Muscle strain, commonly called a pulled muscle, is an acute injury involving overstretching or partial to complete tearing of muscle fibres or the musculotendinous junction. It is the most common soft tissue injury in sports and physical activity. Strains are classified by severity: Grade I (mild — minor fibre tearing, minimal strength loss, return to sport in 1-3 weeks), Grade II (moderate — significant partial tear, localised weakness and bruising, 3-8 weeks recovery), and Grade III (severe — complete muscle or musculotendinous rupture requiring surgical consideration, 3-6 months recovery). The hamstrings, quadriceps, calf (gastrocnemius), groin (adductors), and lumbar erector spinae are the most commonly affected muscles. Hamstring strains alone account for 12-16% of all athletic injuries in running and sprinting sports.
Causes & Risk Factors
Muscle strains typically occur when a muscle is subjected to excessive tensile force — either from a sudden explosive contraction (sprinting, jumping), forced stretch beyond the muscle's elastic limit, or fatigue-induced failure during prolonged activity. The musculotendinous junction is the most vulnerable point, as it represents a zone of high mechanical stress. Risk factors for muscle strain include inadequate warm-up before exercise, muscle fatigue (most strains occur in the latter stages of training or competition), muscle imbalances (e.g., hamstring-to-quadriceps strength ratio below 0.6), reduced flexibility, previous strain (the scar tissue formed during healing is less extensible than normal muscle), age-related muscle stiffening, dehydration, and electrolyte imbalances (reducing muscle contractile efficiency). Sports with high strain risk include sprinting, football, basketball, rugby, and athletics.
Symptoms & Signs
At the moment of injury, patients typically experience sudden acute pain described as a sharp 'pop' or 'snap' sensation within the muscle, often causing immediate cessation of activity. Symptoms include localised pain and tenderness on palpation over the affected muscle or musculotendinous junction, swelling and muscle spasm, bruising (ecchymosis appearing within 24-48 hours — may be extensive in complete ruptures), weakness or inability to contract the affected muscle, and a palpable defect or gap in Grade III complete ruptures. In hamstring strains, pain is felt in the posterior thigh and exacerbated by hip flexion with extended knee. Calf strains present with pain in the posterior lower leg, worsened by ankle plantarflexion and toe raises. Lower back strains cause paraspinal tenderness and pain worsened by movement, twisting, and bending.
Diagnosis & Investigations
Muscle strain is primarily a clinical diagnosis based on mechanism of injury, symptoms, and physical examination. Examination findings: localised tenderness along the muscle belly or musculotendinous junction, pain on active contraction against resistance, pain on passive stretching of the affected muscle, and in severe cases, a palpable gap indicating rupture. MRI is the gold-standard imaging investigation when severity needs accurate assessment — it precisely grades the strain, identifies the location of the tear, quantifies the percentage of cross-sectional area involved, and detects associated haematoma. MRI findings guide return-to-sport decisions and surgical planning for Grade III tears. Ultrasound is a practical first-line imaging modality in sports clinics — less expensive and dynamic assessment is possible, though inferior to MRI for deep partial tears. X-ray excludes avulsion fractures (particularly at the ischial tuberosity in proximal hamstring injuries in adolescents).
Treatment Options
Immediate management: PRICE protocol — Protect (avoid aggravating activity), Rest (relative rest, not complete immobility), Ice (apply for 15-20 minutes every 2 hours for the first 48-72 hours to reduce swelling — wrap in cloth, do not apply directly to skin), Compression (bandage to reduce oedema), and Elevation. Analgesics: paracetamol for pain control; NSAIDs (ibuprofen, naproxen) reduce inflammation and pain — avoid in the first 48-72 hours after acute injury as they may impair the initial inflammatory response needed for healing. Physiotherapy is the cornerstone of rehabilitation: begins with gentle range-of-motion exercises progressing to strengthening, neuromuscular training, and sport-specific conditioning. Return-to-sport criteria: full pain-free range of motion, restoration of strength (symmetrical to unaffected side), and successful completion of sport-specific functional tests. Grade III complete ruptures of the proximal hamstring tendon or quadriceps tendon may require surgical repair.
Complications If Untreated
Most muscle strains heal completely without permanent consequences when managed appropriately. However, severe Grade III complete muscle tears that are not surgically repaired may cause persistent weakness and functional limitation. Repeated incomplete tears at the same site lead to fibrotic scar tissue, reducing muscle flexibility and elasticity — significantly increasing reinjury risk. Return to sport before full healing raises reinjury risk dramatically; hamstring re-injury rates are 12-25% after premature return. Myositis ossificans (abnormal bone formation within injured muscle) can develop after repeated or severe contusions — causing a palpable hard mass and restricted joint movement. Compartment syndrome after severe muscle injury is a surgical emergency requiring fasciotomy to preserve muscle viability and limb function.
Prevention & Injury Reduction
Evidence-based injury prevention programs significantly reduce muscle strain rates. The Nordic Hamstring Exercise program (eccentric hamstring strengthening) reduces hamstring strain recurrence by 50-70% — it should be incorporated into training programs for all athletes. Perform a structured warm-up before activity: 5-10 minutes of light aerobic exercise followed by dynamic stretching (leg swings, lunges, high knees) — static stretching before exercise does not prevent injury and may transiently reduce power output. Avoid sudden spikes in training volume or intensity — follow the 10% rule (increase no more than 10% per week). Address muscle strength imbalances through balanced training programs. Maintain adequate hydration and electrolyte balance during exercise. Return to sport only when fully recovered — premature return substantially increases re-injury risk.
When to Seek Medical Attention
See a doctor promptly if you hear or feel a significant 'pop' at the time of injury and cannot walk without pain, if there is visible deformity or a palpable gap in the muscle suggesting a complete rupture, if bruising is extensive and developing rapidly (suggesting a large haematoma), or if pain and swelling are not improving within 48-72 hours of initial PRICE treatment. A physiotherapist or sports medicine physician should be consulted for any Grade II or suspected Grade III strain to plan rehabilitation and assess return-to-sport readiness. Seek urgent care if the leg becomes very swollen, painful, and tight (compartment syndrome after severe strain) — this is a surgical emergency. Adolescents with suspected hamstring avulsion injury at the ischial tuberosity should be assessed with imaging before resuming sport.
