Overview: Wisdom Tooth Problems
Wisdom teeth (third molars) are the last permanent teeth to erupt, typically appearing between the ages of 17 and 25 years — historically called 'wisdom teeth' because they emerge in early adulthood. In modern humans, the jawbone is frequently too small to accommodate these four teeth (one in each quadrant), leading to impaction — partial or complete failure to erupt into a functional position. Up to 73% of adults have at least one impacted wisdom tooth. The complications of wisdom teeth are among the most common dental problems managed in primary and secondary dental care: pericoronitis (infection of the gum tissue overlying a partially erupted wisdom tooth), dental caries (decay) of the wisdom tooth or adjacent second molar, cyst formation, resorption of adjacent teeth, and crowding. Surgical extraction is the definitive treatment for problematic wisdom teeth and is one of the most commonly performed oral surgical procedures globally.
Causes & Risk Factors
Impaction arises when there is insufficient space in the dental arch for the wisdom tooth to fully erupt into a normal position. Types of impaction (based on tooth angulation on OPG X-ray): mesioangular (most common — 43%; crown angled towards the second molar); horizontal; distoangular; vertical (tooth aligned correctly but no space to erupt). Depth classifications: soft tissue impaction (crown through bone but below gingiva); partial bony impaction (partially covered by bone); full bony impaction (completely covered by bone). Risk factors for complications: partial eruption with overlying operculum (gum flap) — creates a space that traps food debris and bacteria, predisposing to pericoronitis; deep impaction increases cyst risk (dentigerous cyst forms from follicular tissue around the crown); contact with the second molar root creates risk of caries and root resorption. Genetic factors (jaw size relative to tooth size), ethnicity, and dietary changes in modern populations (reduced jaw development with processed food diets) all contribute. The inferior alveolar nerve (IAN) runs close to lower wisdom tooth roots — proximity is assessed on OPG or CBCT (cone beam CT) before extraction.
Symptoms & Signs
Pericoronitis (infection of the pericoronal flap): the most common complication — pain, swelling, and tenderness in the posterior lower jaw; purulent discharge from beneath the gum flap; trismus (difficulty opening the mouth, caused by inflammation spreading to the masseter muscle); halitosis; lymphadenopathy (swollen submandibular and cervical lymph nodes); fever and malaise in severe cases. Dental pain: deep throbbing ache in the posterior jaw, often radiating to the ear, throat, or temple — difficult to distinguish from earache or temporomandibular joint pain. Caries of wisdom tooth or adjacent second molar: sensitivity to hot/cold, toothache. Facial swelling: localised or spreading cellulitis — if spreading rapidly to the submandibular space, floor of mouth, or neck (Ludwig's angina), this is a life-threatening emergency requiring immediate hospital admission. Dentigerous cyst: often asymptomatic and discovered on routine OPG; may cause jaw expansion or root resorption of adjacent teeth. Crowding of lower anterior teeth: attributed to wisdom tooth pressure in some (though the evidence for this causal link is debated).
How It Is Diagnosed
Clinical examination: inspection of the oral cavity for partially erupted wisdom teeth; assessment of the pericoronal flap for signs of infection (erythema, swelling, pus, tenderness); mouth opening measurement (trismus); lymph node examination; recording of facial swelling. Orthopantomogram (OPG — dental panoramic tomograph): the standard dental X-ray for wisdom tooth assessment; shows the position, angulation, and depth of impaction; relationship of roots to the inferior alveolar canal; presence of cyst or root resorption of adjacent teeth. NICE (2020) recommends OPG as the first-line investigation for wisdom teeth. Cone beam CT (CBCT): used when OPG shows close root-to-nerve proximity (increased risk of IAN injury during extraction) — provides 3D assessment of root-nerve relationship and guides surgical planning. Periapical X-ray: supplementary to OPG for detailed assessment of individual tooth roots and periapical pathology. Pus swab: if purulent discharge present — culture and sensitivity to guide antibiotic choice in recurrent or severe pericoronitis.
