Overview: Psoriasis
Psoriasis is a chronic, relapsing, immune-mediated inflammatory skin disease characterised by well-demarcated, erythematous plaques covered with silvery-white scales. It affects approximately 2-3% of the global population — 125 million people worldwide. While predominantly a skin condition, psoriasis is a systemic inflammatory disease with significant comorbidities: psoriatic arthritis (PsA) in 30% of patients; increased cardiovascular disease risk (psoriasis is an independent cardiovascular risk factor); metabolic syndrome; non-alcoholic fatty liver disease; inflammatory bowel disease; uveitis; and a substantial psychiatric burden (depression and anxiety in 20-30%). Psoriasis follows a chronic relapsing-remitting course — with periods of flare and relative remission. Disease severity is categorised as mild (less than 3% of body surface area), moderate (3-10% BSA), and severe (above 10% BSA, or any extent with significant impact on quality of life or inadequate response to topical therapies). Modern biologic therapy can achieve complete skin clearance (PASI 100 — Psoriasis Area and Severity Index score reduction of 100%) in over 40% of patients.
Causes & Risk Factors
Psoriasis is caused by dysregulation of the innate and adaptive immune system, resulting in inappropriate activation of T-helper 17 (Th17) cells and Th1 cells in the skin. Key cytokines: IL-17A, IL-17F, IL-22, and TNF-alpha drive keratinocyte hyperproliferation (cell turnover is 5-7 times faster than normal — the epidermis renews in 3-5 days instead of 28 days), impaired differentiation, and the characteristic skin changes. The IL-23/IL-17 axis is now recognised as the central pathogenic pathway, forming the basis for the most effective biologic treatments. Genetic factors: psoriasis has strong genetic heritability — HLA-Cw6 is the strongest genetic risk factor (present in 60% of early-onset psoriasis patients vs 15% of controls); genome-wide association studies have identified over 80 susceptibility loci, including genes in the IL-12/IL-23 and IL-17 signalling pathways. Risk factors and triggers: family history (first-degree relative increases risk 4-fold); streptococcal throat infection (triggers guttate psoriasis); stress (psychological and physical stress — major trigger for flare); skin trauma (Koebner phenomenon — new lesions at sites of skin injury); medications (beta-blockers, lithium, antimalarials, NSAIDs, ACE inhibitors, withdrawal of systemic corticosteroids); smoking (associated with palmoplantar pustulosis); alcohol excess; and obesity.
Symptoms & Signs
Chronic plaque psoriasis (psoriasis vulgaris — the most common type, 80-90% of cases): well-demarcated, salmon-pink to red plaques with overlying silvery-white (micaceous) scale; typically symmetrical; predilection sites — scalp, elbows (extensor surfaces), knees, lower back, and umbilicus. Characteristic signs: Auspitz sign (pinpoint bleeding on removal of scale — from disrupted dilated capillaries); Koebner (isomorphic) phenomenon (new plaques at sites of skin trauma, surgery, or pressure — pathognomonic of psoriasis). Guttate psoriasis: numerous small (1-2 cm), drop-shaped plaques — typically triggered by streptococcal throat infection in adolescents and young adults; often clears with antibiotic treatment and topical therapy. Pustular psoriasis: generalised (von Zumbusch — acute, severe, with sheets of sterile pustules, fever, and systemic illness — a dermatological emergency); palmoplantar (localised sterile pustules on palms and soles — very difficult to treat). Erythrodermic psoriasis: greater than 90% BSA involvement with diffuse erythema — causes hypothermia, high-output cardiac failure, sepsis risk — a dermatological emergency. Nail psoriasis (present in 50-80% of patients): pitting (multiple small depressions — most specific sign), onycholysis (separation of nail plate), oil-drop sign (yellow-brown discolouration under the nail), and subungual hyperkeratosis. Inverse (flexural) psoriasis: smooth red plaques in skin folds (groin, axillae, submammary) without the typical scale (friction removes scale in flexures). Psoriatic arthritis: joint pain, stiffness, and swelling — peripheral arthritis, dactylitis ('sausage digits'), enthesitis, axial disease.
