Tonsillectomy in adults is the surgical removal of the two palatine tonsils — lymphoid tissue masses situated on either side of the back of the throat — in patients aged 18 years and older. Pre-Operative Medical Assessment

Surgical Techniques for Adult Tonsillectomy

No single technique has been definitively proven superior for all patients, and outcomes are influenced significantly by surgical volume and experience.

Cold Steel Dissection (Extracapsular)

The historical gold standard. The tonsil is dissected from its capsule using scissors and a dissector, with bleeding controlled by ties, sutures, or bipolar diathermy. This technique involves minimal thermal spread, potentially reducing post-operative pain and promoting faster mucosal healing, though operative blood loss may be slightly greater. High-volume ENT centres continue to use this technique with excellent outcomes.

Electrocautery (Hot Dissection)

Monopolar or bipolar electrocautery dissects and cauterises simultaneously, reducing intraoperative bleeding. However, thermal spread into surrounding tissues can cause more post-operative pain and may theoretically increase the risk of secondary haemorrhage. It is one of the most widely used techniques globally due to its efficiency.

Coblation Tonsillectomy

Radiofrequency energy is delivered in a saline medium to dissolve tissue at low temperatures (40–70°C), reducing thermal damage to surrounding structures. Multiple meta-analyses suggest coblation tonsillectomy is associated with less post-operative pain and equivalent haemorrhage rates compared to electrocautery, making it increasingly popular in both adult and paediatric tonsillectomy.

Laser Tonsillectomy

Pain