Treatment Options
Active pericoronitis: irrigation (saline or chlorhexidine 0.2%) of the pericoronal flap using a syringe and blunt needle to remove food debris and bacteria — most effective immediate treatment; antibiotics (metronidazole 400 mg three times daily for 5 days, often combined with amoxicillin 500 mg three times daily for mixed oral infections) — prescribed only when systemic signs are present (fever, lymphadenopathy, spreading swelling); analgesics (ibuprofen 400 mg TDS + paracetamol 1 g QDS for optimum pain relief). Definitive management — extraction: NICE criteria for wisdom tooth removal (based on evidence of pathology): pericoronitis (two or more episodes or one severe episode), unrestorable caries or caries in the adjacent second molar attributable to the wisdom tooth, root resorption of the adjacent second molar, dentigerous cyst, evidence of external root resorption, internal resorption. Prophylactic extraction (removing asymptomatic wisdom teeth without pathology) is NOT recommended by NICE in the NHS. Surgical extraction technique: local anaesthesia (inferior alveolar nerve block for lower teeth; palatal infiltration for upper); mucoperiosteal flap raised; bone removal by surgical bur; tooth sectioning if needed; wound closed with absorbable sutures (Vicryl 3/0). Post-operative care: chlorhexidine rinses from day 2; soft diet; avoid smoking (significantly increases dry socket risk); avoid vigorous rinsing for 24 hours. Dry socket (alveolar osteitis — occurs 2–5% of extractions, up to 38% for lower wisdom teeth): the most common post-extraction complication — delayed healing with loss of the blood clot, exposed bone, severe localised pain radiating to the ear from 2–4 days post-extraction; treatment: gentle saline irrigation, Alvogyl (bismuth iodoform and paraffin paste) dressings to the socket. General anaesthesia (GA) or sedation: for anxious patients, multiple wisdom teeth, deeply impacted teeth, or surgical complexity.
Complications
The most serious complication of wisdom tooth infection is deep fascial space infection progressing to Ludwig's angina — bilateral infection of the submandibular, sublingual, and submental spaces producing floor-of-mouth induration, tongue elevation, dysphonia, dysphagia, and progressive airway compromise; this carries significant mortality without urgent surgical decompression, airway management, and IV antibiotics (co-amoxiclav plus metronidazole). Adjacent second molar damage is a clinically important complication: impacted wisdom teeth cause caries in the adjacent second molar through plaque accumulation in the inaccessible contact area (occurring in up to 25% of cases) and root resorption of the second molar (1-5% of mesioangular impaction cases), potentially resulting in loss of the second molar. Dentigerous cyst formation around the crown of an unerupted tooth can expand causing jaw bone destruction, displacement of adjacent teeth, and root resorption; rare malignant transformation (less than 0.3%) to ameloblastoma or squamous cell carcinoma is described. Dry socket (alveolar osteitis) following extraction occurs in 2-5% of all extractions and up to 38% of lower wisdom tooth removals — causing severe throbbing pain from 2-4 days post-operatively. Inferior alveolar nerve injury during surgical extraction may result in temporary (0.5-2%) or permanent (0.1-0.5%) numbness, tingling, or altered sensation of the lower lip, chin, and teeth — significantly affecting quality of life and requiring specialist neurological follow-up.
Prevention & Lifestyle Management
Optimal oral hygiene around partially erupted wisdom teeth reduces the risk of pericoronitis and caries: use a small-headed toothbrush to clean behind the second molar; water flossers or interdental brushes can dislodge debris from beneath the gum flap; regular chlorhexidine 0.2% mouthwash rinses reduce bacterial load. Regular dental check-ups (at least annually): allow early detection of impaction, caries, or cyst formation — conditions identified and managed early involve less complex treatment. Smoking cessation: smoking significantly increases the risk of pericoronitis, impairs wound healing post-extraction, and dramatically increases dry socket risk (smokers have 3–4 times the dry socket rate of non-smokers). Avoid alcohol and NSAIDS in the immediate post-extraction period if advised. Early review for wisdom teeth in young adults (18–25 years) is advisable — impacted wisdom teeth treated in younger patients carry lower surgical risk as roots are less fully formed and healing is faster.
When to Seek Medical Help
See your dentist promptly if: you experience pain, swelling, or difficulty opening your mouth in the area of the back teeth; you notice pus or a foul taste in the back of your mouth; or you have recurrent episodes of wisdom tooth pain. Attend an emergency dental clinic or NHS 111 (UK) / urgent care on the same day for: severe facial swelling extending below the jaw or towards the neck (Ludwig's angina — a life-threatening infection requiring emergency treatment); difficulty swallowing, breathing, or opening the mouth beyond 2 cm; high fever with wisdom tooth pain. After extraction, seek urgent review for: severe pain developing 3–4 days after extraction (dry socket); signs of infection (increasing swelling, fever, pus, bad taste); or uncontrolled bleeding from the socket not resolving with sustained biting on gauze for 30 minutes.
