Causes & Risk Factors

Impaction arises when there is insufficient space in the dental arch for the wisdom tooth to fully erupt into a normal position. Types of impaction (based on tooth angulation on OPG X-ray): mesioangular (most common — 43%; crown angled towards the second molar); horizontal; distoangular; vertical (tooth aligned correctly but no space to erupt). Depth classifications: soft tissue impaction (crown through bone but below gingiva); partial bony impaction (partially covered by bone); full bony impaction (completely covered by bone). Risk factors for complications: partial eruption with overlying operculum (gum flap) — creates a space that traps food debris and bacteria, predisposing to pericoronitis; deep impaction increases cyst risk (dentigerous cyst forms from follicular tissue around the crown); contact with the second molar root creates risk of caries and root resorption. Genetic factors (jaw size relative to tooth size), ethnicity, and dietary changes in modern populations (reduced jaw development with processed food diets) all contribute. Symptoms & Signs

How It Is Diagnosed

Definitive management — extraction: NICE criteria for wisdom tooth removal (based on evidence of pathology): pericoronitis (two or more episodes or one severe episode), unrestorable caries or caries in the adjacent second molar attributable to the wisdom tooth, root resorption of the adjacent second molar, dentigerous cyst, evidence of external root resorption, internal resorption. Prophylactic extraction (removing asymptomatic wisdom teeth without pathology) is NOT recommended by NICE in the NHS. Surgical extraction technique: local anaesthesia (inferior alveolar nerve block for lower teeth; palatal infiltration for upper); mucoperiosteal flap raised; bone removal by surgical bur; tooth sectioning if needed; wound closed with absorbable sutures (Vicryl 3/0). Complications

The most serious complication of wisdom tooth infection is deep fascial space infection progressing to Ludwig's angina — bilateral infection of the submandibular, sublingual, and submental spaces producing floor-of-mouth induration, tongue elevation, dysphonia, dysphagia, and progressive airway compromise; this carries significant mortality without urgent surgical decompression, airway management, and IV antibiotics (co-amoxiclav plus metronidazole). Adjacent second molar damage is a clinically important complication: impacted wisdom teeth cause caries in the adjacent second molar through plaque accumulation in the inaccessible contact area (occurring in up to 25% of cases) and root resorption of the second molar (1-5% of mesioangular impaction cases), potentially resulting in loss of the second molar. Dentigerous cyst formation around the crown of an unerupted tooth can expand causing jaw bone destruction, displacement of adjacent teeth, and root resorption; rare malignant transformation (less than 0.3%) to ameloblastoma or squamous cell carcinoma is described. Dry socket (alveolar osteitis) following extraction occurs in 2-5% of all extractions and up to 38% of lower wisdom tooth removals — causing severe throbbing pain from 2-4 days post-operatively. Inferior alveolar nerve injury during surgical extraction may result in temporary (0.5-2%) or permanent (0.1-0.5%) numbness, tingling, or altered sensation of the lower lip, chin, and teeth — significantly affecting quality of life and requiring specialist neurological follow-up.

Prevention & Lifestyle Management

Optimal oral hygiene around partially erupted wisdom teeth reduces the risk of pericoronitis and caries: use a small-headed toothbrush to clean behind the second molar; water flossers or interdental brushes can dislodge debris from beneath the gum flap; regular chlorhexidine 0.2% mouthwash rinses reduce bacterial load. Regular dental check-ups (at least annually): allow early detection of impaction, caries, or cyst formation — conditions identified and managed early involve less complex treatment. Smoking cessation: smoking significantly increases the risk of pericoronitis, impairs wound healing post-extraction, and dramatically increases dry socket risk (smokers have 3–4 times the dry socket rate of non-smokers). Avoid alcohol and NSAIDS in the immediate post-extraction period if advised. Pericoronitis recurs because the root cause — the partially erupted tooth with an overlying flap — has not been removed. Each episode may resolve with irrigation and antibiotics, but recurrence is likely unless the wisdom tooth is extracted. After two or more episodes of pericoronitis, or one severe episode with systemic features (fever, lymphadenopathy, trismus), extraction is strongly recommended by NICE.
