Overview of Ovarian Cysts and Their Management
Ovarian cysts are fluid-filled or semi-solid sacs that develop on or within the ovary. They are extremely common: most premenopausal women develop at least one functional cyst during their reproductive lifetime, the majority of which resolve spontaneously within 1–3 menstrual cycles without any treatment.
Ovarian cysts are broadly classified as functional or pathological. Functional cysts — follicular cysts and corpus luteum cysts — arise from the normal physiology of ovulation and are almost invariably benign and self-limiting. Pathological cysts include endometriomas (chocolate cysts from endometriosis), mature cystic teratomas (dermoid cysts), serous and mucinous cystadenomas, borderline ovarian tumours, and primary ovarian malignancies.
Management strategy depends critically on four factors: cyst morphology on ultrasound (simple vs complex), menopausal status, symptoms (pain, pressure, bloating), and serum tumour markers. The International Ovarian Tumour Analysis (IOTA) group's ADNEX model (Assessment of Different NEoplasias in the adneXa, validated 2014) uses ultrasound characteristics and clinical data to estimate probability across five categories: benign, borderline, Stage I cancer, Stage II–IV cancer, and metastatic cancer. ADNEX outperforms CA-125 alone and is increasingly recommended as the preferred malignancy stratification tool.
The NICE Guideline NG126 (Ovarian Cancer: Recognising and Referring, 2023 update) and RCOG Green-top Guideline 34 provide the current UK evidence framework for ovarian cyst management across the reproductive lifespan.
Types of Ovarian Cysts and Their Characteristics
Understanding the cyst type is fundamental to appropriate management. Each type has a distinct natural history, malignancy risk, and treatment pathway:
- Follicular cysts: Arise when a dominant follicle fails to release its egg and continues to grow (typically 3–8 cm). Almost always benign and asymptomatic; resolve spontaneously within 1–3 cycles. Most common type in premenopausal women.
- Corpus luteum cysts: Develop from the corpus luteum post-ovulation. May cause unilateral pelvic pain and, rarely, haemoperitoneum if they rupture or haemorrhage (a cause of acute surgical abdomen). Usually resolve within 6–8 weeks.
- Endometriomas (chocolate cysts): Endometrial glands and stroma implant on and within the ovary, forming cysts containing dark altered blood ("chocolate" fluid). Associated with endometriosis; impair ovarian reserve and IVF outcomes; high recurrence rate post-surgery. Require specific management strategies per ESHRE and RCOG guidance.
- Mature cystic teratoma (dermoid cyst): Most common benign ovarian neoplasm in women under 45 years. Contains ectodermal derivatives (teeth, hair, sebum). Usually slow-growing; small risk of torsion (<2%). Bilateral in 10–15% of cases. Requires surgical excision; does not resolve spontaneously.
- Serous and mucinous cystadenomas: Benign epithelial cysts that may grow very large (mucinous cystadenomas can reach 30+ cm). Resection recommended due to size, growth potential, and need to exclude borderline or malignant forms.
- Borderline ovarian tumours: Low malignant potential; management requires gynaecological oncology input. Fertility-sparing surgery (cystectomy or unilateral salpingo-oophorectomy) is feasible for early-stage disease.
- PCOS-related follicles: Polycystic ovaries contain multiple small antral follicles (<10 mm) arranged peripherally. These are NOT true ovarian cysts and do not require surgical removal. Management focuses on hormonal, metabolic, and fertility-related aspects of PCOS.
Eligibility and Risk Stratification
The decision to treat or monitor an ovarian cyst depends on systematic risk stratification using validated tools and guideline-based thresholds:
- NICE NG126 conservative management thresholds: For premenopausal women, simple unilateral unilocular cysts smaller than 5 cm with normal CA-125 may be managed conservatively with a follow-up ultrasound. Cysts 5–7 cm warrant 3-monthly surveillance. Cysts >7 cm require further MRI characterisation or surgical evaluation. For postmenopausal women, simple unilocular cysts <1 cm with normal CA-125 require no follow-up; cysts 1–5 cm require annual surveillance with CA-125 measurement.
- CA-125 limitations: CA-125 is elevated in approximately 80% of advanced epithelial ovarian cancers but is non-specific: it is also elevated in endometriosis, pelvic inflammatory disease, uterine fibroids, pregnancy, pancreatitis, and other benign conditions. CA-125 should be interpreted alongside USS morphology, not as a standalone marker, and IOTA ADNEX provides superior discrimination.
- Risk of Malignancy Index (RMI): RMI combines USS morphology score, menopausal status, and CA-125 level. RMI ≥200 is the threshold for urgent referral to a gynaecological oncology multidisciplinary team.
- IOTA ADNEX model: Requires trained sonographer input of specific USS features (e.g., number of papillary projections, acoustic shadows, ascites). Provides individualised malignancy probability estimates and is recommended by RCOG and ESGO as superior to RMI alone.
- Symptomatic presentation: Severe pain, rapidly enlarging cyst, features of ovarian torsion (acute severe unilateral pain, vomiting), or haemoperitoneum warrants urgent gynaecological assessment regardless of size.
Menopausal status is the single most important clinical variable: a 5 cm simple cyst in a 28-year-old is very likely benign and functional; the same cyst in a 65-year-old requires thorough malignancy exclusion.
Treatment Options
Ovarian cyst management ranges from watchful waiting to surgical intervention, determined by cyst characteristics and clinical context:
- Conservative management (watchful waiting): Appropriate for premenopausal women with simple unilocular cysts <5 cm and normal CA-125 per NICE NG126. Serial ultrasound at 6 weeks (if symptomatic) or at 12 months (if asymptomatic). Most functional cysts resolve within 1–3 menstrual cycles without intervention. No dietary, herbal, or hormonal treatment accelerates resolution of existing functional cysts.
- Oral contraceptive pill (OCP): Does not shrink existing ovarian cysts. However, combined OCP suppresses ovulation and prevents formation of new functional cysts — appropriate in women with recurrent functional cysts. Progestogen-only pill or hormonal IUS are alternatives.
- Laparoscopic cystectomy (cyst wall excision): Standard surgical approach for benign pathological cysts (dermoids, endometriomas, cystadenomas) and for cysts >5–7 cm or those failing to resolve. The cyst wall is stripped from the surrounding ovarian tissue, preserving as much healthy ovarian cortex as possible. RCOG Green-top Guideline 34 recommends the laparoscopic approach for suspected benign cysts in preference to open laparotomy. Cystectomy preserves fertility and ovarian reserve compared to oophorectomy, though reserve is inevitably reduced for endometriomas.
- Laparoscopic oophorectomy: Removal of the entire ovary. Indicated for postmenopausal women (where ovarian preservation is less critical), suspected malignancy, cysts not amenable to cystectomy, and in women who have completed their family and have recurrent cysts. Bilateral salpingo-oophorectomy (BSO) is performed for confirmed malignancy.
- GnRH agonist therapy: Used pre-operatively to suppress endometriomas before IVF treatment cycles, or to induce temporary amenorrhoea to reduce endometrioma recurrence risk after surgery. Not a primary treatment for structural cysts.
- Ultrasound-guided aspiration: Draining cyst fluid under ultrasound guidance. High recurrence rates (>50%) limit its use; generally reserved for selected symptomatic simple cysts in poor surgical candidates, or during egg collection cycles for IVF.
Benefits of Ovarian Cyst Treatment
The appropriate treatment approach delivers significant clinical benefits matched to the individual patient's presentation and goals:
- Conservative management benefits: Avoids surgical risk and ovarian reserve damage for the majority of functional cysts that will resolve spontaneously. Reduces unnecessary theatre exposure and anaesthetic risk. Reduces cost. Appropriate watchful waiting for simple cysts is safe — NICE NG126 data confirm no increase in malignancy-related mortality from appropriate conservative protocols.