How It Is Diagnosed
Psoriasis is primarily a clinical diagnosis — based on the characteristic morphology and distribution of skin lesions. Skin biopsy: not required for typical presentations but performed in atypical cases — shows acanthosis (epidermal thickening), parakeratosis (nucleated keratinocytes in the stratum corneum), Munro microabscesses (neutrophilic infiltrate in stratum corneum), dilated and tortuous blood vessels in the papillary dermis (forming the Auspitz sign), and a predominantly CD4+ T-cell dermal infiltrate. Disease severity assessment: Psoriasis Area and Severity Index (PASI) — the most widely used composite score (0-72) assessing erythema, induration, scaling, and BSA; PASI 75 (75% improvement from baseline) is the standard regulatory endpoint for biologic trials. DLQI (Dermatology Life Quality Index): 10-item patient-reported outcome (0-30) — essential for treatment eligibility (DLQI above 10 typically required for biologic therapy). Nail Psoriasis Severity Index (NAPSI). Psoriatic arthritis screening: PEST (Psoriasis Epidemiology Screening Tool), PASE, or clinical review — all psoriasis patients should be screened for joint symptoms annually; CASPAR criteria used for formal PsA diagnosis. Blood tests: no specific diagnostic test; CRP and ESR mildly elevated in active disease; screen for comorbidities — fasting glucose, lipids, BP; and before systemic therapy: LFTs, FBC, renal function.
Treatment Options
Treatment is stepwise, based on severity and impact on quality of life. Mild psoriasis (topical therapy): emollients (essential — restore skin barrier, reduce scaling, enhance topical drug efficacy); topical corticosteroids (first-line for acute flares — betamethasone valerate 0.1% or clobetasol 0.05% for scalp; used short-term due to atrophy risk); vitamin D analogues (calcipotriol, calcitriol — inhibit keratinocyte proliferation and have anti-inflammatory effects; combined with betamethasone as Enstilar/Dovobet foam — superior to either alone and first-line in UK NICE guidance); coal tar preparations (older but effective, cosmetically less acceptable); dithranol (highly effective for thick plaques); topical calcineurin inhibitors (tacrolimus, pimecrolimus — for sensitive areas: face, flexures). Moderate-severe psoriasis (conventional systemic therapy): methotrexate (7.5-25 mg weekly oral or SC — the most commonly used systemic; highly effective, cheap; requires FBC and LFT monitoring, folic acid supplementation; teratogenic — reliable contraception essential); ciclosporin (2-5 mg/kg/day — rapid onset, useful for acute severe flares; limited to 12-24 months due to nephrotoxicity and hypertension); acitretin (oral retinoid — particularly for pustular and erythrodermic psoriasis; highly teratogenic — required pregnancy prevention programme). Phototherapy (PUVA or narrowband UVB): effective outpatient treatment for extensive plaque psoriasis. Biologic therapy (moderate-severe psoriasis inadequately responding to conventional therapy): IL-17A inhibitors — secukinumab (Cosentyx, 300 mg SC monthly after loading) and ixekizumab (Taltz) — fastest onset, very high efficacy (PASI 90 in 60-70%); IL-17A/F inhibitor — bimekizumab (Bimzelx) — superior to secukinumab and adalimumab in head-to-head trials; IL-23 inhibitors — risankizumab (Skyrizi, quarterly SC injections after induction), guselkumab (Tremfya), tildrakizumab — durable efficacy, best safety profile; IL-12/23 inhibitor — ustekinumab (Stelara, quarterly SC); anti-TNF — adalimumab (Humira), etanercept, certolizumab pegol — earlier biologics with good efficacy, particularly for PsA and psoriasis. PDE4 inhibitor — apremilast (Otezla, oral, 30 mg BD) — less effective than biologics but oral, no immunosuppression monitoring needed. JAK inhibitors — deucravacitinib (Sotyktu, oral, once daily) — approved for moderate-severe plaque psoriasis, superior to apremilast.
