Ovarian Cyst Treatment — Overview
Ovarian cysts are fluid-filled or solid sacs within or on the surface of the ovary. They are extremely common — approximately 20% of women have an ovarian cyst at any given time, and the vast majority are benign. The management of an ovarian cyst is determined by its nature (type, IOTA classification), size, symptoms, patient age, and malignancy risk rather than the mere presence of a cyst.
Ovarian cysts are broadly classified by their imaging characteristics: simple benign cysts — unilocular, thin-walled, purely anechoic (fluid-only); complex cysts — with solid components, irregular walls, internal echoes, or septations (intermediate risk); and features suggestive of malignancy (vascular solid components, bilateral, ascites, peritoneal deposits). The IOTA ADNEX model (Assessment of Different NEoplasias in the adneXa) is the validated risk stratification tool, providing probability of specific ovarian pathology types — recommended by ESGO/ISUOG guidelines for adnexal mass characterisation. For inconclusive ultrasound, MRI provides superior soft tissue characterisation.
The most common ovarian cysts in reproductive age women are functional cysts — follicular cysts (developing follicle fails to rupture, grows to 3–8 cm) and corpus luteum cysts (post-ovulation; may fill with blood, appearing haemorrhagic on ultrasound). These almost universally resolve spontaneously within 1–3 menstrual cycles. Other benign types: dermoid cysts (mature teratomas) — contain ectoderm-derived tissue (teeth, hair, fat); most common ovarian neoplasm in women under 30; bilateral in 10–15%; require laparoscopic cystectomy. Endometriomas — ovarian 'chocolate cysts' of endometriosis. Serous and mucinous cystadenomas — common benign epithelial cysts, may grow large. Malignant ovarian tumours require urgent specialist oncological assessment.
Types of Ovarian Cysts and Their Management
- Functional cysts (follicular and corpus luteum): Premenopausal women: simple, unilocular cysts <5 cm in reproductive age women — watchful waiting with repeat ultrasound in 6–12 weeks. Resolution rate: 75–80% at 6 weeks; 95% by 12 weeks. No medical treatment (OCP does not accelerate resolution of existing functional cysts — disproven in RCTs — though OCP use prevents new functional cysts). - Dermoid cyst (mature cystic teratoma): Pathognomonic ultrasound features (fat, calcification, hair — 'Rokitansky nodule'). Growth rate average 1.8 mm/year. Indications for surgery: symptomatic; >5–6 cm (torsion risk increases significantly above 5 cm); desire to confirm diagnosis; growth. Laparoscopic cystectomy (cyst wall removal, preserving surrounding healthy ovarian cortex) is the preferred approach. Key technical point: dermoid contents must be contained during removal — rupture risks chemical peritonitis (sebum spillage), though usually manageable with thorough irrigation. Recurrence after cystectomy: 3–5%.
- Endometrioma (ovarian endometriosis): Laparoscopic cystectomy (stripping technique) is preferred over fenestration and ablation — lower recurrence rate (20% vs. 40% at 2 years). Impact on ovarian reserve: cystectomy can reduce AMH 30–50% — measure AMH before surgery; weigh fertility impact carefully; in women with poor ovarian reserve, particularly with bilateral endometriomas, watchful waiting and direct IVF may be preferable to repeated cystectomy. - Postmenopausal ovarian cyst: Higher malignancy risk — any ovarian cyst in a postmenopausal woman requires careful assessment. Simple, unilocular cysts <5 cm with normal CA-125: low malignancy risk; watchful waiting with 6-monthly ultrasound and annual CA-125 — accepted management by RCOG/NICE. - Ovarian torsion — gynaecological emergency: Ovarian torsion (twisting of the ovary and/or tube on its vascular pedicle) causes ischaemia and is a surgical emergency. Presentation: acute severe unilateral pelvic pain, nausea/vomiting, peritonism; may have preceding similar episodes (spontaneous partial torsion). Ultrasound: reduced or absent Doppler flow (not reliably present in all cases — clinical suspicion drives surgical decision). Treatment: emergency laparoscopy — detorsion (untwisting) of the ovary and cystectomy. Modern practice: detorsion should be attempted even if the ovary appears darkened/ischaemic — ovarian recovery after detorsion is possible in 85–90% of cases (ovary should not be removed solely on appearance). Cystectomy at same procedure. Time to surgery is critical — delay beyond 24 hours significantly worsens ovarian salvage rate.

Risk stratification assessment includes:
- Transvaginal ultrasound (TVUS) by experienced sonographer — IOTA classification of adnexal morphology; ADNEX model malignancy probability calculation; assessment of solid components, vascular flow (Doppler), septations, wall irregularity, bilateral involvement, and ascites
- MRI pelvis: when ultrasound is inconclusive; characterisation of dermoid cysts; assessment of extent of endometriosis; evaluation of large or complex cysts pre-operatively
- CA-125: most useful in postmenopausal women (80% of ovarian cancers are epithelial — elevated CA-125 in 80% of advanced ovarian cancer). Less useful in premenopausal women (CA-125 elevated in endometriosis, fibroids, PID, peritonitis, adenomyosis — many benign conditions). HE4 + CA-125 combined (ROMA score): better discrimination of benign vs. malignant in postmenopausal women than CA-125 alone.
- Serum AFP, LDH, hCG: for germ cell tumours in young women (particularly dermoid + solid component, or in women aged <25 with a solid ovarian mass)
Surgical indication thresholds (benign cyst):
- Cyst ≥5–6 cm persisting >3 menstrual cycles (in premenopausal women)
- Symptomatic cyst (pain, pressure) regardless of size
- Complex features: solid components, irregular thick walls, multiple septations, bilateral, increased vascularity (Doppler)
- Dermoid ≥5 cm or symptomatic — torsion risk; confirmed or highly suspected dermoid <5 cm asymptomatic: watchful waiting acceptable
- Any postmenopausal cyst with ADNEX malignancy probability >10–20% or elevated CA-125