- Laparoscopic cystectomy benefits: Removes symptomatic cysts (pain, pressure, dyspareunia) definitively. Provides histopathological diagnosis — critical for excluding borderline tumours and malignancy, and for confirming endometriosis. Preserves ovarian tissue and ovarian reserve compared to oophorectomy. Minimally invasive approach: typically 1–2 night hospital stay, reduced adhesion formation compared to open surgery, faster recovery.
- Endometrioma treatment benefits: Surgical excision of endometriomas reduces dysmenorrhoea, dyspareunia, and chronic pelvic pain in the majority of patients. Pre-IVF management of endometriomas (surgery or GnRH suppression) may improve egg quality and retrieval outcomes in selected patients, though evidence is nuanced.
- Malignancy detection: Systematic IOTA/RMI-based stratification ensures women at elevated risk are expedited to gynaecological oncology, maximising the chance of early-stage diagnosis when curative surgery is most effective. Ovarian cancer detected at Stage I has a 5-year survival rate exceeding 90%.
- Quality of life: Resolution of bloating, pelvic pain, urinary frequency, and sexual discomfort associated with large or symptomatic cysts significantly improves daily functioning and wellbeing.
Risks and Complications
Both conservative and surgical management carry specific risks that must be discussed with patients before a treatment plan is agreed:
- Ovarian reserve damage (cystectomy for endometrioma): The most clinically significant risk of laparoscopic cystectomy. The stripping technique inadvertently removes healthy primordial follicles embedded in the cyst wall, reducing anti-Müllerian hormone (AMH) levels and diminishing ovarian reserve. This effect is greater with bilateral endometrioma surgery and increases with each repeat cystectomy. Repeat surgery for endometrioma recurrence carries a significant risk of premature ovarian insufficiency. Fertility counselling and consideration of egg freezing before surgery is recommended for premenopausal women.
- Recurrence: Endometrioma recurrence rates are 40–50% at 5 years without post-operative hormonal suppression. Functional cyst recurrence is common and expected. Dermoid cyst recurrence is rare (<5%). Cystadenoma recurrence after complete excision is low.
- Intraoperative haemorrhage: Injury to ovarian hilum vessels during cystectomy (1–2% requiring conversion to oophorectomy). Rarely, damage to adjacent structures (ureter, bowel, bladder) occurs during laparoscopic surgery (<1%).
- Adhesions: Post-operative pelvic adhesions occur in a minority of patients and may impair tubal patency, contributing to subfertility. Adhesion risk is lower with laparoscopic than open surgery.
- Premature menopause: If bilateral oophorectomy is performed in premenopausal women, immediate surgical menopause ensues with risks of cardiovascular disease, osteoporosis, and cognitive effects; hormone replacement therapy (HRT) is recommended until the natural age of menopause.
- Failure to diagnose malignancy: Over-reliance on serum CA-125 without IOTA/RMI stratification risks undertriage of borderline tumours or early-stage malignancy. All excised cysts must be sent for histopathology.
Follow-Up and Surveillance
Follow-up protocols are determined by cyst type, treatment choice, and individual malignancy risk factors, primarily guided by NICE NG126:
- Conservative management — NICE NG126 intervals: Simple cysts <5 cm premenopausal: repeat USS at 12 months; if unchanged and asymptomatic, discharge. Simple cysts 5–7 cm: repeat USS at 4-month intervals for 1 year. Any cyst developing solid components, new septations, or increasing CA-125: urgent gynaecological oncology referral.
- Postmenopausal surveillance: Simple cysts 1–5 cm: annual USS and CA-125 for a minimum of 3 years (or until menopause for ≥3 years without change). Any suspicious morphological change warrants immediate specialist review.
- Post-cystectomy follow-up: USS at 6–12 weeks post-operatively to confirm cyst resolution and exclude early recurrence. Histopathology result review to confirm benign diagnosis or guide further management if unexpected findings (e.g., borderline tumour on final pathology).