Complications
Psoriasis is a systemic inflammatory disease with complications that extend significantly beyond skin involvement. Psoriatic arthritis (PsA): affects 25-35% of psoriasis patients, typically developing 10-15 years after skin onset; causes joint inflammation, pain, stiffness, and — when untreated — progressive joint destruction leading to deformity and disability; five clinical patterns include oligoarthritis, polyarthritis (rheumatoid-like), spondylitis, DIP joint predominant, and arthritis mutilans (most destructive). Cardiovascular disease: psoriasis — particularly severe psoriasis — is an independent cardiovascular risk factor; patients with severe psoriasis have a 50% increased risk of myocardial infarction and a 30% increased stroke risk; chronic systemic inflammation accelerates atherosclerosis. Major adverse cardiovascular events are the leading cause of premature mortality in psoriasis. Metabolic syndrome: insulin resistance, obesity, hypertension, and dyslipidaemia are significantly more prevalent in psoriasis patients; independent of BMI. Inflammatory bowel disease: Crohn's disease (not ulcerative colitis) shares genetic and immunological pathways with psoriasis — prevalence 3-fold higher in psoriasis. Uveitis: anterior uveitis occurs in psoriasis with spondyloarthropathy — risks permanent visual impairment if untreated. Psychological complications: depression (prevalence 20-30%) and anxiety are significantly more prevalent than in matched controls; social stigma from visible lesions, social withdrawal, and impaired sexual function are common.
Prevention & Lifestyle Management
While psoriasis cannot be prevented (given its genetic component), triggers can be identified and avoided to reduce flare frequency and severity. Streptococcal infection (particularly in guttate psoriasis): treat throat infections promptly with antibiotics; tonsillectomy may be considered in recurrent streptococcal-triggered guttate psoriasis. Stress management: psychological stress is one of the most commonly reported triggers; mindfulness, CBT, yoga, and structured relaxation have evidence for reducing psoriasis severity alongside conventional treatment. Smoking cessation: strongly associated with palmoplantar pustulosis — cessation can lead to significant improvement; also reduces cardiovascular risk (elevated in psoriasis). Alcohol reduction: heavy alcohol use worsens psoriasis severity, reduces response to treatment, and potentiates hepatotoxicity from methotrexate — patients on methotrexate should be abstinent or minimal alcohol consumers. Weight loss: obesity worsens psoriasis severity — BMI above 30 reduces biologic efficacy; weight loss of greater than 5% improves treatment response. Skin care: use fragrance-free emollients daily; avoid harsh soaps and shower gels; minimise trauma to skin (shaving carefully, avoiding scratching); wear soft cotton clothing.
When to See a Doctor
See a GP for initial assessment if you develop: new red, scaly plaques on elbows, knees, scalp, or lower back; nail changes (pitting, brown discolouration, or nail separation); or widespread skin redness with scaling. Urgent GP or emergency dermatology review is needed for: sudden onset of widespread pustules on red skin with fever (generalised pustular psoriasis — a dermatological emergency); or rapidly spreading skin erythema affecting most of the body (erythrodermic psoriasis — also an emergency, as it causes temperature dysregulation and cardiovascular compromise). Request referral to a dermatologist for: psoriasis significantly impacting quality of life (sleep disruption, visible areas, joint symptoms); psoriasis involving hands, feet, face, or genitals regardless of extent; any joint pain, stiffness, or swelling in a patient with psoriasis (possible psoriatic arthritis — early treatment prevents joint damage); or failure to respond to two topical treatments. All patients on systemic therapy should attend regular monitoring appointments.
