Severity assessment tools used in clinical practice:

Eligibility: Mild, Moderate, and Severe Disease Classification
Treatment pathway is determined by disease severity using PASI, BSA, and DLQI:
- Mild psoriasis (PASI <10, BSA <10%, DLQI ≤10): Topical therapy alone is appropriate. Over-the-counter emollients, prescribed topical corticosteroids (potent or very potent), vitamin D analogues (calcipotriol), and their combination (Enstilar/Dovobet foam) are first-line. Patient education on the chronic relapsing-remitting nature, trigger avoidance (stress, streptococcal infections, beta-blockers, NSAIDs, lithium, withdrawal of systemic steroids), and emollient use is foundational.
- Moderate-to-severe psoriasis (PASI ≥10 or BSA ≥10% and DLQI >10): Qualifies for escalation to phototherapy or systemic therapy after inadequate topical response. NICE specifies phototherapy or conventional systemic therapy before biologic eligibility in most cases.
- Biologic eligibility (NICE criteria): PASI ≥10 and DLQI >10 plus either: (a) phototherapy failure, intolerance, or contraindication, or (b) conventional systemic failure, contraindication, or intolerance. At least one conventional systemic (methotrexate, ciclosporin, or acitretin) must have been trialled unless contraindicated. Pregnancy, active severe infection, latent TB (requiring 2-month LTBI treatment before anti-TNF initiation), and active malignancy require individual risk assessment.
- Special populations: Children (<18 years): etanercept, adalimumab, secukinumab, and ixekizumab licensed for paediatric plaque psoriasis. Pregnancy: ciclosporin (safest option); avoid methotrexate, acitretin, and most biologics in first trimester. Psoriatic arthritis co-existent: prefer IL-17 or IL-23 biologics or TNF inhibitors with evidence for both skin and joint outcomes.
Treatment Options: Step-Up Therapeutic Ladder
Step 1: Topical Therapy (Mild Psoriasis)
- Topical corticosteroids: Cornerstone of mild psoriasis management. Potent (betamethasone valerate 0.1%) or very potent (clobetasol propionate 0.05%) preparations used for body plaques; milder agents (hydrocortisone 1%) for face and flexures to avoid atrophy and telangiectasia. Step 2: Phototherapy (Moderate-to-Severe / Extensive Disease)
- Narrowband UVB (NB-UVB, 311–313 nm): Treatment of choice for widespread plaque and guttate psoriasis; 30+ sessions over 10–15 weeks. Clearance rates 60–75%; superior to broadband UVB. Safe in pregnancy; preferred over PUVA for most patients. Minimal erythemogenic dose (MED) calculated to personalise dosing. Maintenance therapy extends remission.
- PUVA (psoralen + UVA): 8-MOP oral psoralen + UVA irradiation; 20+ sessions over 8–10 weeks. Cumulative lifetime dose limit (1,500 J/cm² UVA) due to PUVA-related squamous cell carcinoma risk and ocular damage.
Step 3: Conventional Systemic Therapy
- Methotrexate (MTX): Anchor conventional systemic therapy for moderate-to-severe plaque psoriasis and psoriatic arthritis. Initial 5 mg test dose; escalate to 15–25 mg weekly (oral or SC). Folic acid 5 mg once weekly (or daily, omitting MTX day) reduces mucositis and hepatotoxicity. LFTs, FBC, renal function monitored regularly. Hepatotoxicity (fibrosis with cumulative dose >1.5 g) monitored by enhanced liver fibrosis (ELF) blood test (replacing routine biopsy). Teratogenic; contraception required in both sexes.
- Ciclosporin: Rapid onset (2–4 weeks); useful for severe flares, erythroderma, and as a bridging therapy. 2.5–5 mg/kg/day; limit to 1–2 years continuous use due to nephrotoxicity and hypertension. Monitor BP and creatinine fortnightly initially. Not suitable for long-term maintenance.
- Acitretin: Retinoid; particularly effective for palmoplantar, erythrodermic, and pustular psoriasis. 25–50 mg/day. Potent teratogen (2-year contraception after stopping). Dyslipidaemia and hepatotoxicity monitoring required. Synergistic with NB-UVB (Re-UVB or RePUVA) allowing lower doses of each modality.
Step 4: Biologic Therapy (Moderate-to-Severe; NICE-Eligible)
- IL-17A inhibitors: Secukinumab 300 mg SC monthly (ERASURE/FIXTURE trials; PASI90 at 16 weeks: 59–79%). Ixekizumab 80 mg SC every 2–4 weeks (UNCOVER trials; PASI100 in 35%). - IL-23 p19 inhibitors: Risankizumab 150 mg SC every 12 weeks (IMMvent, IMMhance; PASI90 at 52 weeks 81%). Guselkumab 100 mg SC every 8 weeks (VOYAGE 1/2; PASI90 75%). - Secukinumab (ERASURE/FIXTURE trials): PASI90 in 59–79% and PASI100 in 28–44% at 16 weeks — significantly superior to etanercept and ustekinumab. Maintained efficacy at 52 weeks. Also effective for nail, scalp, and palmoplantar disease.
- Risankizumab (IMMhance, 52 weeks): PASI90 in 81% and PASI100 in 56% at 52 weeks — among the highest sustained response rates reported for any biologic. Quarterly injections after induction reduce patient burden.
- Bimekizumab (BE SURE trial, 16 weeks): PASI100 (complete skin clearance) in 67% versus 46% for secukinumab — a clinically meaningful superiority. Highest PASI90 and PASI100 rates reported across head-to-head biologic trials to date.
- Treat-to-target (PASI90 at week 16): European consensus recommends switching or intensifying therapy if PASI90 is not achieved at 16 weeks. This strategy prevents prolonged exposure to suboptimal treatments and drives earlier, more complete skin clearance with associated DLQI improvement.
Risks, Side Effects, and Safety Monitoring
Safety monitoring requirements vary significantly across psoriasis treatment modalities:
- Topical corticosteroids: Skin atrophy, telangiectasia, striae, and perioral dermatitis with prolonged use on sensitive skin areas. Hepatotoxicity: fibrosis with cumulative dose >1.5 g; ELF test (pro-C3, hyaluronic acid, TIMP-1) replaces routine biopsy in most guidelines. Bone marrow suppression (FBC monitoring essential; avoid in impaired renal function). Teratogenic (men and women must use effective contraception during treatment and for 3 months after). Follow-Up: Treat-to-Target Monitoring

Topical therapy:
