapse and Hemorrhoids — PPH): A circular stapling device is introduced through the anal canal and fires a circular row of staples that resects a ring of rectal mucosa and submucosa above the hemorrhoidal cushions, reducing blood supply and retracting prolapsed tissue back into the anal canal without excising the hemorrhoids themselves. PPH causes significantly less postoperative pain than excisional techniques (as the staple line is above the sensitive dentate line) and allows faster return to normal activity (1–2 weeks). However, PPH has a higher long-term recurrence rate (15–25% at 5 years vs. 5–10% for excisional surgery) and specific complications including staple-line stenosis, staple migration, and rectovaginal fistula (rare). PPH is best suited to Grade III internal hemorrhoids with limited external component.
Transanal hemorrhoidal dearterialization (THD / Doppler-guided hemorrhoidal artery ligation — DGHAL): A Doppler probe identifies the six terminal branches of the superior hemorrhoidal artery at the anorectal junction, which are then ligated with absorbable sutures. A mucopexy (lifting suture) repositions prolapsed internal hemorrhoids. THD is essentially painless (no tissue excision), allows same-day discharge, and has recurrence rates of 8–15% at 3 years — higher than excisional hemorrhoidectomy but appropriate for Grade II–III disease, particularly in patients wishing to avoid postoperative pain.
Laser hemorrhoidectomy / laser hemorrhoidoplasty (LHP): A diode or Nd:YAG laser fiber is inserted submucosally into the hemorrhoidal tissue, delivering energy that causes submucosal fibrosis and shrinkage without excising tissue. LHP results in minimal postoperative pain, same-day discharge, and rapid return to work (3–5 days). Published series report symptom relief in 80–90% of Grade II–III hemorrhoids, though long-term controlled data remain more limited than for excisional techniques. Laser excisional hemorrhoidectomy (using laser instead of scalpel/electrocautery for traditional excision) reduces intraoperative bleeding and may reduce postoperative discomfort compared to electrocautery excision.

Risks & Complications

- Postoperative pain: The most significant concern with conventional excisional hemorrhoidectomy; moderate-to-severe pain for 1–3 weeks is expected. Multimodal analgesia (regular acetaminophen, NSAIDs, topical lidocaine, sitz baths) is essential. Recovery & Follow-Up

Pain management is the primary concern — a scheduled regimen of acetaminophen (1 g four times daily) plus an NSAID (ibuprofen or ketorolac), with topical lidocaine gel and warm sitz baths (10–15 minutes, 2–3 times daily) provides effective analgesia for most patients. Opioid analgesics are prescribed for breakthrough pain but should be minimized to avoid constipation, which worsens pain significantly.
Wound care and diet (Weeks 1–4): A high-fiber diet (25–35 g/day, supplemented with psyllium husk if dietary intake is inadequate) and adequate fluid intake (minimum 2 L/day) are mandatory to produce soft, easily passed stools. Straining must be avoided. Sitz baths after each bowel movement promote wound cleanliness and sphincter relaxation. First bowel movement typically occurs on days 2–4 and is usually uncomfortable; patients should be forewarned and reassured.
Return-to-activity timeline: Sedentary work: 7–14 days (conventional); 3–7 days (PPH/THD/laser). Physical labor or prolonged sitting: 3–6 weeks. Exercise and sports: 4–6 weeks. Driving: when able to perform emergency braking without discomfort (typically 1–2 weeks). Sexual activity: 4–6 weeks.
Follow-up appointments: Clinic review at 4–6 weeks to assess wound healing and exclude stenosis. Patients with persistent or recurrent symptoms should be reviewed at 3–6 months. Long-term, patients are advised to maintain a high-fiber diet indefinitely to reduce recurrence risk, avoid prolonged sitting on the toilet, and report any rectal bleeding to exclude neoplastic causes.

RBL provides symptom control in 70–80% of Grade I–II hemorrhoids with multiple sessions; long-term recurrence rates are higher than surgery. Post-banding pain can be significant but is self-limiting.
- Sclerotherapy: Injection of a sclerosing agent (phenol in oil, sodium tetradecyl sulfate) into the submucosal tissue reduces blood supply and causes fibrosis. Effective for Grade I–II bleeding hemorrhoids; less effective than RBL for prolapse. Very low complication rates.
- Infrared photocoagulation (IRC): Infrared energy coagulates hemorrhoidal tissue. Effective for Grade I–II internal hemorrhoids with minimal bleeding; multiple sessions usually needed.
- Conservative management: Dietary fiber supplementation (Cochrane reviews show significant reduction in bleeding and risk of symptoms persisting), increased fluid intake, topical preparations (hydrocortisone, lidocaine), sitz baths, and avoidance of straining. Adequate for most Grade I–II hemorrhoids.
- Hemorrhoid energy therapy (HET bipolar system): A disposable anoscope-mounted bipolar device coagulates hemorrhoidal tissue in office settings; equivalent to RBL for Grade I–II disease.
