Dermal fillers have an excellent safety profile when injected by qualified, experienced practitioners with detailed anatomical knowledge. Temporary bruising and swelling are the most common side effects. The most serious complication — vascular occlusion with skin necrosis or vision loss — is extremely rare (estimated at less than 1 in 10,000 treatments) but underscores the importance of choosing a trained medical professional (board-certified dermatologist, plastic surgeon, or aesthetic physician) over non-medical providers. Hyaluronic acid fillers have the added safety advantage of being reversible with hyaluronidase enzyme.
Several measures extend the longevity of professional wrinkle treatments: (1) Daily broad-spectrum SPF 30+ sunscreen prevents UV damage from undoing treatment gains; (2) Prescription retinoid use stimulates ongoing collagen synthesis between treatment sessions; (3) Avoiding smoking and excessive alcohol preserves skin quality; (4) Maintaining regular treatment intervals prevents wrinkles from becoming deeply established between sessions; (5) Antioxidant serums (vitamin C) provide complementary photoprotection; and (6) Adequate hydration and nutrition support collagen and hyaluronic acid maintenance in the skin.
A surgical facelift (rhytidectomy) physically repositions and removes excess skin and tightens the underlying SMAS layer, producing the most dramatic and longest-lasting results (7–12 years). Non-surgical facelift treatments — including combinations of botulinum toxin, dermal fillers, RF microneedling, HIFU (Ultherapy), and thread lifts — can meaningfully improve facial contour and wrinkles without surgical incisions or general anesthesia, with minimal downtime. However, they cannot replicate the tissue repositioning of surgery. They are best suited for patients in their 30s–50s with mild to moderate laxity, or as maintenance after surgical rejuvenation. For significant skin laxity and jowling, surgery remains the gold standard.
