As caries progresses through enamel and dentine to reach the pulp, it causes pulpitis (pulpal inflammation — either reversible or irreversible), which if untreated progresses to pulpal necrosis, periapical abscess formation, and potential spread of infection to the face, neck, and airway (Ludwig's angina — a life-threatening emergency). Toothache is classified by its clinical features as: reversible pulpitis (sharp, short-duration pain triggered by hot/cold, resolving within seconds), irreversible pulpitis (spontaneous, prolonged, severe throbbing pain, thermal sensitivity lingering minutes), and periapical periodontitis (percussion tenderness, constant aching pain). Correctly characterising the type of toothache guides treatment planning.
Causes & Risk Factors
Dental caries: demineralisation of tooth enamel and dentine by acids produced by cariogenic bacteria (Streptococcus mutans, Lactobacillus) fermenting dietary sugars; progresses through enamel (often painless), dentine (dentinal hypersensitivity — brief sharp pain to cold), and into the pulp (pulpitis). Pulpitis: inflammation of the dental pulp from caries, trauma (cracked tooth, recent dental work), or exposed dentine. Reversible pulpitis — pain resolves within seconds of removing stimulus; irreversible pulpitis — spontaneous, lingering, or intense pain indicating irreversible pulpal damage requiring root canal treatment or extraction. Periapical abscess: infection at the tooth root apex from necrotic pulp — causes constant severe aching, swelling, and percussion tenderness. Cracked tooth syndrome: pain from chewing or biting on a specific tooth, often with cold sensitivity; common in teeth with large fillings or in bruxists. Periodontal (gum) disease: pain from periodontal abscess, acute necrotising ulcerative gingivitis (ANUG), or advanced periodontitis. Temporomandibular joint (TMJ) dysfunction: jaw pain radiating to teeth from TMJ inflammation or bruxism (tooth grinding). Non-odontogenic causes of facial pain mimicking toothache: trigeminal neuralgia (lancinating electric shock pain), atypical odontalgia (chronic neuropathic dental pain post-extraction), sinusitis (maxillary — upper molar pain), cardiac pain (MI may cause left jaw pain — always consider), and salivary gland disorders.
Symptoms & Signs
Reversible pulpitis: brief sharp pain (2-10 seconds) triggered by cold, sweet, or acidic stimuli — resolves immediately when the stimulus is removed. The tooth is vitality-tested as vital. Typically caused by early caries or exposed dentine. Irreversible pulpitis: spontaneous throbbing pain (often nocturnal — lying down increases pulpal pressure); thermal sensitivity lasting minutes after stimulus removal; pain may be severe and poorly localised (pulpal pain is poorly localised as the pulp has no proprioceptive fibres). Analgesics provide partial relief. Periapical abscess: constant, severe, localised aching pain; localised swelling (parulis — gum boil), facial swelling; extreme tenderness on percussion (tapping the tooth); pain on biting. The tooth tests non-vital (necrotic pulp). Fever and trismus (difficulty opening mouth) indicate spreading infection — urgent assessment required. Periapical periodontitis (acute): all symptoms of abscess without swelling — early infection at the apex. Cracked tooth: sharp pain on biting or releasing from biting on the specific tooth; cold sensitivity; difficult to diagnose. Dental abscess with facial cellulitis: rapidly spreading diffuse swelling of the face and neck, trismus, dysphagia (difficulty swallowing), and systemic illness (fever, malaise) — warning signs of potential airway compromise.
How It Is Diagnosed
Dental examination: history (character, duration, triggers, and location of pain); visual inspection (caries, fractures, swelling); percussion test (tapping with a dental mirror handle — extreme tenderness suggests periapical pathology); palpation (soft tissue swelling, lymphadenopathy); vitality testing: electric pulp test (EPT — passing a small electrical current through the tooth to test pulp viability) and thermal tests (cold — Endo-Ice spray or cold CO2 stick; heat — warm gutta-percha); periodontal probing (pocket depths, bleeding on probing — assesses periodontal disease contributing to the pain). Dental radiographs: periapical X-ray (individual tooth view) — identifies caries extent (approximal — between teeth), periapical radiolucency (periapical abscess or granuloma), bone loss (periodontal), root fractures, and root canal morphology. Bitewing X-rays: approximal caries, bone levels, and restorations. CBCT (cone beam CT): for complex cases — cracked teeth, atypical pain, and implant assessment. Referred pain sources to exclude: maxillary sinus tenderness and congestion (sinus X-ray or CT); cardiac history (ECG/troponin if cardiac-origin jaw pain is suspected); TMJ examination (clicking, range of motion, tenderness); cranial nerve examination for trigeminal neuralgia.