- Endometrioma post-surgical management: Hormonal suppression strongly recommended to reduce recurrence: combined OCP (continuous), progestogen-only pill, GnRH agonist, or levonorgestrel IUS. Annual surveillance USS while on suppressive therapy. Pre-conception counselling regarding ovarian reserve and IVF timing.
- Borderline tumour follow-up: Gynaecological oncology-led surveillance with 6-monthly USS and CA-125 for a minimum of 5 years; annual thereafter. Fertility-sparing surgery patients require particularly close surveillance.
- Red-flag symptoms requiring urgent review: New onset severe unilateral pain (possible torsion or haemorrhage), rapidly increasing abdominal girth, new ascites, unexplained weight loss — require same-day or next-day assessment.
Cost Factors
The cost of ovarian cyst treatment varies widely based on management approach, clinical setting, and geography:
- Conservative management: Ultrasound surveillance is the lowest-cost option. NHS: funded. Private UK: USS approximately £150–350 per scan; CA-125 blood test £50–100 private.
- Laparoscopic cystectomy (UK private): Approximately £3,000–£7,000 all-inclusive (consultation, anaesthesia, theatre, histopathology, one post-operative review). NHS: funded when meeting clinical criteria. Robotic-assisted laparoscopic cystectomy adds significant cost (>£10,000 private) with no demonstrated clinical benefit over standard laparoscopic cystectomy for benign cysts.
- Complexity factors increasing cost: Bilateral cysts vs unilateral; concurrent salpingo-oophorectomy; suspected borderline tumour (requires frozen section); endometriosis with adhesiolysis (longer theatre time); conversion to open laparotomy; overnight vs day-case admission.
- Histopathology: Mandatory for all excised cysts; included in NHS funding; additional cost in private settings (~£150–400).
- GnRH agonist suppression therapy (e.g., leuprorelin, goserelin): Monthly or 3-monthly depot injection; NHS prescription-funded when licensed; private cost £120–350 per injection.

Alternatives to Standard Treatment
Alternative management approaches exist depending on the cyst type, patient preferences, and clinical circumstances:
- Watchful waiting (for most functional cysts): The most appropriate "alternative" to surgery for premenopausal women with simple cysts <5 cm per NICE NG126. Most resolve spontaneously, avoiding any surgical risk. Serial ultrasound monitoring provides safety netting.
- Oral contraceptive pill: Does not shrink existing cysts but prevents new functional cyst formation by suppressing ovulation. Appropriate for women with recurrent functional cysts who wish to avoid repeated surveillance or surgery.
- GnRH agonist therapy for endometrioma: Three to six months of GnRH agonist (e.g., leuprorelin) before IVF reduces endometrioma size and may improve egg retrieval outcomes. Not a definitive treatment — cysts recur after cessation — but a valid alternative to surgery in specific fertility contexts.
- Ultrasound-guided aspiration: Suitable for patients unfit for surgery or with a large symptomatic simple cyst. High recurrence rate (>50% within 12 months for functional and cystadenoma cysts) limits its use to selected circumstances.
- Open laparotomy: Reserved for very large cysts (>10–15 cm), suspected malignancy (where laparoscopic port-site seeding risk must be avoided), or where laparoscopic access is not feasible. Associated with longer recovery, greater adhesion risk, and higher post-operative morbidity than laparoscopy.
- Expectant management during pregnancy: Ovarian cysts identified in the first trimester of pregnancy are monitored conservatively unless torsion or malignancy is suspected. Most simple cysts resolve by mid-pregnancy. Laparoscopic surgery, if required, is safest in the second trimester.
- Complementary therapies: No herbal remedy, dietary supplement, or complementary therapy has demonstrated efficacy in resolving ovarian cysts in rigorous clinical trials. Patients should be advised that these approaches do not substitute for evidence-based ultrasound surveillance.