Treatment Options
Analgesics for immediate pain relief: ibuprofen 400mg three times daily with food (anti-inflammatory) combined with paracetamol 500-1000mg four times daily provides better pain relief than either alone for dental pain; clove oil (eugenol) provides temporary topical relief via TRPV1 desensitisation. Reversible pulpitis: remove the caries and place a filling or dental restoration — pain resolves once the irritant is removed and the pulp recovers. Sensitive dentine (no caries): desensitising toothpaste (stannous fluoride or potassium nitrate), fluoride varnish application, and dentine bonding agents. Irreversible pulpitis: root canal treatment (endodontics) — removal of the necrotic or inflamed pulp tissue, cleaning and shaping of the root canal system (using nickel-titanium rotary files and sodium hypochlorite irrigation), obturation with gutta-percha and root canal sealer, and restoration of the crown (typically with a post-core and crown for posterior teeth). Success rate approximately 85-90% over 10 years. Alternatively, extraction of the tooth — followed by prosthetic replacement (implant, bridge, or denture). Periapical abscess: drainage is the primary treatment — incision and drainage (I+D) if fluctuant soft tissue abscess present; tooth extraction or root canal treatment removes the source of infection. Antibiotics (amoxicillin 500mg three times daily for 5 days, or metronidazole 400mg three times daily if anaerobes suspected — penicillin allergy: clindamycin 300mg four times daily): indicated only if there is systemic spread (fever, trismus, facial swelling, lymphadenopathy) — antibiotics do not penetrate into the avascular abscess cavity and must not replace drainage. Ludwig's angina (bilateral submandibular, sublingual, and submental space infection — floor of mouth boardlike induration, drooling, respiratory compromise): airway management (intubation or tracheostomy), IV antibiotics (co-amoxiclav + metronidazole), surgical decompression — ICU management. Periodontal abscess: drainage, debridement, and irrigation. Cracked tooth: depends on extent — superficial crack: bonding/crown; crack extending to pulp: root canal plus crown; vertical root fracture: extraction.
Complications
The most serious complication of untreated dental infection is spread beyond the tooth to adjacent facial spaces — potentially becoming life-threatening. Dental abscess can spread to the buccal, masseteric, and submandibular spaces causing progressive facial cellulitis with trismus (difficulty opening the mouth), dysphagia, and systemic sepsis. Ludwig's angina — bilateral infection of the submandibular, sublingual, and submental spaces — is a surgical emergency: the floor of the mouth becomes board-like, the tongue is displaced upward, and progressive airway compromise threatens life; mortality remains approximately 8% even with appropriate treatment. Cavernous sinus thrombosis, a rare intracranial complication via the pterygoid plexus, causes fever, proptosis, and meningism with significant mortality. In diabetic or immunocompromised patients, dental infections spread more rapidly and are significantly more likely to become life-threatening. Osteomyelitis of the jaw occurs when dental infection invades the cortical bone, causing severe pain, swelling, and bone necrosis requiring surgical debridement and prolonged antibiotic therapy. In children, chronic periapical abscesses overlying developing permanent tooth buds can cause permanent enamel hypoplasia in the successor tooth. Untreated pulpitis progressing to pulpal necrosis permanently destroys the tooth's vitality, invariably resulting in tooth loss without timely root canal treatment.
Prevention & Lifestyle Management
The vast majority of toothache is preventable through effective oral hygiene and dietary management. Toothbrushing: brush twice daily with fluoride toothpaste (at least 1,450 ppm fluoride — standard adult UK toothpaste) for 2 minutes; brush last thing at night and on one other occasion; do not rinse after brushing (spitting out excess toothpaste allows fluoride to remain on teeth). Interdental cleaning: use floss or interdental brushes once daily — dental caries and periodontal disease originate between teeth where brushing cannot reach. Fluoride: fluoride toothpaste is the single most evidence-based dental prevention measure — it incorporates into tooth enamel (as fluorapatite), making it more acid-resistant and promoting remineralisation. When to See a Doctor
See a dentist as soon as possible for any persistent toothache lasting more than 1-2 days, visible swelling in the face or gum, sensitivity to hot that lingers for minutes, or a tooth broken by caries.
